[Adrenal cortex insufficiency caused by metastatic tumors].

Clinical features and the management of adrenal insufficiency are considered with reference to etiology, diagnosis, and treatment. Specifically considered are the management of patients with Addison's disease and metastases. Adrenal insufficiency may go unrecognized in the cancer patients because features such as nausea, vomiting, anorexia as well as orthostatic hypotension are not specific and may be mistakenly attributed to advancing malignancy or side effects of therapy. Prompt recognition and treatment of adrenal insufficiency can avert potentially life-threatening situations.

Transforming growth factor beta 1 is significantly elevated in plasma of patients suffering from renal cell carcinoma.

The levels of active and latent transforming growth factor beta 1 (aTGF-beta 1, ITGF-beta 1, respectively) in plasma samples were measured by enzyme-linked immunoabsorbent assay (ELISA). Samples were collected from patients suffering from renal cell carcinoma (RCC) before they underwent tumour resection. In all cases tested, the levels of latent TGF-beta 1 were much higher (n = 23, 41.0 +/- 13.9, range 19.3-78.1 ng/ml) than in healthy controls (n = 21, 3.8 +/- 2.9, range 0.6-9.9, P < < 0.0001), while active TGF-beta 1 did not differ that impressively (n = 38, 1.2 +/- 1.3, range 0.0-4.5 for RCC, n = 21, 0.1 +/- 0.2, range 0.0-1.1 in controls, P < 0.001). As for TGF-beta 1 production by proximal tubulus cells has been shown, it was speculated that these high TGF-beta 1 levels might be due to secretion by the tumour, which usually originates from proximal tubuli. Indeed, production of TGF-beta 1 was found in culture supernatants, and it was possible to show TGF-beta 1 mRNA expression in tumour samples. This TGF-beta 1 was predominantly secreted in the latent form. This is in contrast to data from other authors, who found TGF-beta 1 to be secreted mainly in the active form, but need not mean that it is inactive locally as the low pH encountered within tumours is in the range necessary for its activation. It is concluded that elevated latent TGF-beta 1 is common in RCC, is at least partially produced by the tumour and might be a cause for local immunosuppressive effects within the tumour.

Induratio penis plastica--a factor of erectile dysfunction?

There are still diverse management options in Peyronie's disease and diverse opinions about the coincidence of Peyronie's disease and erectile dysfunction as well. The connection between Peyronie's disease and ED has been proved by some papers but on the other hand authors have refused to accept this claim. We have found erectile dysfunction in about 30% of our patients. We investigated 61 patients with Peyronie's disease. As diagnostic criteria assessment of angulation and erectile dysfunction are proposed. The tunica albuginea plication technique was used in 35 patients with good results. In our investigation we did not find any coincidence of Peyronie's disease with erectile dysfunction.

[Adrenal gland carcinoma--status of diagnostic and therapeutic possibilities].

Innovations in diagnosis and therapy of adrenal neoplasms require a discussion about these tumors. Especially, the articles deal with the radiologic evaluation of the adrenal gland, technical as well as perioperative considerations of adrenalectomy, and adrenocortical carcinoma. Several conclusions can be drawn from our own results and a review of the literature. Firstly, the diagnostic accuracy should improve with advances in magnetic resonance imaging. Secondly, all asymptomatic nonfunctional adrenal masses about 3 cm require removal. Thirdly, adrenocortical carcinomas are generally curable only if small and localized. Fourthly, among the adrenal tumors, pheochromocytomas--especially malignant pheochromocytomas--deserve a careful approach: adequate preparation and early ligation of the adrenal vein after transabdominal access are the main goals. Finally, adjuvant treatment regimens for more advanced adrenocortical carcinomas using mitotane and conventional multiagent chemotherapy need to be evaluated in randomized trials.

Pneumocystis carinii infection in the middle ear.

Pneumocystis carinii is the opportunistic pathogen frequently causing pneumonitis in the acquired immunodeficiency syndrome. Extrapulmonic manifestation of P carinii is unusual and is commonly associated with severe systemic illness, other immune deficiency status, malignancy, or immune suppression. We describe a case of acquired immunodeficiency syndrome with manifestations of P carinii otitis media with severe otalgia and conductive hearing loss.

Radioiodination of S-type lipopolysaccharide. Possibilities and limitations of labelling of the carbohydrate chain by periodate oxidation.

S-type Lipopolysaccharide (LPS) from E. coli O 139:K 82 (B) was radiolabelled by coupling 125I-tyramine to the carbohydrate chain of LPS after oxidation of sugar residues carrying vicinal hydroxy groups, with sodium metaperiodate. The specific activity amounted to 1.3 kBq/microgram LPS. As LPS is a mixture of molecules with different carbohydrate chain lengths that are associated to large micelles of high kinetic stability, the preparation and the properties of the tracer exhibit some peculiarities. Most important, partial cleavage of the carbohydrate chains of LPS is caused on radioiodination by the procedure described. As a consequence, the tracer differs markedly from the starting LPS with respect to carbohydrate chain length distribution. Another feature of special interest is the variation of the specific activity (radioactivity/unit mass) for LPS molecules of different carbohydrate chain lengths. Because of the kinetic stability of LPS micelles, the equilibration of radioiodinated and non-radioactive LPS aggregates requires their solubilization by a detergent and its subsequent removal by dialysis. It could be demonstrated that short-chain LPS molecules predominantly associate to larger micelles than long-chain molecules. Regardless of these restrictions, the 125I-LPS proved to be useful for certain analytical purposes.

