Ruptured retroperitoneal mucinous cystadenocarcinoma with synchronous gastric carcinoma and a long postoperative survival: case report.

We describe an 86-year-old woman with a long survival following surgey for a massive retroperitoneal mucinous cystadenocarcinoma and a synchronous gastric carcinoma. Computed tomography showed a huge tumor with septation and calcification. Upper gastrointestinal radiography showed the additional gastric lesion. At operation, the 23 x 20 x 12-cm retroperitoneal tumor had ruptured. Tumor resection and distal gastrectomy including regional lymph nodes were performed. Mucinous peritoneal implants were removed as completely as possible. Histologically, the mucinous tumor showed limited invasion, whereas the poorly differentiated gastric adenocarcinoma showed no serosal invasion. Among 18 retroperitoneal mucinous cystadenocarcinomas reported in the English literature since 1965, only ours was associated with gastric carcinoma. Despite peritoneal implants, our patient has survived for 6 years without clinical recurrence. As at other sites, retroperitoneal mucinous cystadenocarcinoma often grows slowly. Total removal, even after peritoneal dissemination, can result in long survival.

Water extract of Helicobacter pylori stimulates interleukin-8 secretion by a human gastric epithelial cell line (JR-St) through protein tyrosine phosphorylation.

BACKGROUND: Infection by Helicobacter pylori induces cytokine production in gastric mucosal cells. Production of interleukin-8 (IL-8) is known to be markedly increased and is believed to play an important role in gastric mucosal inflammation. The aim of this study was to elucidate the effects of soluble factors of H. pylori on IL-8 production in a gastric epithelial cell line, JR-St. METHODS: JR-St cells were cocultured with a H. pylori water extract, live H. pylori or culture medium supernatant for 24 h, then the IL-8 secreted into the culture medium was assayed. The effects of three different inhibitors; (i) an inhibitor of protein kinase C (PKC); (ii) an inhibitor of PKC and protein kinase A (PKA); and (iii) an inhibitor of protein tyrosine kinase (PTK) were also compared. Specific induction of IL-8 mRNA was also examined. RESULTS: Water extract of H. pylori increased IL-8 secretion 7.72-fold, more than the control. The increase was concentration dependent. Live bacteria, supernatant and water extract significantly stimulated IL-8 secretion. Addition of live bacteria increased IL-8 secretion most strongly, while the effect of water extract was small (22% that of live bacteria). Secretion was not inhibited by the PKC inhibitor staurosporine or the inhibitors of PKA and PKC H7. However, secretion was significantly reduced by the PTK inhibitor herbimycin in a dose-dependent manner. Furthermore, 24 h exposure to water extract increased IL-8 mRNA expression, suggesting water extract increased production of IL-8. CONCLUSIONS: Some soluble factors of H. pylori can stimulate IL-8 production by JR-St cells. Stimulation was not dependent on PKA or PKC but was, at least partially, dependent on protein tyrosine phosphorylation. This suggests that soluble factors of H. pylori can play an important role in mediating the inflammatory response of H. pylori gastritis.

[The role and significance of acid suppressive drugs in the eradication of H. pylori].

Recently a new triple therapy with PPI and two antimicrobials is widely accepted instead of classical triple therapy. PPI has direct and indirect effects on H. pylori. Practically a single use of PPI is almost noneffective for eradication of H. pylori. In the combination therapy PPI is supposed to contribute to successful eradication through the inhibition of acid secretion. Because a raise of pH in stomach protects antimicrobials from the degradation by acidic environment. The decrease in the volume of gastric juice also contributes to concentrate antibiotics. Recently interaction between PPI and clarithromycin on metabolic enzyme in liver was reported that explains the synergistic effects of these drugs. The heterogeneity of genotype of metabolic enzyme was also elucidated which might be responsible for the difference in the effect of PPI between the patients. To accomplish successful eradication, full inhibition of acid secretion has to be done. To consider the interactions between PPI and antimicrobials and the heterogeneity of genotype of metabolic enzyme may improve eradication therapy of H. pylori.

Multiple myeloma mimicking bone metastasis from breast cancer: report of a case.

A 72-year-old woman with a history of early breast cancer suffered a fracture of the eighth thoracic vertebra resulting in paraplegia. Magnetic resonance imaging (MRI) showed spinal cord compression by a tumor between the ninth and tenth thoracic vertebrae. Local radiotherapy was begun under the diagnosis of metastatic breast cancer, but bone marrow aspiration and biopsy subsequently revealed plasma-cell proliferation rather than adenocarcinoma. This case report serves to demonstrate that clinicians should consider multiple myeloma as a cause of lytic bone lesions without extraskeletal metastases even in patients with a history of breast cancer.

[Bleeding in chronic refractory peptic ulcer].

The authors reviewed the clinical problems of the bleeding in refractory ulcers. Incidence of refractory and easily-relapsing ulcers are about 40% of all chronic ulcers and their bleeding rate seemed to be more than 10% of them. We discussed the relationship between bleeding and risk factors such as age, NSAID use, H. pylori infection, etc. The consideration to them is very important in the present time, being difficult to predict ulcer bleeding.

Point mutations of the c-Ki-ras gene in carcinoma and atypical epithelium associated with congenital biliary dilation.

