Long-term prognosis of chronic idiopathic membranous glomerulonephritis. An analysis of 334 cases with particular regard to tubulo-interstitial changes.

A retrospective long-term study (average follow-up time 5.2 years) of 334 patients with idiopathic membranous glomerulonephritis (MGN) was carried out with the following results: 1) MGN was found to have a relatively good prognosis when all cases were considered together: 5-year kidney survival rate (KSR) -88%, and 10-year KSR -77%. 2) Univariate survivorship analysis showed the following morphological and clinical parameters to be associated with an increased risk of terminal renal insufficiency or death from renal disease: a) tubulo-interstitial changes; b) glomerular stage III as opposed to stages I and II; c) elevation of serum creatinine concentration at the time of the biopsy; d) arterial hypertension at the time of the biopsy. 3) Multivariate analysis showed that only tubulo-interstitial changes (interstitial fibrosis and/or acute renal failure) found at the time of the biopsy and their clinical correlate, serum creatinine concentration, were significant and therefore of definite prognostic importance. 4) Unsystematic therapy with steroids and/or cytostatic agents does not improve the long-term prognosis of MGN. 5) The cause of disease in the tubulo-interstitial system in MGN is discussed. Interstitial fibrosis is considered to develop possibly as a consequence of unresorbed interstitial edema which can develop during an episode of acute renal failure. Coexisting T-cell-mediated disease in the region of the intertubular capillaries is also considered as a possible factor in the development of interstitial fibrosis.

[Chemodectoma of the larynx (2 case reports)].

We report two cases of chemodectoma of the larynx. A 70-year-old man underwent a laryngectomy in April 1984 for a tumor diagnosed as a squamous cell carcinoma but which postoperatively proved to be a chemodectoma. Ipsilateral cervical metastases appeared 2 years later and were removed by local excision. Shortly thereafter he presented with recurrent nodes in both sides of the neck as well as cutaneous metastases on the trunk. He was given chemotherapy using a DTIC regime (5-3,3-dimethyl-1-triazino imidazole-4-carboxamide). A 53-year-old lady underwent a total laryngectomy in May 1984 for an extensive chemodectoma of the larynx. The tumor had already invaded the trachea and thyroid gland. Distant metastases were not found. So far she is free of recurrence. We discuss the histology, site, clinical course and therapy of this extremely rare tumor.

[Eosinophilic gastroenteritis with ascites].

Eight years after the onset of bronchial asthma a 28-year-old woman fell ill with abdominal pain, diarrhoea and rapidly increasing ascites. Increased numbers of eosinophils in the protein-rich ascitic fluid and histologically demonstrated eosinophilic infiltration of the intestinal mucosa, together with the history and symptoms, established the diagnosis of eosinophilic gastroenteritis. The disease proved self-limiting without specific treatment. Within four years of first manifestations there were two short recurrences with rapid regression without specific treatment.

[Subglottic plasmacytoma: diagnosis and prognosis].

A subglottic primary extramedullary plasmacytoma (typ IgA-Lambda) of the larynx is reported. These tumours are very rare. The diagnosis is made more difficult by unspecific symptoms and can only be confirmed by histopathology. Early diagnosis and differentiation between primary extramedullary plasmacytoma and metastasis from a multiple myeloma are very important for the prognosis of the disease. Whether immunology can help to solve this problem is still doubtful: few cases have so far been examined by this method. Paraproteins are often not secreted in extramedullary plasmacytoma; the peroxidase-anti-peroxidase method is therefore helpful for classification of the tumour.

Mitoxantrone metabolism in the isolated perfused rat liver.

The hepatobiliary pharmacokinetics of mitoxantrone, a new anthracenedione derivative, was studied in the isolated perfused rat liver. Mitoxantrone was administered in doses of 0.2 and 0.4 mg/kg body weight. Multiple bile samples were obtained for 4 hours. Mitoxantrone and three metabolites were separated by high-performance thin-layer chromatography (HPTLC) and measured at 610 nm. Following 0.2 mg mitoxantrone/kg body wt, 25.8% +/- 2.6% of the administered dose was excreted in the bile during 4 h, the major metabolite M1 accounting for 80% of this. After 0.4 mg mitoxantrone/kg body wt the amounts excreted were lower and light microscopic examination showed disseminated areas of cell necrosis.

[Non-Hodgkin lymphoma with gastrointestinal manifestations].

The gastrointestinal tract is one of the most frequent places of manifestation of an extranodal non-Hodgkin's lymphoma. We have collected the dates of 27 patients--14 men and 13 women--with non-Hodgkin's lymphoma of the gastrointestinal tract. The age of the patients varied widely, the 7th decade being the most represented one. The stomach was the most frequent place of manifestation. In 2 cases the lymphoma spread over the whole gastrointestinal tract. At the time of diagnosis 12 patients had already reached stage IV. The histologic examination according to the Kiel-classification showed 11 lymphomas of low malignancy and 6 lymphomas of high malignancy. The prognosis was essentially determined by the stage, less by the histologic type of the lymphoma. Finally the possibilities of therapy are discussed.

[Unusual course of morbus Hodgkin during remission of acute lymphatic leukemia].

6 years after the beginning of acute lymphatic leukemia and 3 years after the end of its therapy M. Hodgkin stage Ia was diagnosed in a then 18-years-old girl. From the same location--left neck--enlarged lymph nodes had been removed already during the maintenance therapy of leukemia. A malignant disease was neither proven nor excluded at that time, when signs of immunodepression were present. The patient is now in complete remission for both diseases.

Simultaneous occurrence of perimembranous glomerulonephritis and glomerular amyloidosis.

The results of electron microscopic examination of renal biopsies from 3 patients with rheumatoid arthritis treated with penicillamine are presented. All 3 patients developed a nephrotic syndrome upon discontinuation of penicillamine therapy. When viewed with the electron microscope, segmental forms of perimembranous glomerulonephritis (Stages I-II of Ehrenreich and Churg) and glomerular renal amyloidosis Grade I-III were observed. In all three cases the nephrotic syndrome was considered to be due to the simultaneous occurrence of the two disease processes. In 2 cases perimembranous glomerulonephritis with immuno-complex-deposits was assumed to be the dominant factor in the causation of the disease, in the other case amyloidosis was the principle abnormality.
