Schinzel-Giedion syndrome.

A 2-month-old girl was brought to the Department of Pediatrics at Wakayama Rosai Hospital because of poor feeding since 1 month of age. She was the third child of young healthy non-consanguineous parents whose first son was healthy but whose second son had died of 18 trisomy. Physical examination showed midfacial hypoplasia with coarse dysmorphic features, choanal stenosis, remarkable abdominal distention and bilateral talipes equivarus. Abdominal ultrasonography, computed tomography and drip infusion pyelogram showed left severe hydronephrosis and right moderate hydronephrosis. Having diagnosed Schinzel-Giedion syndrome, a left ureteroneocystostomy with tailoring was performed to preserve renal functions and to eliminate the urinary tract infection at the age of 3 months.

Post-traumatic high flow priapism: a case report.

A case of post-traumatic high flow priapism in a 32-year-old man is reported. The diagnosis was based on cavernous blood gas analysis and left internal pudendal arteriography. He was treated by unilateral super-selective embolization of the fistula with coils and gelatin sponge. At 1 month after embolization, erectile function had recovered, as demonstrated by measurement of nocturnal penile tumescence.

[Urinary schistosomiasis: report of a case].

A 31-year-old Japanese man visited our department with macroscopic hematuria. He had stayed in Asia, Middle East and Africa during the past several years. He was diagnosed with urinary schistosomiasis because examination of urine and a biopsy specimen of the bladder mucosa revealed Schistosoma haematobium ova. Cystoscopy revealed granulomatous lesions and polypoid patches at the right ureteral orifice, posterior wall and dome of the bladder. Radiological examination showed no obstructive uropathy. Histologically, moderate to severe urothelial dysplasia was found but without evidence of malignancy. The disease has been under good control following treatment with praziquantel. Today international exchange between Japan and other countries is common. Therefore, physicians should consider the possibility of schistosomiasis in patients who have been to endemic areas.

[Idiopathic retroperitoneal fibrosis localizing in pelvic cavity: a case report].

We report a case of idiopathic retroperitoneal fibrosis localized in the pelvic cavity. A 54-year-old female was found to have a right hydronephrosis by abdominal ultrasonogram and was admitted to our department. Drip infusion pyelography (DIP) and computed tomography (CT) revealed a right hydroureteronephrosis and a right lower periureteral mass. The mass was confirmed to show low intensity on both T1 and T2 weighted image in magnetic resonance imaging (MRI), which suggested fibrous component. Various tumor marker values were within normal range. Tumor extirpation including lower ureter and ureterovesiconeostomy were performed after confirming pathologically the absence of malignancy by fresh frozen section during surgery. The tumor was diagnosed histologically as idiopathic retroperitoneal fibrosis. We briefly reviewed 223 cases of idiopathic retroperitoneal fibrosis in the Japanese literature.

[A case of complete response with radiation and chemotherapy using cisplatin, etoposide and bleomycin].

A 37-year-old man, who had been treated with inguinal orchiectomy and radiotherapy of retroperitoneal and iliac lymph node for r-testicular tumor 6 months earlier, was admitted with a complaint of left leg pain in May 1992. CT scan of pelvic bone revealed osteolytic change of left pubic bone with soft tissue mass. Bone scintigram showed significant uptake of radioisotope on left pubic bone. Biopsy of left pubic bone confirmed histologic findings compatible with that of previous testicular cancer. He was treated with PEB chemotherapy (CDDP, Etoposide and Bleomycin) and radiation therapy. After treatment, the majority of the tumor mass disappeared on CT scan. On repeat biopsy specimens, significant fibrotic change was observed and no cancer cells were recognized. He is alive with no evidence of disease.

[A case of complete response was achieved with arterial infusion chemotherapy including CDDP, etoposide and pirarubicin].

A 67-year-old man was admitted with the chief complaint of macroscopic hematuria in May 1990. Endoscopic examination showed a bladder tumor at the right lateral wall. Biopsy proved Grade 3 transitional cell carcinoma. Transurethral ultrasonogram, CT scan and MRI revealed T2-T3a invasive bladder cancer. Preoperative chemotherapy by balloon occluded arterial infusion using CDDP 75 mg, etoposide 100 mg and pirarubicin 50mg was performed. Repeat endoscopy after chemotherapy revealed significant necrotic change of tumor. Radical cystectomy and pelvic node dissection with ileal conduit urinary diversion were performed in August 1990. Significant necrosis of bladder wall was observed and no cancer cells were recognized in the cystectomy specimen. The patient is alive at this writing with no evidence of disease for 2 years.
