[Covering skin-soft tissue defects with a free forearm flap in exposed osteosynthesis material in the area of the lower leg].

Soft tissue defects in the lower leg with exposed internal fixation plates frequently pose problems to the surgeon. Substantial defects in the distal lower leg should be covered by free tissue transplantation. Very good results can be achieved in such cases using the radial forearm flap. This flap can be used for covering exposed internal fixation plates in selected patients. The authors present four such cases. The flaps healed well in all cases and fracture healing was without delay. Bone fixation procedure did not have to be changed.

[Inhalation injury in burn patients].

The clinical course of inhalation injury is variable. The routine use of fiberoptic bronchoscopy provides an accurate and safe diagnosis; by this examination it is possible to recognize alterations down to the terminal airway. Biopsies of tracheobronchial mucosa can ensure the diagnosis. By means of fiberoptic examination and blood gases it is possible to realize a differentiation of inhalation injury. According to the analysis of our patients an inhalation injury was found in 20.1%. The mortality rate due to the inhalation injury amounts to 14.4%.

[Initial treatment of burns].

Burns, scalds and caustic burns all bring about an increase in microvascular permeability, with volume losses into the interstitium and at the surface of the body. The first aid measures to be taken include removal of the source of heat, maintenance of the vital functions, institution of infusion therapy with Ringer's lactate, and sterile covering of burn wounds, followed by rapid transportation of the victim to the nearest hospital. The crucial immediate step to be taken at the hospital, when large-area burn wounds are present, is rapid parenteral administration of fluid. Local management of the burns must be effected under sterile conditions. If an inhalation injury is suspected careful diagnostic procedures must be carried out. Therapeutic measures depend on the diagnosis. Finally, reasons for transferring victims to a burns unit are given.

[Sequelae of a minor injury of the hand in an anomaly of the brachial artery].

Numerous anatomical variations of the upper extremity arteries have been described, none of which have caused circulatory disturbance. One of these anomalies will be discussed (atypical double proximal ramification of the A. brachialis). In this case there were considerable trophic disorders of the skin and soft tissues of the arm following a minor injury, to the extent that one digit had to be amputated.

[Possibilities for using the tensor fasciae latae flap].

The TFL flap is an unusual musculo-cutaneous flap because a large island of skin (three times the area of the muscle element) can be safely elevated on a small muscular pedicle. Because of this the flap has a wide application for the repair of defects in the groin, perineum and lower abdomen or to cover the trochanter, ischium and sacrum.

[Results of surgically treated scaphoid pseudarthrosis using the Matti-Russe method].

For the treatment of scaphoid pseudarthrosis which is recognized sufficiently soon after an accident and where are no arthritic changes, the best method is that described by Matti-Russe; in our series we found healing in 95%. By grafting an autologous cortico-cancellous bone from the ilium we had no failures when we diagnosed and operated on the pseudarthrosis in the first five years after injury. We, therefore, now operate also on fractures with delayed union. The resection of the radial styloid process in combination with the Matti-Russe method has shown no advantages, and we have abandoned this modification.

[Pedicle flap plastic surgery in the primary care of large skin and soft tissue defects of the upper extremity].

Pedicle flaps from the chest or abdomen are still most suitable to cover large skin and soft tissue defects on the upper extremity. Compared to free tissue transfer with microvascular anastomosis, the conventional techniques ave advantages with regard to size, technical execution, technical differentiation and rate of healing in. Every skin and soft tissue defect on the upper extremity, however large it is, can be primarily treated by means of a pedicle flap.

[Opponens plastic surgery using the tendon of the m. extensor indicis].

For restoring the opposition of the thumb in lesions of the median nerve, numerous surgical procedures have been reported. In severe injuries with considerable scarring in the palmar wrist area the motor muscles of the forearm are useless and if the ulnar nerve is equally involved, opposition cannot be restored even with the Huber-Nicolaysen-Littler technique. In such cases we have recently come to carry out opponensplasties using the tendon of the extensor indicis with good success. The method is described and results are demonstrated.

[Isolated loss of function of the flexor pollicis longus muscles in moderate polyneuropathy].

