[Imaging of breast neoplasms].

Imaging is crucial in early diagnosis of breast cancer. Mammography is still the most simple and reproducible method, except in dense breast. In these circumstances, ultrasonography may be helpful. Magnetic resonance imaging has some indications such as the diagnosis of local relapses and preoperative staging.

[Internal supporting mammary lamina. Results of the detection of breast lesions].

In order to stabilize the curves of a surgically remodelled breast, Bustos associated to a periareolar mammaplasty a synthetic internal supporting lamina. At the beginning, in 1985, a smooth perforated silicone sheet was used. Since then, other materials were proposed (radioopaque ou radiotransparent, resorbable or not). Because of their superficial situation, in front of the mammary gland, it was essential to study the consequences of the presence of such a material on the mammographic follow-up. We radiographied all the different types of sheet in vitro, and also in vivo, 3 to 19 months after surgery. At the same time we studied the modifications in the breast tissue, in 117 patients after surgical treatment for ptosis or mammary hypertrophy, using different kinds of incisions (periareolar or anchor-shaped, J or L shaped, inferolateral oblique or lower vertical incisions). We also studied the effects of mammary implants covered with Dacron, since some internal mammary supporting lamina are made with this material. The silicone sheet gives some artifacts and may calcify. The resorbable sheet has no effects on the mammography. The radiotransparent non resorbable ones causes no problems on short terms, but we are not sure whether some calcifications appear on long term or not.

Mammographic lesions suggestive of radial scars: microscopic findings in 40 cases.

PURPOSE: To analyze the histologic appearance of mammographically detected lesions suspected of being radial scars. MATERIALS AND METHODS: Mammographic and pathologic findings in 40 patients with a preoperative diagnosis of radial scar were reviewed retrospectively. RESULTS: Pathologic examination revealed 20 pure radial scars, 12 pure carcinomas, and eight malignant lesions (seven tubular carcinomas and one infiltrating ductal carcinoma) associated with a radial scar. At mammography, no difference was noted between benign and malignant lesions according to size and shape of the spicule, size of the central core, and the presence of calcifications. CONCLUSION: Because of the association of radial scars with borderline and malignant lesions, a spiculated lesion suggestive of a radial scar found at mammography must always be surgically removed. The surgeon and pathologist must be aware of the mammographic aspects of radial scar, however, to avert excessive surgery, because all cancers in this study had a good prognosis.

[Study of the efficacy and tolerance of Iopamiron 370 compared to Telebrix Hystero in hysterosalpingography].

Hysterosalpingography is used to study the state of the uterine cavity as well as to analyse the patency of the tubes and the state of the tubal mucosa. It is difficult to find a contrast medium that has both the ideal qualities, be well tolerated and able to show fine pictures that have good contrasts, while at the same time are sufficiently viscous to allow up examination. The contrasts products that are used to carry out hysterosalpingography are often classed as viscous (Telebrix Hystero) and fluid (Iopamiron). This prospective randomised study compares local tolerance and efficiency of the two products. Fifty patients had Iopamiron and 48 Telebrix Hystero in July 1988 and February 1989. There was a statistical difference between the two groups which was significantly in favour of Iopamiron on the following features: Pain as noted on a questionnaire with steps of five points each (p = 0.02), Pain as described by the patient immediately after the examination on a visual analogue scale with p less than 0.01. Overall appreciation of the two products according to their tolerance and the efficiency of the products, which is to say the clearness of the pictures obtained with a p = 0.05.

[Cancer of the breast and breast prostheses].

Plastic surgery operations designed to modify the breast volume do not increase the risk of cancer. Xeroradiography provides the best images of operated breasts. The least interfering prostheses are the most radiolucent and, in particular, retropectoral prostheses. The diagnosis of cancer is based on the detection of microcalcifications and star-shaped images. It is guided by clinical examination which is precise as the content of the breast is thin an lies on top of the anterior surface of the implant. The complications of prostheses (shells, collapse, rupture, displacement) have been well studied. After breast reconstruction examination of the contralateral breast is therefore of prime importance due to the risk of bilateral cancer.

Breast disease in the male: galactographic evaluation.

Seven men with unilateral nipple discharge underwent galactography. In two patients the diagnosis was carcinoma, two were benign papillomas, one was a breast abscess, and two were ductal ectasia. Galactography is useful in men and women with nipple discharge, especially when the discharge is bloody and there is no palpable tumor. The precise location of an intraductal lesion through the use of galactography guides the biopsy and makes conservative surgery easier.

Non-Hodgkin's lymphomata: clinical and immunological data in relation to histology.

Two hundred and forty-four previously untreated patients seen since 1964 in the department of haematology at Saint-Louis hospital were analysed. Clinical data included results of initial work-up and prognosis evaluated by survival rate matched with principal variables: age, sex, histopathology, staging, incidence of clinical and biological systemic symptoms. Immunological data included results of systematic studies made in each category of non-Hodgkin's lymphomata. Hyperbasophilic lymphoma, a special group recently characterized, is discussed.
