Skeletal changes and growth in experimental uremia.

Longitudinal growth; bone and growth zone histology; growth cartilage and bone mineralization (tetracycline technique); bone Ca content (neutron activation analysis); bone radiology; serum and urine chemistry; urinary cAMP and serum 25-OH-vitamin D3 were studied in a long-term model of experimental uremia in the rat. Uremia was induced by two-stage subtotal nephrectomy with irradiation of the remaining parenchyma. Ccr in the experimental group was 113 +/- 5.8 micron1/min X 100 g (19.8% of controls) and serum creatinine 1.67 +/- 0.04 mg% (5.1 X control value). Uremic animals were pair-fed with sham-operated controls. In the proximal tibia delayed transformation of cartilage into primary spongiosa with appearance of chondro-osteoid and delayed transformation of primary spongiosa into secondary spongiosa was observed (rickets). Increased amounts of osteoid were present although 25-OH-vitamin D3-levels were high. There were only modest signs of secondary hyperparathyroidism (osteoclast counts; urinary cAMP). In spite of the presence of bone disease, longitudinal growth was not reduced in uremic animals as compared with pair-fed sham-operated animals, but was significantly reduced as compared with ad lib fed control animals. In contrast, weight gain was significantly diminished in uremic animals as compared with pair-fed sham-operated control animals. It is concluded that diminished intake of food is the major determinant of growth retardation in preterminal experimental renal failure.

[Heart diagnosis using m-mode-echography. Continuous registration of transverse left-ventricular internal diameters. IV. Patients with coronary artery disease (author's transl)].

"Relative Cross-sectional area Differences" (RCD) and "Relative mean Velocity of Contraction" (RVC) can precisely be determined by means of impulse-reflected ultrasound. They are highly invariant under physiological conditions and closely related to the degree of non-ischemic myocardial disorders. To evaluate the diagnostic information of these parameters in coronary artery disease (CAD), echocardiographic examinations were performed on 105 subjects. The results were compared with the angiocardiographically determined ejection fraction and with minimal cardiac transit times. The heart-dynamic data were grouped according to the manifestation of CAD and correlated to each other. Furthermore the parameter's capability in identifying differences in hemodynamics in normals and in patients with CAD was tested. In more than 90 per cent of patients with CAD, RCD and RVC exceeded the normal range. For clinical diagnosis the results lead to certain limitations in the validity of the echocardiographic parameters. In cases of infarction localized at the apex and in myocardial aneurysmas, RCD and RVC reflect the function of the whole myocardium, not the ejection fraction of the left ventricle. In those cases of infarction of the posterior or posterolateral wall, the value of RCD and RVC could not be verified. If infarction occurs at other localizations (septum, anterior wall), RCD and RVC show the degree of reduced myocardial function without any limitation.

Investigations into human thorotrastosis. Tissue concentrations of 232-Th and late effects in 13 autopsy cases.

The subjects of investigation were 13 dead thorotrast patients, 10 male, 3 female, with ages ranging from 45 to 79 years. Two thousand organ and tissue specimens were investigated by means of autopsy and by both microscopic-autoradiographic and neutron activation analysis in order to detect late effects and to determine on approximate mean concentration of 232Th (mg per g of tissue). A comparison between late effects and concentrations of the dye medium led to the following conclusions: 1. 232Th is, after intravascular injection, deposited in all organs and tissues of the human body. 2. The highest mean concentrations are shown in the spleen, liver, bone marrow, and lymph nodes. 3. The distribution of 232Th is inhomogeneous in all organs and tissues. The variations of maximum and minimum concentration lie around factor 2.2 X 10(0)-2.4 X 10(5). 4. Late effects occur only in spleen, liver, lymph nodes, and bone marrow, but not in organs and tissues that show a mean concentration of 232Th under 10(-1) mg per g tissue. 5. It is highly probable that tumors of thorotrast patients in organs other than spleen, liver lymph nodes, and bone marrow are not caused by deposition of 232Th or ThO2.

[The pattern of distribution of free amino acids in human amniotic fluid related to the duration of pregnancy. Part I: Changes in concentration in normal pregnancy (author's transl)].

Gas-chromatographic measurement of 18 free amino-acids from about 500 specimens of amniotic fluid between the 10th and 41st week of pregnancy. Statistical methods showed different concentrations of amino-acids in amniotic fluid during pregnancy. Serine, cysteine, methionine and tryptophane show constant concentration. Valine tyrosine and hydroxyproline continuously decrease. The other amino acids tested here show a biphasic course of concentration, falling by about 40-70% between the 10th and the 30th week after which they remain stable. Alanin, threonine, glycine and ornithine show a clear increase of concentration. This diversity of concentration of amino-acids during pregnancy hints at the importance of various factors for the composition of amniotic fluid.

[Animal experimental studies of positive tumor demonstration with radio-iodine labelled albumin macroaggregates].

131I-labeled macroaggregates were injected into the femoral artery of rats bearing a Yoshida sarcoma at the lower extremity and of healthy control animals. The radioactivity was measured in tissue samples taken from the tumour and in unaffected muscle tissue. The tumour tissue showed a considerably higher uptake of radioactivity than the not-neoplastic tissue of control animals. The difference was increased when more time elapsed between the injection and the measurement. After 48 hours the ratio was 25:1 in favour of tumour tissue. In contrast to the transplanted Yoshida sarcoma, hepatomas in Wistar rats induced by N-nitroso-morpholin showed a considerably smaller increase of radioactivity than the control group when injected with 131I-MAA. The fixation ratio between tumour and normal liver tissue was 1:5.9. In contrast to autochtone liver tumours rats with transplanted tumours are considered as a suitable model for testing labeled albumin particles like 131I-, 111In- or 99mTc-labelled albumin microspheres developed for angioscintigraphic purposes.

Bone volume and mineral density of iliac crest spongiosa in uremia.

Bone mass and bone mineral content were measured in non-dialyzed and dialyzed uremic patients. Bone mass, measured by micromorphometry and a gas displacement method, was higher in uremic than in age and sex matched control subjects (micromorphometry-U:25.8 +/- 8.24%; Co:15.6 +/- 4.38; gas displacement-U:211 +/- 66 mm3/cm3; Co:191 +/- 45). In hemodialyzed patients, bone mass was lower the longer the patients had been on dialysis (r = 0.38; P 0.05). Bone mineral content (specific weight) was diminished in uremia (1.82 +/- 0.095 g/ml; controls 1.854 +/- 0.0173). In hemodialyzed patients, specific weight was higher, as was Ca content of bone assessed by neutron activation analysis. It is concluded, that negative Ca balance was the major cause of bone loss and that bone loss is thus preventable.