A 60 year old female patient was referred to us with the diagnosis of a traumatic rupture of the flexor pollicis longus tendon. Because of disturbances in sensitivity in the superficial branch of the radial nerve, we carried out a neurological examination. A mild polyneuropathy was found, but there was no evidence against the presence of a trauma-induced rupture of the flexor pollicis longus tendon. In the surgical revision, a rupture could not be detected. However, the musculature of the flexor pollicis longus differed markedly in color and consistency from the other flexors and the histological examination showed atrophy patterns of the muscle fibers which are typical for polyneuropathy of the distal type.

[Anaesthesia for the severe burn patient (author's transl)].

By means of medical records, anaesthesia protocols and autopsy reports on 59 severely burnt patients of all age and risk groups, the three anaesthetic procedures mainly used in this group of patients-NLA, inhalation narcosis with halothane and Ketamine mono-anaesthesia-were retrospectively compared with regard to intra-operative complications and mortality. All the patients were in the initial phase of the treatment of burns, and had to undergo a variety of surgical interventions; these included changes of dressing, tracheotomy, necrotomy and skin grafts. Special pathophysiological problems of burns with particular consideration of the cardiocascular system, the lungs, the kidneys, and the electrolyte and acid-base balance are pointed out. Attention is drawn to the decisive importance of "preanaesthetic management" and to the specific technical problems of anaesthesia in patients suffering from burns. Each of the anaesthetic procedures discussed proved to be fundamentally suitable for the anaesthesia of persons suffering from very severe burns. Each of the anaesthetic procedures discussed proved to be fundamentally suitable for the anaesthesia of persons suffering from very severe burns. It should be left to the individual to decide which procedure in his experience should be preferred for a particular case.

[Skin and soft tissue defects at the lower extremities -- primary and secondary procedures (author's transl)].

Methods of surgical reconstruction which are available for wound closure on the lower extremities, following injury of full-thickness defects, are discussed -- especially in relation to indications of primary and secondary procedures. Special mention is given to cross-leg-flaps and tube pedicles of the legs. Indications and technical considerations involved in the treatment of 31 patients with cross-leg-flaps and 38 patients with tube pedicles are reported, as the results obtained from these operations. Out of 50 abdominal tube pedicles (38 patients), 12% were unsuccessful. 30 of the 31 cross-leg-flap were grafted successfully to their predestined sit.

[Experiences with nasotracheal intubation in the primary management of severe facial burns of face and neck (author's transl)].

Experiences with nasotracheal intubation in the primary management of 157 severe burns of the face and neck are reported. The indication for intubation is provided by the pathophysiology of these cases. Complications are minimized by a careful technique that makes due allowance to the individual anantomical features. The advantages and disadvantages of the method are discussed.

[Lengthening of the 1. metacarpal according to Matev after loss of the thumb].

In cases where loss of the thumb has occurred, the gripping function of the hand can be pariallly restored by the process of extending the first metacarpel. In our opinion the method according to Matev, involving distraction and the interpositioning of a bony wedge is a relatively simple operation and compared with similar methods has the advantage of achieving positive results in regard to the length. The only prerequisite for this method is good skin and soft tissue condition in the area of the stump.

[Treatment of arthroses of the carpal bones and the first carpometacarpal joint with liquid silastic implants].

In cases of resection arthroplasties of the carpometacarpal joint of the thumb we implant liquid silastic. This implant provides a good adaptation to the individual local situation and shows a good resistance to deformation. On the basis of our initial experience with this procedure we regard it as highly apt to provide an exactly fitting substitute in the wrist--especially in cases of an extirpation of the lunate bone.

[Therapeutic possibilities in fibro-lipomatous hypertrophy of the hand and finger nerves].

If the main nerves of the forearm and wrist are effected with fibro-lipomatous hypertrophy, decompression can be performed as a palliative measure. As the disease is not pregressive, resection or defatting of the hypertrophic fascicles should be avoided. The fibrous and lipomatous tissue cannot be separated from the fascicles without damaging them, even if the finest microsurgical technique are applied. As the disturbance of motor function increases, reconstructive surgery is necessary. In the fingers the "tumor" can be resected completely; the sensation of functionally important areas should be reconstructed with a neurovascular island flap or local pedicle flaps.
