HLA--Bw54 (Bw22-J, J-1) antigen in juvenile onset diabetes mellitus in Japan.

HLA--B8 and/or Bw15, associated with juvenile onset insulin-dependent diabetes mellitus (JDM) in Caucasians, have a very low frequency in the Japanese population. Thus, we were interested in investigating the association between JDM and HLA antigen in a Japanese population. Eighty-nine patients with JDM and 128 unrelated random controls were HLA-typed by the microlymphocytotoxicity test. The data revealed a significant, positive association between this type of diabetes mellitus and HLA--Bw54 (Bw22-J, J-1) antigen (Japanese-specific split antigen of HLA--Bw22).

Association of human leukocyte antigen (HLA)-B5 and -BW22-J with hepatic cirrhosis of apparently unknown etiology.

The majority of patients with hepatic cirrhosis in Japan do not have a history of acute hepatitis or alcoholism. Twenty-nine patients with hepatic cirrhosis which were thought as random samples from cirrhotic patients without a history of acute hepatitis or alcoholism were Human Leukocyte Antigen (HLA) typed. Association of HLA-B5 and -BW22-J with hepatic cirrhosis of apparently unknown etiology was found in p less than 0.003, and p less than 0.01, respectively (128 controls). A predisposition linked to HLA-B5 or -BW22-J related gene or genes seems to play a role in the development of hepatic cirrhosis, at least in a substantial part of patients with hepatic cirrhosis of apparently unknown etiology in Japan.

Direct duodenal invasion of hepatocellular carcinoma. Intestinal hemorrhage treated by transcatheter arterial embolization.

This report describes a case of direct duodenal invasion of hepatocellular carcinoma with massive intermittent gastrointestinal (GI) bleeding. Progressive anemia was intractable by supportive therapy alone, and repeated blood transfusion was necessary. Transcatheter arterial embolization was finally carried out, which dramatically reduced the amount of transfusion. Owing to severe blood loss, patients with GI tract involvement generally have a poor prognosis.

A possible null phenotype in the Cromer blood group complex.

The red blood cells of a Japanese man failed to react with anti-Cra sera and with three Cromer-related sera, B.P., G.T., and K.T.O. His serum reacted with all red blood cells tested except his own. This phenotype, which may represent a null phenotype in the Cromer complex, supports the suggestion that the phenotypes of B.P., G.T., and K.T.O. are Cromer related.

A potent cold autoagglutinin that recognizes type 2H determinant on red cells.

An example of an IgM (lambda) cold autoagglutinin against a Type 2H determinant of the ABO blood group system is described. The direct antiglobulin test was positive due to C3d on the patient's red cells. The antibody in the patient's serum was active at 37 degrees C, and the degree of agglutination was almost the same at 37 and 4 degrees C. It agglutinated group O red cells preferentially, but not Bombay (Oh) red cells. The specificity was determined by adsorption with synthetic oligosaccharide immunoadsorbent.

The effect of anterior cruciate ligament graft elongation at the time of implantation on the biomechanical behavior of the graft and knee.

This investigation determined the effect that anterior cruciate ligament graft elongation at the time of surgical reconstruction has on the long-term biomechanical behavior of the graft and knee joint. We chose the canine model for anterior cruciate ligament reconstruction, using the medial third of the patellar tendon with attached proximal bone block. Elongation of the graft was measured immediately after graft fixation during passive knee flexion using the Hall effect transducer. The dogs were divided into either Group 1 (graft elongation behavior within the 95% confidence limits of the normal anterior cruciate ligament) or Group 2 (graft elongation behavior more than the 95% confidence limits of the normal anterior cruciate ligament). All of the dogs were sacrificed 18 months postoperatively, and we evaluated anteroposterior load displacement (i.e., anteroposterior laxity) of the knee and the structural properties of the graft. The anteroposterior laxity behavior of the reconstructed knees in Group 2 was significantly more than that of Group 1. Group 2 had significantly less linear stiffness of the graft than Group 1. There was no difference in the ultimate failure load and absorbed energy at failure values of the grafts between Groups 1 and 2. The findings from this investigation indicate that the graft elongation behavior at the time of reconstruction is a critical factor that influences the long-term success of anterior cruciate ligament reconstruction.

The effect of cyclic displacement on the biomechanical characteristics of anterior cruciate ligament reconstructions.

This study was conducted to clarify the influence of cyclic displacement on the structural properties of four types of femur-graft-tibia complexes used to reconstruct the anterior cruciate ligament. Forty hindlimbs from pigs were used. In two groups, bone-patellar tendon-bone grafts were secured with interference screws (group A) or the suture-post technique (group B). In two groups, multistrand flexor tendons were fixed using the tape-staple technique (group C) or the sutures-tied-over-a-button technique (group D). In each group, five femur-graft-tibia complexes underwent tensile failure tests without cyclic displacement. The other five complexes underwent 5000 cycles of cyclic elongation for 2 mm, and then underwent the tensile failure tests. The initial stiffness significantly decreased after cyclic displacement in each group, although there were no significant differences in the linear stiffness and the ultimate failure load between the tests with and without cyclic displacement. These findings suggest that 5000 cycles of repetitive elongation of the femur-graft-tibia complex by 2 mm does not jeopardize the graft fixed with the procedures used in this study, despite a slight but significant increase of an anterior-posterior laxity of the knee.
