Prostate-specific antigen in prostatic carcinoma.

To evaluate the clinical and prognostic value of prostate-specific antigen (PSA) for the detection of tumor and tumor growth after therapy, 520 sera from 246 patients with prostatic carcinoma, 990 sera from patients with BPH, and 1,488 sera from patients with other urological diseases were analyzed. The values ranged from 0.1 to 1,828.9 ng/ml. 51% of all values were about 2.5 ng/ml, and 76.8% of all values about 10 ng/ml. The commercial recommendation for the cutoff values is 2.5 ng/ml (IBL, FRG). In patients with benign prostatic hypertrophy this cutoff means 61% false-positive results, which makes the test highly sensitive but unspecific. In prostatic carcinoma patients this borderline means a false-negative result in 9.75% (24 of 246). By determining the cutoff at 10 ng/ml in our series, a false-negative result appeared in 14.6%. Therefore a plea is made for the 10-ng/ml cutoff. In follow-up studies a marked decline in PSA values after transurethral resection or antiandrogen therapy (orchiectomy/Zoladex/ICI/flutamide, Essex). Generally, the greater the PSA levels the more advanced the stage of disease. These data suggest that PSA may be a useful adjuvant marker for monitoring tumor growth in patients with regionally confined tumor.

[Technetium polyphosphate and technetium diphosphonate. A comparative study].

99mTc-labelled phosphates are the bone seeking agents of choice. The comparison of 99mTc-polyphosphate (Poly-P) and 99mTc-ethane-1-hydroxy-1,1-diphosphonate (EHDP) shows a better in vitro stability of the latter during the first two hours after preparation. The blood clearance of EHDP is superior to Poly-P resulting in reduced background activity at scanning time. Renal excretion over a 4-hour period of urine sampling showed no significant differences between both substances. Blood activity is independent of diuresis.

[The clinical value of sequential renal scans with 131 iodo-hipuran and renal perfusion with 99mtc compounds using computer analysis (author's transl)].

Two methods of sequential scintigraphy are considered critically: a) Quantitative computer evaluation of nephrograms obtained by a gamma camera using 131Iodo-Hipuran. b) Quantitative flow studies of the kidneys after bolus injections of 99mTc preparations. Statistical evaluation of data from 171 patients examined by both methods has shown that the isotope nephrogram is not sufficiently sensitive for diagnostic purposes. Isotope nephrography combined with estimations of effective renal plasma flow on each side separately is of value to the urological surgeon by providing important information regarding severe degrees of renal functional impairment. Perfusion measurements, using a gamma camera and 99mTc preparations, are a promising survey method for the diagnosis of renal artery stenosis. (Much of this work is contained in a dissertation by C.
