Diuretics in the treatment of hypertension.

With 30 years of experience, it is possible to claim that diuretics are well tolerated antihypertensive drugs producing a significant decrease in systolic and diastolic blood pressure as good as obtained with beta-blockers, converting enzyme inhibitors, calcium antagonists and centrally acting drugs. Recently diuretics have been criticised because of a substantial number of important side-effects, and are disregarded by some authors as first-line agents for the treatment of mild hypertension. Diuretics seem however safe in the majority of hypertensive patients who do not present special problems. Using low doses, which are equally effective, most toxic properties can probably be avoided. Combination therapy, especially with ACE inhibitors, has not only additional effects, but lowers also the potential adverse effects, caused by diuretics. We are convinced that diuretics are still important as first-line agents for the treatment of arterial hypertension in the general population.

Visceral leishmaniasis in an adult with HIV infection.

We present the first case of visceral leishmaniasis (VL) in a Spanish patient with HIV infection living in Belgium. After four weeks of stibogluconate and zidovudine treatment, the initially low CD4 count improved, and the splenomegaly regressed. VL is becoming frequently reported in association with HIV infection, especially in countries where leishmaniasis is endemic. The apparent effect of VL on the CD4 count may cause problems in the staging of HIV infections.

Bacteriological study of 92 cases of proven infectious endophthalmitis treated with pars plana vitrectomy.

92 cases of proven infectious endophthalmitis underwent pars plana vitrectomy first to identify the responsible agent and directing the antibiotherapy, second to remove the abscess and third to make room for intravitreal injection of antibiotics. A wide spectrum of organisms was isolated (16 different agents). Prognosis of endophthalmitis depends on (1) the responsible agent and (2) the reason for developing the endophthalmitis. The prognosis after lens implantation was rather favorable, whereas after vitrectomy the results were disastrous. Good results were obtained if the infecting agent was Staphylococcus epidermidis. Results after Streptococcus, Staphylococcus aureus, Bacillus or Pneumococcus were bad. Of the mycotic cases, only half of the Candida cases were cured. Other mycotic infections had a worse outcome.

[Retinal detachment following blunt trauma].

In the first part of this study a description of the different types of ocular trauma is given. This is followed by the pathogeny of retinal detachment after blunt trauma. Then the specific characteristics of the tears and other signs of non-perforation traumatic retinal detachment are discussed. Finally a description is given of the operative technique used in those cases.

Contusions

Malignant melanomas of the choroid and ruthenium applicators.

Fourty patients with a malignant melanoma of the choroid were treated with ruthenium applicators. Depending on the height of the tumour, the patients were divided into 3 groups. In the first group with a tumour height superior to 6 mm the results were poor as well from a functional point of view as considering tumour regression. In a second group with a tumour height between 5 and 6 mm the results were favourable concerning tumour regression, but only in 50% of the cases did the eye remain functional. The results in the third group with a tumour height between 2 and 5 mm were satisfactory. Tumour regression was obtained in 96% of the eyes; 86% of the eyes remained functional. Tumour related death (7.5%) was not correlated with initial height of the tumour at the time of diagnosis.

Scleral pockets and lyophilized sclera in retinal detachments.

For our buckling operations in retinal detachment, we prefer lyophilized sclera because it is well tolerated without tissue reaction, and very easy to handle. Its inertness is demonstrated by histopathological examination of sclera fragments taken during reoperations.

Freeze Drying

Polyglycolic acid sutures in cataract and extraocular muscle surgery.

Dexon was used as suture material in 26 cases of cataract surgery and in 46 cases of extraocular muscle surgery. It is better than chromic catgut in muscle surgery, since it produces less conjunctival irritation and no granulomas. In cataract surgery, on the contrary, it causes a pronounced irritation of the anterior segment and rather ofter a severe uveitis, probably because the sutures are external.

[Importance of the viability of the endothelium of corneal grafts].

Since we use a very strict control of the viability of the endothelium of the graft, we have no more failures in corneal grafting not even in cases with bad prognoses, such as Fuchs' endothelial dystrophy. On the contrary, in corneal grafting without this control, we had failures in 20% of the cases.

Cell Survival

[Myasthenia syndrome after peroral treatment with penicillamine].

A 59-year-old woman has been treated for 5 months with penicillamine for a rheumatoid arthritis and developed a myasthenia syndrome with ptosis, diplopia, dysphagia and asthenia. This myasthenia was slowly cured after suppression of the penicillamine and administration of Mestinon, so that after 3 months, the anticholinesterase medication could be stopped.