Behaviour of heart rate and electrical muscle activity in the simultaneous dynamic action of larger and smaller sets of muscles in the permanent performance threshold range.

This study was undertaken to assess more comprehensively and with greater differentiation the reduction in the absolute electrical muscle activity (aEA) determined by different investigations in the permanent performance threshold range of the cardiovascular system (HKS) by means of the behaviour of the aEA of the highly stressed leg muscles and little stressed forearm muscles. Six healthy, untrained male subjects operated a hand ergograph alone in one test, and, in four more tests, in combination with a constant bicycle ergometer load of 40, 60, 80, and 100 W, respectively. As a result of these tests, a relative reduction in aEA for the leg muscles and an absolute reduction in aEA for the forearm muscles could be identified in the permanent performance threshold range of the HKS. Therefore, aEA is also considered to be an indicator of general stress that can define the permanent performance threshold range of the HKS more completely. The reduction in aEA is probably an expression of a negative feedback from the cardiovascular system to the neuromuscular system induced by the central nervous system.

[Encounter with the so-called Munchausen syndrome in the psychiatric clinic].

The term "Münchhausen's syndrome" comprises a group of patients characterised by habitual presentation for hospital treatment, mostly by means of a dramatic situation of an apparent acute illness caused by an either simulated or artificially produced disorder. The authors discuss a case that had been presented to them, and turn their attention to the problems of pathology, psychodynamics and differential diagnosis connected with this particular patient. Diagnostic classification may differ widely, depending on the individual case. In this particular woman patient the authors tend to diagnose, more likely than not, a borderline personality disturbance as defined by the Diagnostic and Statistical Manual (DSM III) of the American Psychiatric Association, this diagnosis being made mainly on the grounds of psychodynamic deliberations.

[New derivatives of 10,11-dihydro-5H-dibenz[b,f]azepine with antiarrhythmic action].

After an introduction about the importance of the 10, 11-Dihydro-5H-dibenz[b,f]azepine system for the drug research and according to experiences about the change of pharmacodynamic effects in the field of the phenothiazine bases by dialkylaminoacyl substitution in the following report some new 3-carbalkoxyamino-5-omega-aminoacyl-10,11-dihydro-5H-dibenz[b,f]++ +azepines and their intermediates for synthesis are described. In result of pharmacological investigations which showed antiarrhythmic activities the substance 3-carbethoxyamino-5-dimethylaminoacetyl-10,11-Dihydro-5H-dib enz[b, f]azepine X HCl (GS 015, AWD 19-166, Bonnecor) was selected for clinical tests. For studying the biotransformation of this substance the expected main metabolites were prepared by chemical synthesis.

[Antiarrhythmic effectiveness of 3-carbalkoxyamino-5-(omega-aminoacyl)-10,11-dihydro-5H-dibenz-[b,f]- azepines].

The derivatives of a novel structure series of dibenzazepines dispose of intense antiarrhythmic properties. The relations between structure and effect in comparison with the antiarrhythmically active derivatives of phenothiazine (Ethmozine) are discussed. When substituting the beta-aminopropionyl chain with cyclic residue by means of a dimethylaminoacyl chain there appears a marked antifibrillatory action besides of the intense antiarrhythmic one. The compound 17, the 3-carbethoxyamino-5-dimethylaminoacetyl-dibenzazepine, proved to be the most efficacious compound in the course of the basic screening on two models: action on the effective refractory period in the rabbit's atrium and aconitin-induced arrhythmia in the conscious rat. In comparison with Ethmozin, an antiarrhythmic agent of the phenothiazine type, 17 shows a somewhat lower efficacy in case of i.v. application, but a distinctly intenser one was stated after oral administration. A profound test on the models: two-step coronary ligature in the dog according to Harris and electrofibrillation in the cat's heart, revealed an equally intense antiarrhythmic action but a considerably intenser antifibrillatory one. Therefore the compound 17 (abbreviated designation in the USSR: GS 015 or in the GDR: AWD 19-166) was provided for a thorough pharmacological and toxcological study.

[The inhibition of various bacterial proteinases with natural proteinase inhibitiors(author's transl)].

Proteus mirabilis, citrobacter intermedius, staphylococcus aureus haemolyticus and staphylococcus albus produce extracellualr proteinases with maximum activity at pH 7.8. These proteinases are inhibited practically completely by human and animal blood serum. This inhibitory activity of human serum is caused by alpha2-macroglobulin. Ovomucoid showed nearly the same inhibitory activity as alpha2-macroglobulin.
