Regional genetic differentiation of Phlebotomus sergenti in three Moroccan foci of cutaneous leishmaniasis caused by Leishmania tropica.

Phlebotomus sergenti was identified morphologically in samples from three Moroccan foci of leishmaniasis caused by Leishmania tropica in the provinces of Azilal, Essaouira and Taza. Three primary mitochondrial DNA lineages were identified, and they could be markers for regionally distributed cryptic species. Greater mitochondrial diversity in Azilal indicated that this central province could have been the origin of dispersal of P. sergenti or the zone of secondary contact. All except one of the 21 mitochondrial haplotypes showed a marked regional distribution, and this indicates that vector control would not always be followed by rapid, long-distance reinvasion. Only mitochondrial haplotype SER18 was a putative marker for long-distance dispersal, for which there is no evidence of human assistance.

Treatment of renal colic by desmopressin intranasal spray and diclofenac sodium.

The vasopressin analogue, 1-desamino-8-arginine vasopressin (desmopressin), is a potent antidiuretic without the pressor effects of vasopressin. A total of 18 patients with acute renal colic due to stone disease received 40 microgramsf1p4mopressin intranasal spray with encouraging results. There was a significant decrease in the colic pain intensity from an initial mean visual analogue score of 67 +/- 17 mm. to 39 +/- 36 mm. within 30 minutes (p < 0.001). Eight patients (44.4%) had complete pain relief within 30 minutes of administering intranasal desmopressin spray. Nine of 10 patients who required intramuscular diclofenac sodium achieved complete pain relief within another 30 minutes. In other words, when intranasal desmopressin spray was administered before diclofenac sodium, 94.4% of the patients achieved complete pain relief and were discharged home. The mechanism of analgesic action of desmopressin in renal colic is uncertain. At the peripheral level, desmopressin may alleviate the acute renal colic through its potent antidiuretic effect or by relaxing the renal pelvic and ureteral smooth muscles. The central analgesic effect of desmopressin by stimulating the release of the hypothalamic beta-endorphin is proposed. We conclude that intranasal desmopressin spray can be used successfully in the treatment of renal colic. It may also replace prostaglandin synthetase inhibitors in treating renal colic with the advantage of avoiding the potential side effects. Further studies are needed to investigate whether the combination of desmopressin with analgesics or spasmolytic drugs offers competitive results compared with those achieved by prostaglandin synthetase inhibitors in the treatment of renal colic.

Treatment of renal colic by prostaglandin synthetase inhibitors and avafortan (analgesic antispasmodic).

In a study of the pain-relieving effect of 3 drugs commonly used to treat acute renal colic in this hospital, intravenous indomethacin and intramuscular diclofenac (prostaglandin synthetase inhibitors) were compared with intravenous Avafortan (analgesic antispasmodic). As first-line analgesics, prostaglandin synthetase inhibitors, if given intravenously, offer an effective alternative to Avafortan. Of 145 patients studied, 32 required a second injection for complete relief of pain. Administering a second dose of prostaglandin synthetase inhibitors resulted in equally significant pain relief rate even though the route was intramuscular.

Structure-function relationship of human spinal ligaments.

A combination of light, scanning and transmission electron microscopy was used to investigate the morphology and ultrastructure of normal human spinal ligaments sampled from adult surgical specimens. The ligamenta flava consist mostly of dense elastic fibers, whereas the supraspinous and interspinous ligaments are preponderantly collagenous. In all ligaments, the collagen fascicles are characterized by a regular crimp structure. The inner collagen fibers of interspinous ligaments tend to be oriented parallel to the spinous processes while those of the peripheral layers run in postero-cranial direction. The presence of proteoglycan filaments is clearly demonstrated in all of the ligaments examined. They are mainly located at the d band of the collagen fibrils. These findings are discussed in relation to the function of the posterior ligamentous system. It is suggested that the interspinous ligaments are able to transmit tension from the thoracolumbar fascia to the spine. Finally, the spinal ligaments are thought to be involved in the control mechanism of the spine.

Degeneration of the human lumbar spine ligaments. An ultrastructural study.

The fine structural alterations in human lumbar spine ligaments were studied in tissue samples obtained from 8 young patients operated upon for idiopathic scoliosis and from 10 adult patients operated upon for herniated discs. The ultrastructure of the scoliotic ligaments was found to be normal. The majority of cells encountered were the fibroblastic-like cells. The collagen-proteoglycans interaction was similar to that described in other normal soft tissues. However, two forms of degenerative changes were demonstrated in posterior ligaments of patients with herniated discs. Metaplasia of ligamentous tissue into fibrocartilage was a common finding. The fibroblasts were replaced by chondrocytes. A few cells which had suffered necrosis were found. Alterations in the collagen-proteoglycans arrangement were also evidenced. This form of degeneration resembled that commonly associated with wear and tear phenomena. A second form of degeneration due to ageing was found to coexist with the aforementioned alterations.

A light and electron microscopic study of spinal ligament innervation.

The innervation of the posterior ligamentous structures of the human lumbar spine was studied by light microscopy, scanning electron microscopy and transmission electron microscopy. Three types of nerve endings were recognized: free nerve endings, Paciniform corpuscles and Ruffini corpuscles. The free nerve endings, which are thought to act as nociceptors, were found in the superficial layers of all ligaments. A few free nerve endings were also identified within the supraspinous and interspinous ligaments. The Paciniform corpuscles were predominantly found in the supraspinous ligament. The Ruffini corpuscles were located in the interspinous and flaval ligaments. These findings suggest that the posterior ligamentous structures could be involved in the spinal control system.

Alpha heavy chain disease (report of 18 cases from Iraq).

The clinical and pathological features of 18 new patients with alpha heavy chain disease seen at two referral centres in Baghdad, Iraq, are described. The series included 14 males and four females ranging in age from 14 to 47 years. Almost all patients presented because of long-standing abdominal pain and diarrhoea. The tissue diagnosis and extent of the disease were established at laparotomy in most patients. Peroral jejunal biospy was used in a number of patients, mainly for follow-up. The serological abnormality was confirmed by immunoselection technique. Most of the patients had extensive thickening of the bowel wall and/or tumour masses of the small intestine and mesenteric nodes. Histopathological sections showed muscularis. Preliminary results of the treatment, including two long remissions, are reported. In general, our observations agree with those made by other authors, mostly from the Middle East and Africa. We believe that a high index of clinical suspicion, routine use of the immunoselection, and recognition of the early pathological changes may hopefully lead to the detection of more cases before the frank neoplastic phase of the disease.
