Surgical treatment of concurrent cancer of the esophagus and stomach.

Out of a total of 250 resections for carcinoma of the esophagus done in our department between 1965 and 1983, there were 11 patients with a concurrent esophageal cancer, nine being gastric cancer and seven of them early gastric cancer. In these nine, four underwent resection of the esophagus and lesser curvature of the stomach with gastric tube reconstruction. Two of the four patients are alive 10 and 3 yr postoperatively. We hold the view that detailed preoperative gastric examination is most important to plan the surgical repair and that the small gastric tube may serve as a substitute by resecting the lesser curvature of the stomach in cases of early proximal gastric cancer.

Mucoepidermoid carcinoma of the esophagus--a case report.

We treated a 66-year old woman with mucoepidermoid carcinoma of the esophagus. The histologic features of this tumor seemed to originate from esophageal glands and their ducts. This deduction was based on the subepithelial growth pattern and the presence of in-situ carcinoma showing a glandular or squamous pattern at the location of the esophageal gland duct. Although the biological nature of this tumor was not elucidated, the prognosis is similar to that seen with the ordinary type of esophageal squamous carcinoma.

Improved results of surgery for esophageal carcinoma in 148 patients.

Since the late 1970's, there has been a remarkable decrease in the mortality of patients with esophageal cancer. Factors such as progress in pre- and post-operative management, operative technique, and anesthesia all play a contributory role in this improvement. Among 251 Japanese patients with esophageal carcinoma who underwent esophageal resection and reconstruction in our department of surgery since 1965, those treated from 1965-74 and others treated from 1975-1984 were investigated in detail. It became clear that pulmonary complications and anastomotic leakage were the two major complications related to operative mortality. The former has decreased by intensive postoperative care with strong emphasis on cough dynamics, and the latter because of the long gastric tube we devised and which has a good blood supply. These positive events make feasible early postoperative irradiation and cancer chemotherapy.

A contact-insensitive subpopulation in Syrian hamster cell cultures with a greater susceptibility to chemically induced neoplastic transformation.

We previously have identified a subpopulation of contact-insensitive (CS-) cells which lacks density-dependent inhibition of cell division in primary and low-passage cultures of Syrian hamster embryonic (SHE) fibroblastic cells. Further, we have shown that the proportion of these CS- cells declines as a result of the stable phenotypic conversion of the CS- cells to contact-sensitive (CS+) cells. To determine whether these transient CS- cells are more sensitive to carcinogenic/mutagenic perturbation, the susceptibility to neoplastic transformation and somatic mutation induced by N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) was examined in clonally isolated cell cultures containing various proportions of CS- cells (0.02-4%). The frequencies of morphological transformation, focus formation, and neoplastic transformation showed a positive correlation to the proportion of CS- cells in the treated cultures. In contrast, the frequency of MNNG-induced somatic mutation at the Na+,K+-ATPase locus was similar among cultures varying in their proportion of CS- cells. Thus, there is a transient subpopulation of CS- cells in primary SHE cell cultures that is more susceptible to neoplastic transformation although equally susceptible to induced point mutation. This dissociation between somatic point mutation and neoplastic transformation indicates a fundamental difference in the nature of these two phenomena. A possible relationship between the propensity of CS- cells (versus CS+ cells) to carcinogen-induced neoplastic transformation and the state of differentiation of the CS- cells is discussed.

[Clinical evaluation of cefminox in surgery].

Cefminox (CMNX, MT-141) was administered to 7 cases with postoperative infections including subphrenic abscess and wound abscess, and the clinical effect was good in 2 cases, fair in 2, poor in 2 and unknown in 1. A daily dose was 2 g in 7 cases. The maximum total dose and duration were 22 g and 11 days respectively. Side effect which was observed during the test period was 1 case of drug eruption. No abnormal laboratory findings related to this drug were noted.

Surgical treatment for carcinoma of the esophagus in the elderly patient.