OBJECTIVES: Congenital biliary dilation (CBD) may be accompanied later by gallbladder carcinoma or bile duct carcinoma. These cancerous lesions are frequently associated with atypical epithelium that was suspected of being precancerous. To determine whether atypical epithelium may be precancerous, we examined the DNA sequence of the c-Ki-ras gene at codon 12 in nine cases of CBD concurrent with seven gallbladder carcinomas, one bile duct carcinoma, and one bile duct atypical epithelium. METHODS: Tumor specimens were surgically removed from nine patients with CBD at Nagoya University Hospital between 1979 and 1988. Tumor was isolated by microdissection, and DNA was amplified by a two-step polymerase chain reaction which then was directly sequenced. RESULTS: Four of seven gallbladder carcinomas and one bile duct carcinoma contained the c-Ki-ras point mutation at codon 12, and atypical epithelium associated with these carcinomas had the same mutation. One case of atypical bile duct epithelium also contained the mutation. CONCLUSIONS: These results indicate that the c-Ki-ras point mutation at codon 12 may be responsible for either cancer or atypical epithelia associated with CBD.

[A case of early gastric cancer transformed from adenoma by chemotherapy and radiotherapy].

A 69-year-old male had a gastric adenoma at the greater curvature of the antrum. At the same time, malignant lymphoma of the liver and several bones were disclosed, but these tumors had become remarkably smaller after chemotherapy, radiotherapy and transcatheter hepatic arterial chemoembolization. Five and one half years later, however, the gastric adenoma had developed into gastric cancer that invaded submucosal layer. It is suspected that some chemotherapeutic agents and radiation were associated with malignant transformation of gastric adenoma.

[A case of myokymia-hyperhidrosis syndrome with muscle involvement].

The present report deals with an unique case of myokymia-hyperhidrosis syndrome. A 46-year-old man developed generalized relatively slow, undulating movement of the muscles, excessive sweating, muscle cramps and easy fatigability since three years ago. On admission, he had generalized myokymia, abnormal movements consisting of slow, undulating and worm-like muscular contractions. There was no muscle atrophy or weakness, or sensory deficits. Deep tendon reflexes were diminished and no Babinski sign or other pathological reflexes were present. Muscle tone was in normal range. Neither fasciculation nor myotonia was found. Laboratory examinations revealed elevation of BMR with normal thyroid function. The value of anti-Ach receptor antibody was below 0.6nmol/l. EMG recordings showed (1) continuous spontaneous repetitive discharges of NMU at a frequency of 30-50Hz, with duration of 1-2 seconds and amplitude of about 1 mV, were demonstrated at rest and the findings were consistent with myokymia, (2) waxing phenomenon was observed on repetitive peripheral nerve stimuli, which didn't improve by the tension test, and (3) high amplitude, long duration and polyphasic NMU, a sign of denervation, were observed on voluntary contraction. Muscle biopsy specimens from the left quadriceps showed a number of internal nuclei. On electron microscopy, clustered mitochondria and filamentous bodies were very often found in subsarcolemma. From these observations, damage to peripheral nerves, neuromuscular junctions and muscles may be responsible for the generalized myokymia in our patient.

Phaeohyphomycosis caused by Phialophora richardsiae.

A case of phaeohyphomycosis caused by Phialophora richardsiae is presented. The patient was a 30-year-old man with end stage malignant lymphoma. A subcutaneous abscess that developed on the dorsum of the right foot was removed surgically but immediately recurred. The causative mold was isolated from the pus in the lesion and identified as P. richardsiae. This is the fifth known case of phaeohyphomycosis caused by P. richardsiae and the first recorded in Japan.

Hidrotic ectodermal dysplasia: a clinical and ultrastructural observation.

Hidrotic ectodermal dysplasia (HED) was observed in a 49-year-old Japanese man. His clinical signs included alopecia, dystrophic nails and palmoplantar keratoderma, but his teeth, facial appearance and sweating were normal. In his family, 9 members in 5 generations were affected, suggesting that the disorder has an autosomal dominant mode of inheritance. Biopsy specimens of the hyperkeratotic lesions of the palm and sole were studied with an electron microscope. The most prominent feature was the increase of the number of desmosomal discs in the thickened stratum corneum, suggesting that hyperkeratosis observed in HED is due to the delayed desquamation of the stratum corneum.

Primary carcinoid tumor of the gallbladder: resection of a case metastasizing to the liver and analysis of outcomes.

Gallbladder carcinoid tumor is a rare and aggressive neoplasm, usually lacking specific symptoms, as they typically are unassociated with the carcinoid syndrome, despite frequent hepatic spread. The patient was an 81-year-old man with right upper quadrant pain who underwent radical surgery for carcinoid tumor of the gallbladder with liver metastasis (preoperative diagnosis, carcinoma). We analyzed the outcome of previously reported cases of gallbladder carcinoid. Increasing tumor size and depth of invasion progressively compromised the 2-year survival. These findings underscore the importance of early detection.

Hepatopancreatoduodenectomy for squamous cell carcinoma of the gallbladder.

Herein we present a case of a 57-year-old woman with squamous cell carcinoma of the gallbladder, with an advanced, large tumor infiltrating the liver and duodenum, but curatively resected by hepatopancreatoduodenectomy. She has been doing well without any sign of recurrence more than 5 years after operating. Hepatopancreatoduodenectomy is an adequate procedure for en bloc resection of advanced squamous cell carcinoma of the gallbladder because it often grows expansively. No residual tumor cells owing to hepatopancreatoduodenectomy shall fortunately result in a good prognosis.

Malignant fibrous histiocytoma of the gallbladder.

We report two extremely rare cases of primary malignant fibrous histiocytoma (MFH) of the gallbladder. The first case occurred in a 70 year old woman who presented with a large tumor of the gallbladder and multiple liver metastases. The second case involved a 74 year old man with a small submucosal tumor of the gallbladder and a single large liver metastasis. Histologically, these tumors consisted of spindle cells in a storiform pattern intermingled with bizarre giant cells. Both of these patients died of liver failure 3 months post-operatively.
