Indications of particle radiation therapy in the treatment of carcinoma of the esophagus.

Studies were made to evaluate the role of radiations in the treatment of carcinoma of the esophagus to confirm indications for charged particles. Results of the studies showed that prognosis of the patients treated with radiations depend strongly on length of tumor as well as on invasion of tumor cells into the adventitia of the esophagus. It was concluded that patients suffering from carcinoma of the esophagus less than 8 cm length are indicated for particle radiations.

Combination therapy of local administration of OK-432 and radiation for esophageal cancer.

Local administration of OK-432, a biological response modifier, was combined with radiation in 30 patients with esophageal cancer as a pilot study. Complete response was obtained in 22 of 30 patients (73.3%); and partial response was obtained in the remaining eight cases (26.7%). Complete response was obtained in all eight patients with a tumor less than 5 cm in length. In the group with tumors 5-10 cm in length, complete response was obtained in 11 of 14 cases (78.6%); and partial response was obtained in 3 of 14 cases. In the group with a tumor length of more than 10 cm, complete response was obtained in 3 of 8 cases (37.5%); and partial response was obtained in 5 of 8 cases. One-year, 2-year, and 3-year survival rates of the 30 patients, including four patients who died of other diseases, were 67.9%, 40.8% and 29.0%, respectively. This new combination therapy may contribute not only to local control, but also to survival.

[Entrapment of broken guidewire in the right coronary artery during percutaneous transluminal coronary angioplasty].

The distal 2 cm end of guidewire was broken and retained in the right coronary artery (RCA), during percutaneous transluminal coronary angioplasty for 99% narrowing of the middle RCA in a 53-year-old male patient with effort angina. Because of the stable condition of the patient, elective A-C bypass surgery was performed successfully 2 weeks after this event. However, the broken end of guidewire was not found out in the opened RCA lumen. One and a half year postoperatively, the patient is doing well while the guidewire remains in the same position.

[New combined therapy of radiation and local administration of OK-432 for esophageal cancer--local control rate and two-year survival rate].

We devised new combination therapy of radiation and local administration of OK-432 and performed for 26 patients with esophageal cancer between March, 1987 and December, 1988 as a pilot study. The average age was 73 years. Among 26 patients, male were 20 and female were six. Patients were irradiated at the schedule of under 2 Gy/day and the dose of TDF (time, dose and fractionation factor) 100 totally. OK-432, 10KE was endoscopically administered around cancer lesion at the beginning of the radiotherapy, and two weeks later, 5KE was given in the same manner. Complete response was obtained 20 out of 26 cases (77%) and partial response was obtained in the remaining six cases (23%). All six patients with tumor length less than five cm showed complete response. All 16 patients who could not eat food orally before treatment, became to take food enough orally after the treatment and could discharge in good condition. One year and two years survival rate of 26 patients by Kaplan Meier method were 67.4% and 47.2%, respectively. Six patients with tumor length less than five cm, are all alive without a sign of recurrence. This combination therapy will improve not only the response rate and the survival rate, but also the quality of life of patients with esophageal cancer.

[Normal tissue tolerance to high LET radiotherapy].

Based on the clinical evaluations about over 1800 patients who had been treated with fast neutrons at NIRS, the radiobiological properties of high LET radiations were discussed. The most favorable clinical results by fast neutron treatment had been revealed in such diseases as follows; salivary gland tumors, prostate cancer, Pancoast type lung cancer, osteosarcoma, soft tissue sarcoma. The characteristics of these tumors as to the radiobiological properties and the dose distribution are, 1) relatively slow growing tumors and higher RBE for tumor tissue, and 2) capability of correct delivery of a big radiation doses to the target, without any severe radiation complications. Normal tissue tolerance (NSD formulas) for each tissues were also discussed.

[A case report of redo A-C bypass for MCLS performed 13 years after initial surgery].

This is the first report of re-aortocoronary bypass for Kawasaki's disease. The patient is a 22-year old female. She was afflicted with Kawasaki's disease at the age of 6 and had the symptom of angina pectoris at the age of 9. She went through the first aortocoronary bypass, to the left anterior descending artery (LAD) and the right coronary artery (RCA) with saphenous vein graft (SVG). After the first operation, the graft to LAD occluded totally, but she remained asymptomatic and well for 11 years. She had a sudden recurrence of anginal attack at the age of 20. The examinations confirmed ischemia of the anterior wall (LAD area) and the lateral wall (LCX area). Coronary angiography revealed patent SVG with a moderate sign of sclerotic change. We decided on re-aortocoronary bypass, left mammary artery to LAD and gastroepiploic artery to LCX, when she was 22-years old. LIMA was anastomosed to LAD, but LCX was not revascularized, because LCX was not identified and exposed in the operation. In spite of incomplete revascularization, postoperative stress ECG test was negative. Tl-myocardial scintigram confirmed no ischemia of the anterior wall and greatly reduced ischemia in the lateral wall. She became asymptomatic and returned to normal life.

[The combination therapy of local administration of OK-432 and radiation for 2 cases of lower bile duct carcinoma].

The combination therapy of local administration of OK-432 and radiation was performed for two patients with lower bile duct carcinoma. Initially, 10KE of OK-432 was given locally, and irradiation was performed according to the schedule of 180-200 rad/5 fractions/week and given 5,000-5,220 rad totally. One case received 5KE of OK-432 locally 17 days after first injection. Both cases showed complete response and survived 4 months and 59 months without any sign of recurrence, respectively.

