[The determination of bone age by the Greulich-Pyle and Tanner-Whitehouse methods as a basis for the growth prognosis of tall-stature girls].

Different height predictions of 48 untreated girls with constitutionally tall stature (bone age 10.0-14.0 years) were compared with the final height. The following conclusions were drawn: The height prediction according to Bayley-Pinneau provided the best results. However, by this method the final height of the younger girls is markedly underestimated while that of the older one's is slightly overestimated. Mathematical methods for correction of these errors and for estimation of the growth rates are given in the text.

Long-term care and management of Wilson's disease in the GDR.

Diagnosis, long-term management and family investigations of Wilson's disease are provided by selected clinical institutions in the GDR. From 187 patients detected since 1949, 111 are alive. In spite of the principal effectiveness of penicillamine treatment, confirmed by the disappearance of most of the central nervous system symptoms and successful professional rehabilitation of many patients, insufficient therapeutic discipline, psychosocial disturbances and penicillamine side-effects forcing its substitution by zinc or triethylenetetramine dihydrochloride in 14 cases need our further attention.

Comparison of two tests to recognize or exclude 5 alpha-reductase deficiency in prepubertal children.

Plasma testosterone (T, nmol/l) and dihydrotestosterone (DTH, nmol/l) were measured in 54 children and young adults with male pseudohermaphroditism (46XY, no defect of steroid biosynthesis) 4 h after im injection of testosterone propionate (25 mg/m2, group 1, N = 18), or before and 2, 4 and 6 days after hCG (5000 IU/m2 im, group 2, N = 36). The response to hCG was also studied in 5 control children (unilateral cryptorchidism, group 3) and that to testosterone propionate in a gonadectomized child with confirmed 5 alpha-reductase deficiency. Mean T (133.1 +/- 14.0, SEM) and DHT (17.1 +/- 2.6) in group 1 were higher than in group 2 (17.3 +/- 2.1 and 2.9 +/- 0.4), but there was not significant difference in the T/DHT ratios (group 1: 10.7 +/- 2.0; group 2: 7.2 +/- 0.6). Following testosterone-propionate, there was a negative correlation of T with age (r = -0.723). After hCG, T and DHT were lower in the prepubertal children than in those under 2 or over 10 years, and the T/DHT-ratio rose with age. Two children from group 1 had a T/DHT-ratio above 18, but urinary aetiocholanolone/androsterone (Ae/A) ratios were normal. In the child with 5 alpha-reductase deficiency, the T/DHT ratio was 60, and the urinary Ae/A ratio high. We concluded that the two tests are suitable for confirming or excluding 5 alpha-reductase deficiency in prepubertal children, in whom basal DHT is too low for evaluation, but that physiological age-related changes in 5 alpha-reductase activity have to be taken into consideration.

Growth in thyrotoxicosis.

To study the effect of thyrotoxicosis on growth, 46 children and adolescents presenting with thyrotoxicosis have been reviewed and followed up for several years, in most cases until adult height was achieved. At presentation the children were underweight for height but were above expected heights even if already well advanced in puberty. Despite bone ages often being very advanced at diagnosis, the condition had no adverse effect on ultimate stature, which was commonly greater than that expected on the basis of parental height. Wight was rapidly regained on treatment, but nevertheless the younger subjects continued to be underweight despite adequate prolonged control of thyrotoxicosis.

Effects of bacterial sepsis on protein metabolism in infants during the first week.

In the presence of bacterial infection during the first week of life a catabolic state evolves more rapidly than in the later life. Hyperaminoacidaemia and subsequent hyperaminoaciduria, elevated urea concentrations in serum, and increased renal nitrogen losses are the most important metabolic changes. These findings correlate with the clinical course of the disease and refer to the primary disturbance of cell metabolism as the cause of the metabolic changes observed. While no relation could be observed between metabolic response and bacterial species, there is a significant dependence on the time interval from the occurrence of first clinical symptoms at the beginning of antibiotic treatment. If this period is longer than 8 h, serious metabolic disturbances will usually be found. Due to the limited amino acids utilization, especially by the liver, an adequate energy intake should be the primary aim of nutrition, whereas a protein intake of more than 2.0 to 2.5 g/kg X day may further aggravate the metabolic imbalance.

Variability in the Proteus syndrome: report of an affected child with progressive lipomatosis.

In 1983 the Proteus syndrome was delineated by Wiedemann et al. [12]. We report a 10-month-old girl, a further child affected by the new syndrome. The typical signs are macrodactyly, hemihypertrophy, pigmented nevi, hyperkeratosis, and subcutaneous hamartomatous tumours. Our patient shows an aggressive lipomatosis on the trunk and local relapses after surgical interventions in the regions involved. Histology of the adipose tissue showed considerable anisocytosis and increased cell volume.

[Effect of hormonally-induced prolongation of pregnancy in rabbits on maternal and fetal serum lipids].

The alterations of the lipid metabolism produced by placental insufficiency were investigated in animal experiments. Pregnant rabbits received Chlormadinon-acetate by intramuscular injection. The pregnancy was prolongated for 4 days, and a placental insufficiency developed. The lipid metabolism of the mothers and the fetuses was investigated by determination of beta-lipoproteides, triglycerides, cholesterol and HDL- and LDL/VLDL-cholesterol. The most important result is an increase of the concentrations of triglycerides and cholesterol of the fetuses during prolongation of pregnancy. Of the different cholesterol fractions estimated the LDL/VLDL-cholesterol-ratio excellently reflects the changes of the lipid metabolism during prolonged pregnancy.

Clonidine (Haemiton) as a reliable stimulus of growth hormone secretion in patients with short stature.

In 14 patients with short stature aged 4-14 years the effect of clonidine has been investigated. A single oral dose of 0.15 mg/m2 surface area is followed by a marked increase of hGH concentration in blood with a peak value between 60 and 90 min after the administration of the drug. The hGH increase caused by clonidine was comparable to the rise during insulin-induced hypoglycemia. In 5 patients both stimuli failed to increase hGH release. The test procedure is simple. It shows only minimum side effects and can therefore be especially recommended in outpatients.

[Activities of various enzymes in brown fat tissue of rabbits in the pre- and post-natal period].

Newborn rabbits have mainly brown or multilocular fat tissue. The activity of G-6-PDH shows a prenatal decrease. The G-6-PDH activity is clearly lower in postnatal as compared to the fetal period. This is due to the minor role of fatty acid synthesis with reduced fat storage during this time. The high capacity of glycolysis becomes apparent by the high activity of hexokinase and lactate dehydrogenase. The activity of both enzymes, however, shows only slight postnatal changes, so that HK and LDH in contrast to pyruvate kinase in the postnatal period apparently have no regulatory effect. The altogether high activity of the NADP specific isocitrate dehydrogenase indicates the high turnover rates in the Krebs cycle and suggests the adjustment of the metabolism to a high energy turnover.
