Postoperative pulmonary complications in upper abdominal surgery. A randomized clinical comparison between physiotherapy and blow-bottles.

Blow-bottles were compared with conventional postoperative pulmonary physiotherapy in a randomized trial of 98 patients undergoing gastric or biliary tract surgery. Radiological evidence of postoperative pulmonary lesions found by "blind" evaluation of the films occurred with the same frequency in the two groups (30.6%). Blow-bottles were found preferable to physiotherapy because they are less time-consuming.

Biliary Tract Surgical Procedures

Palpebral cellulitis.

Patients with palpebral cellulitis may turn in the first place to a skin clinic. The clinical picture and the course of disease are illustrated here by four case histories. The importance of differentiating between collateral orbital edema, palpebral cellulitis and orbital cellulitis is stressed.

Aniridia, cataract and gonadoblastoma in a mentally retarded girl with deletion of chromosome II. A clinicopathological case report.

A case of bilateral aniridia, cataract and mental deficiency is described in a girl with an interstitial deletion of the short arm of chromosome 11. The child died of pneumonia and on autopsy bilateral gonadoblastoma was detected, but no Wilms' tumour. Partial deletion of the short arm of chromosome 11 seems to be a clinically recognizable syndrome with aniridia, cataract and renal, gonadal or genital dysmorphogenesis.

Cataract

The effect of blood-transfusions on renal allograft survival.

The relation between previous bloodtransfusions and renal allograft survival was studied retrospectively in 88 patients, who had been transplanted for the first time. The allograft survival rate was higher in the group who had received bloodtransfusion than in the group who had not. This difference was significantly greater when considering patients who had not previously been pregnant, e.g. patients without previous stimulation of the immuno system. The material shows no difference in allograft survival between previously transfused and non transfused patients, when considering age, sex or disease of kidney. In this retrospective study it is concluded that previous transfusions improved the prognosis of the transplantation. A prospective study has yet to be done, in order to enlighten the influence of various other factors on the allograft survival rate such as length of period or uraemia and of dialysis.

Adolescent

The ophthalmoscopic and fluorescein angiographic picture of diabetic retinopathy.

The ophthalmoscopic and fluorescein angiographic findings in diabetic retinopathy are reviewed. It is pointed out that the fundus lesions seen in diabetic retinopathy are characteristic, but not specific for diabetes. It is still not settled which role alterations in the microcirculation, alterations in the tissue metabolism, hypoxia or defects in the oxygen delivery system may play as provoking factors in the development of the diabetic retinopathy. Future investigations should concentrate on early diabetes and must include studies on the retinal blood-flow, electroretinographic and histopathologic changes.