There were 250 Japanese patients with carcinoma of the thoracic esophagus who had been admitted to the Second Department of Surgery at the Kyushu University Hospital from 1972 to 1982. Resection for carcinoma of the esophagus was performed upon 160 of these patients (64 per cent). These 160 patients were classified into three groups according to age, and preoperative examination, postoperative complications, mortality and late results were reported. In patients more than 70 years old, the incidence of abnormal findings at preoperative examination was high with a statistical difference and the incidence of postoperative complications was also high in elderly patients. Although the over-all mortality within one month after operation was seen in eight of 160 patients (5 per cent) surgically treated, the rate was 12.8 per cent when confined to those patients more than 70 years old. Thus, the indication for surgical treatment should be carefully determined for elderly patients with carcinoma of the esophagus. If the elderly patient can tolerate the surgical procedure, the five year survival rate is equal to that of younger patients. Since there is an increase in people more than 70 years old, the surgeon must be prepared for the challenge of treating elderly patients with carcinoma of the esophagus.

Preoperative hyperthermo-chemo-radiotherapy effective for carcinoma of the esophagus.

We prescribed hyperthermo-chemo-radiotherapy preoperatively for 13 patients with esophageal carcinoma. Viable cancer cells disappeared in four out of 13 patients, and all of these four patients survive without recurrence 3 to 34 postoperative months, at this writing. In another seven out of the 13, cancer cells were extensively damaged, despite the presence of a small number of viable cancer cells, and three out of these seven died with a recurrence. For two, the treatment was ineffective, and they died of liver metastasis and a local recurrence, respectively. These findings suggest that preoperative hyperthermia combined with chemotherapy and radiation is highly effective for treatment of carcinoma of the esophagus.

Hormone conditioned cancer chemotherapy for recurrent breast cancer prolongs survival.

Left suprarenal-inferior mesenteric venous shunt (Inokuchi) was prescribed for 80 patients with recurrent breast cancer and the efficacy of hormone conditioned cancer chemotherapy was assessed. The patients were separated into 3 groups according to the historical regimen of combined chemotherapy: Group I; surgical hormone therapy alone, Group II; surgery plus short term chemotherapy, and Group III; surgery plus long term chemotherapy. The 5 year survival rate of the responsive patients to the surgical hormone therapy was as high as 84.6 per cent in Group III, as compared to that of Groups I and II, 41.7 per cent and 16.7, respectively. Survival was not prolonged in non-responsive patients, regardless of the group. These findings indicate that surgical hormone therapy combined with postoperative long term cancer chemotherapy is a valid and effective method for treating recurrence of breast cancer.

[A case of carcinoma originating from accessory breast tissue of the axilla].

We report a case of carcinoma originating from accessory breast tissue of axilla and review 33 reported cases of carcinoma in heterotopic breast tissue. Although the incidence of accessory breast cancer is not high, early diagnosis and treatment are necessary, keeping in mind the possibility of this carcinoma when subcutaneous nodules of uncertain origin are found around the breast. Surgical treatment which must at least include wide local extirpation including the surrounding tissues and dissection of the axillary lymph nodes is necessary and adjunctive chemotherapy and irradiation appear to be useful.

Experimental study on trans-nodal cancer chemotherapy for metastatic lymph nodes.

As complete dissection of metastatic lymph nodes in the upper mediastinum is impossible in conventional surgery for esophageal carcinoma, we devised a form of intraoperative local adjuvant cancer chemotherapy for metastatic lymph nodes of the mediastinum, via tracheal bifurcation lymph nodes located at the anatomical pivot in the upper mediastinal lymph flow. Emulsion type bleomycin (BLM) was injected into the bifurcation lymph nodes of mongrel dogs, and BLM was identified in the pulmotracheal lymph nodes within 20 minutes. When the cervicothoracic lymph nodes in the ipsilateral side were dissected beforehand, the BLM levels in the contralateral side doubled those seen in the controls. In rabbits with VX2 carcinoma, the level of BLM decreased in lymph nodes with metastasis, however, the spread was still evident even when about 75 per cent of the nodes were occupied with metastasis.

Cough dynamics in oesophageal cancer: prevention of postoperative pulmonary complications.