Adenocarcinoma

[The combined therapy of local administration of OK-432 and radiation in carcinoma of the trachea].

A CASE REPORT: A newly developed treatment method of local administration of OK-432 bronchoscopically combined with irradiation was applied to a patient with carcinoma of the Trachea complaining dyspnea and dysphagia. The tumor showed extra-wall growth. Dyspnea was improved in a week and the tumor disappeared after the treatment. A patient became to take food orally and could discharge in well condition.

Biological Products

[Histological effect of preoperative irradiation in bladder cancer].

Forty-seven patients with high grade invasive bladder cancer were treated with preoperative radiation therapy (910 rad by fast neutron or 3000 rad by X-ray for two weeks) followed by radical cystectomy and urinary diversion. Stage down effect was observed in 32% of patients when comparison was made between the clinical and pathological stages. Those who showed stage down effect had better prognosis when compared those without it. Histopathological effect of Grade 2B or Grade 3, according to the criteria described by Ohboshi and Shimosato, was noticed in 49% of the patients. Five year survival was as follows: 100% (PT0, PTis), 75% (PT1), 100% (PT2), 47% (PT3), 0% (PT4). These results clearly showed the improvement of prognosis compared to the historical control.

Present status of fast neutron therapy in Asian countries.

The present status of the treatment with fast neutrons performed in Asian countries is reviewed and the experiences with respect to the radiobiological indications are presentated and discussed. There are three facilities under operation, the National Institute of Radiological Sciences (NIRS) in Chiba, the Institute of Medical Science (IMS) in Tokyo and the Korea Cancer Center Hospital (KCCH) in Seoul. The clinical experiences can be summarized as follows: Fast neutrons are the treatment of choice for carcinoma of the salivary gland, Pancoast tumor of the lung, osteosarcoma, soft tissue sarcoma and malignant melanoma. Provided the isodose planning can be improved, it seems that also squamous cell carcinoma of the head and neck and esophagus, adenocarcinoma of the lung, stage I and prostatic adenocarcinoma can be benefit from neutron therapy. The same holds for malignant meningioma, while the benefit for glioblastoma multiforme has not yet been confirmed. Studies are going on for the treatment of other cancers and for evaluating the possible role of neutron therapy in combination with surgery.

Prognostic significance of Langerhans' cell infiltration in radiation therapy for squamous cell carcinoma of the uterine cervix.

Prognostic significance of Langerhans' cell (LC) infiltration in cancer nests was studied in 391 patients with squamous cell carcinomas of the uterine cervix. They were treated with radiation therapy alone. Langerhans' cells were identified by immunohistochemical staining for S100 protein. Langerhans' cells were present mainly in the intercellular spaces of tumor cells. The LC infiltration rates were higher in stage II (31.0%) or stage III (26.9%) than in stage I (17.5%) or stage IV (7.8%). The patients with LC infiltration showed a significantly better five-year survival rate than those without LC infiltration (68% and 56.1%, respectively). This significant difference was observed especially in stage III and it was suggestive in stage IV diseases. These results suggest that LCs in cancer nests may play a significant role in the immunologic defense against cancer in advanced stage of cervical cancer.

A method for selective evaluation of the effectiveness of fast neutrons in radiotherapy.

A methology is proposed for extracting and analyzing specific fast neutron effects on radiotherapy. This should be applied to the 1,500 patients treated at NIRS, Chiba in the past ten years. As the patients have been treated with different modalities based on generally combined radiation or neutron boost techniques at first the biologically equivalent doses per fraction for the photon and neutron component according to the Ellis concept were derived. In a second step treatment parameters, as the dose components and total dose as well as diagnostic parameters (tumor typing, grading and staging) will be related to the clinical outcome (survival, local control etc.). This may be done by multivariate discriminant or survival analysis.

Analysis of Variance

[Fast neutron radiotherapy of prostate cancer].

Between 1976 and 1987, sixty eight patients suffering from prostate cancer were treated with fast neutron radiotherapy at the NIRS hospital in Chiba, Japan. Tumor doses of TDF 100 were administered to the prostate gland, the regional lymphnodes, and the true pelvis, using the two opposing fields by fast neutron alone or with a boost. The overall local or regional tumor recurrence rate was found to be 10% and local control seemed to correlate with the histological differentiation of the cancer tissue, though there was no definite correlation with hormone treatment. A relatively high incidence of grade 3 and 4 radiation complications were observed, 21% for grade 3 and 7% for grade 4, due to subcutaneous fat necrosis caused by an inferior dose distribution of the fast neutron machine. The 5-year survival rate was seen to be 52% in total and 70% for patients in stage C. The modality of radiotherapy for prostate cancer is discussed.

Adenocarcinoma

[Fast neutron therapy in the National Institute of Radiological Sciences; 10 years' experience and future study].

A total number of 1623 patients were treated with fast neutrons produced by bombarding a thick Beryllium target with 30 MeV deuterons between November 1975 and December 1987. The results of clinical trials with fast neutrons have shown that carcinoma of the salivary gland and the prostate and Pancoast tumor of the lung were indications for fast neutron therapy, and that the patients suffering from osteosarcoma, malignant melanoma and soft tissue sarcoma had indications for fast neutrons when fast neutrons were combined with surgery. Neither carcinoma of the pancreas nor glioblastoma multiforme had indications because of complications of normal tissues. High LET radiation therapy will be evaluated by using heavy ions characterized by Bragg peak combined with biological effects.
