Observations of the course in 20 patients who underwent a long-term anticoagulation due to postthrombotic syndromes resulted in a clinical improvement according to the degrees of severity of the GDR section phlebology as well as in an increase of the plethysmographically measurable venous regurgitation. Thus the late sequelae of the postthrombotic syndrome may possibly reduced by long-term anticoagulation. In one third of the patients despite the anticoagulant therapy a decreased antithrombin-III-value was found.
A procedure is described for producing prothrombin complex concentrates from human citrate plasma by adsorption to DEAE-sephadex A 50. The technical appliances developed are described. As compared with the level of technology an efficient manufacture of the preparations can be ensured by essentially simplifying the technique at a charge volume of 10 l.
In 30 patients with acute myocardial infarction in 4-hour intervals the temporary course of the ACTH and cortisol in the plasma was pursued up to the 60th hour. Here 3 different patterns of behaviour were found. In group 1 (11 patients) normal ACTH and cortisol values are present during the period of examination. In group 2 (13 patients) ACTH in increased at admission to hospital, and a decrease of the values to the normal takes place after the beginning of the infarction within 4 to 8 hours, whereas the plasma cortisol remains clearly increased. Increased ACTH and cortisol values during the whole period were measured in group 3 (6 patients). The different hormone constellations found are possibly the expression of a different reaction behaviour of the adrenocortical system in acute myocardial infarction. There is a weak positive correlation between the creatine kinase and the plasma cortisol. The size and the duration of the increase of ACTH and cortisol seems to be connected with the degree of severity of the acute myocardial infarction. An increase of ACTH and cortisol for a longer time is to be regarded as unfavourable for the course of the acute myocardial infarction, while uncomplicated courses are connected with normal ACTH and cortisol in the plasma.

A series of ampholytes is investigated concerning its influence on the activity of factor VIII. While positively charged ampholine (polyamino-polycarboxylic acids) may increase the activity of factor VIII, positively charged amino acids and dipeptides do not have this property. A group of substances (servalyte) constructed similarly to ampholine which besides amino groups and carboxyl groups has as charge carriers also phosphate and sulphate groups increases the activity of factor VIII like ampholine. Obviously not only the net charge of the molecule plays an essential role in the influence on the activity of factor VIII, but also size and configuration.

As compared to control animals, guinea-pigs to the shaved backs of which a dose of 0.16 ml of a 0.12% solution of PES/100g. of body mass had been applied twice daily for periods of 28 and 90 d, respectively (5 d of application being followed by 2 d without treatment), showed erythema, loss of hair, slower increase in mass, increased heart rate, smaller body mass/kidney mass and body mass/spleen mass ratios, increases in leucocytes, ASAT, ALAT, LDH I and LDH III. In the 28-d test, slight inflammatory symptoms of the liver, kidneys and heart were observed in the experimental animals. These symptoms were more marked in the 90-d test, granulomata in the livers being particularly striking. A pneumonia of moderate to very marked degree is suggestive of the activation of a clinically latent PES infect whereby the inflammatory alterations in the kidneys might be explained by the formation of metastases.
IN general the therapy of thrombosis during pregnancy is carried out with heparin, in which case the initial hospital treatment may be continued later on with double subcutaneous applications of heparin in the out-patient department. Apart from coagulation-physiological parameters plethysmography and/or ultrasound Doppler method serve for the control of the course. Within the prophylaxis of recidivations in earlier thromboembolism during pregnancy regular clinical and apparative controls are performed, in which cases in the last third of the pregnancy the possibilities of the consideration work are used. Immediately pre-, intra- and postpartally heparin is injected with low dose up to the optimum phenprokoumon regulation (beginning with the 2nd day post partum). Thromboses in the anamnesis are no essential risk, when an optimum prophylaxis of thrombosis is guaranteed during pregnancy and puerperium

The effectiveness of streptokinase treatment of patients with myocardial infarction as compared to the control group and patients given anticoagulants is analysed. It is shown that the frequency of recurrent myocardial infarctions, congestive circulatory insufficiency, shock, and thromboembolic complications is lower in the group of patients treated with streptokinase than in the control group. Streptokinase improves the course of the disease and the prognosis in patients with preinfarction angina pectoris. The effect of streptokinase is higher in the first 3--6 hours after the development of myocardial infarction and in individuals under the age of 65. The effectiveness of the treatment does not diminish when the drug is injected for a short time and in small doses (300,000--750,000 U).
Issuing from the question of the value of a prophylactic anticoagulation for the purpose of prevention of arteriosclerosis 25 patients with haemophilia were examined with the aim to detect possible connections between coagulation potential and arteriosclerosis taking into consideration factors of risk and age. Indeed, also in these patients who were "anticoagulated" from birth factors of risk and references to an existing arteriosclerosis were found. Correlation analyses, however, indicated tendencies, which suggest a protective influence of the hypocoagulability concerning factors of risk and arteriosclerosis.
Seven patients with haemophilia A CP were injected in a dosage of 1 to 3 double units, which were stored for 127 days partly deep-frozen, partly lyophilized and after this time contained fibrinogen fission products (16--256 microgram/ml). Though in two patients after the transfusion positive WA of the injected fibrinogen fission products were established, a relevant influence on the fibrinogen fission product levels of the patients is to be expected only after administration of larger quantities of such preparations. The therapeutic efficacy of the stored CP was insufficient, since despite good in-vitro-activities of factor VIII the increase of factor VIII in four patients was slight and retarding and did not take place in three patients.
In a chronological representation a survey of important duties of the transfusing physician in blood transfusions is given. When fulfilling conscientiously the special, legal and organisational demands, the transfusing physician may authoritatively influence the decrease of disturbances of transfusion.