The value of the cough reflex and its pressure and flow in the immediate postoperative period was measured in patients who underwent subtotal oesophagectomy and reconstruction for oesophageal cancer and other thoracic surgery. Twenty-two patients with subtotal oesophagectomy and reconstruction were found to have an extensive reduction in cough reflex and force, compared with those who had undergone pulmonary lobectomy. Damage to the vagal nerve caused by dissection of the paratracheal lymph nodes, and injury to the diaphragm during operation for oesophageal cancer appeared to be responsible for the inability to cough. It was of interest that the patients who showed a high value of cough reflex and force in the immediate postoperative period had few pulmonary complications. The postoperative measurement of cough ability may be a useful indicator for safe extubation of the intratracheal tube, as a means of preventing pulmonary complications after thoracic surgery. Continuous mechanical ventilation and frequent suction to remove retained secretions are strongly recommended for the patients who are unable to cough properly.

Clinical efficacy of the stapled anastomosis in esophageal reconstruction.

We have clinically evaluated Russian and American stapling devices used for esophageal reconstruction and compared the results following stapled anastomosis with those following hand-sutured procedures, both performed by the same surgeon. With the Russian stapler, anastomoses performed in 17 patients with carcinoma of the thoracic esophagus resulted in only one (5.9%) anastomotic leak; in 12 hand-sutured anastomoses and in 11 anastomoses done with an American stapler, no anastomotic complication occurred. Thus, a long gastric tube with good blood supply is of paramount importance, and if the technical details of surgical stapling can be overcome, a stapled anastomosis appears to be as safe as a manual suturing for patients undergoing esophageal reconstruction.

Transtracheal mediastinal lymphography for visualization of metastatic lymph nodes in carcinoma of the esophagus.

Mediastinal lymphography was carried out preoperatively upon 15 patients with carcinoma of the thoracic part of the esophagus. The tracheal bifurcation was punctured, and contrast medium was slowly injected. Mediastinal lymphograms were taken within 24 hours and, again, three and seven days after this injection. The paratracheal lymph nodes were demonstrated in almost all of the patients, and a filling defect was observed in three patients, leading to possible metastatic involvement. In seven patients, the right pulmonary hilar lymph nodes were demonstrated, and no node was considered to be metastatic. In three, paraesophageal lymph nodes were preoperatively diagnosed as metastatic. All seven lymph nodes in which metastasis was suspected were confirmed to be metastatic at pathologic examination. The smallest node correctly assessed as metastatic was 6 by 6 millimeters.

Problems in esophageal bypass for unresectable carcinoma of the thoracic esophagus.

The clinical evaluation of esophageal reconstruction in both resectable and unresectable carcinoma of the thoracic esophagus is described. Our operative technique for obtaining a long gastric tube and creating and end-to-side anastomosis has been highly successful, and the anastomotic failure rate was remarkably lowered in patients with resectable carcinoma of the esophagus. However, anastomotic leakage remains a serious problem for those undergoing esophageal bypass for unresectable carcinoma of the esophagus. The most likely cause of anastomotic failure after bypass seems to be the tug on the anastomosis with each swallow.

A qualitative and quantitative assay for cells lacking postconfluence inhibition of cell division: characterization of this phenotype in carcinogen-treated Syrian hamster embryo cells in culture.

We have developed a qualitative and quantitative assay system for detecting cells lacking postconfluence inhibition of cell division (contact insensitivity, CS-) in golden Syrian hamster embryo cells in culture by measuring the number of cells able to form colonies on a lethally irradiated, confluent monolayer of a contact-sensitive established cell line. A subpopulation in normal low-passage cultures of golden Syrian hamster embryo cells temporarily exhibits this CS- phenotype at very low frequency (approximately 4 x 10(-3)) but quickly loses the property within a few passages in vitro. This phenotype is invariably exhibited by various tumorigenic cell lines at very high frequency (7 to 50 x 10(-2)) and appears to correlate with the anchorage-independent growth phenotype. The temporal acquisition of the CS- phenotype by tertiary-passage golden Syrian hamster embryo cells following exposure to N-methyl-N'-nitro-N-nitrosoguanidine was examined. Cells with a stably heritable CS- phenotype are detected after approximately 20 posttreatment population doublings. In contrast, anchorage-independent cells are not detected until 35 to 95 posttreatment population doublings. These CS- cells appear to be preneoplastic cells, since clonally isolated CS- cells did not exhibit anchorage-independent growth until further passaging in vitro. The results suggest that acquisition of the CS- phenotype represents an early stage in neoplastic progression.
