The effect of indomethacin on prostaglandin E2 in human cornea and conjunctiva.

We studied the inhibitory effect of topical 1% indomethacin aqueous suspension on prostaglandin E2 in 20 conjunctivae extracted during cataract surgery and 16 corneas extracted during penetrating keratoplasty. Thirty minutes before each surgical procedure, indomethacin or placebo (0.9% sodium chloride solution) was instilled into the eye to be operated on in a masked fashion for conjunctiva and selectively for cornea. The excised corneal buttons and conjunctival tissue were homogenized and assayed for prostaglandin E2 concentration using radioimmunoassay. The levels of prostaglandin E2 in the conjunctiva significantly decreased (P less than .0033) after administration of indomethacin (203.7 +/- 219.5 pg/mg) compared with the placebo-treated tissues (849.8 +/- 564.1 pg/mg). The levels of prostaglandin E2 in the corneas significantly decreased (P less than .0157) after administration of indomethacin (522.5 +/- 570.2 pg in the cornea) compared with the placebo-treated tissues (1636.3 +/- 1344.1 pg in the cornea). Indomethacin may serve as a potent anti-inflammatory agent in conjunctival and corneal inflammatory disorders.

Extreme deepening of the anterior chamber during phacoemulsification.

An unusual deepening of the anterior chamber during a phacoemulsification procedure was observed in 3 patients with cataracts: 1 with high myopia, 1 with a lens opacification following a vitrectomy procedure, and 1 with pseudoexfoliation syndrome. This deepening hindered emulsification of the nucleus in the posterior chamber. Early delivery of the nucleus from the capsular bag allowed phacoemulsification to proceed at the pupillary level. The "sinking" of the lens toward the vitreous cavity appears to be related to a lack of vitreous pressure due to liquefaction, or to loosening of the zonular support in the pseudoexfoliation syndrome.

Microsurgery of the retina with a needle-guided 193-nm excimer laser.

This article presents a method used to guide the beam from an argon fluoride excimer laser to make it suitable for microsurgical purposes and confine it to areas that can be varied in dimension from 1 micron to tens or hundreds of microns. This approach guides the excimer laser beam with an articulated mechanical arm and confines it with variable-diameter tapered tubes, possibly allowing the use of this laser in in vitro retinal surgery with endolaser techniques. Currently, because of the lack of a delivery and focusing system for the 193-nm argon fluoride beam and its absorption by biologic liquids, this laser is used exclusively in ophthalmology for topical applications, such as corneal sculpting. This new method resolves these problems in a unique way with impressive results. Specifically, it was shown that, with this needle-guided excimer laser, it is possible to remove retinal tissue accurately without detectable damage to surrounding cells. Applications of this new technique in retinal surgery are discussed.

The effect of topical administration of indomethacin on symptoms in corneal scars and edema.

We conducted a masked randomized study of 50 patients to evaluate the effect of administration of topical indomethacin 1% suspension on symptoms in corneal scars, edema, infiltrates, and erosions. Patients with symptoms (photophobia, pain, itching, burning sensation, foreign-body sensation, and tearing) were treated with topically administered indomethacin 1% or placebo and monitored for eight weeks. The severity of the complaints was rated and the scores were evaluated (Wilcoxon rank-sum test). Of the 25 patients treated with indomethacin, 21 (84%) had improvement in symptoms and the severity of each of the symptoms was significantly decreased. Of the 25 patients treated with placebo, one (4%) had improvement in symptoms without statistical change of the severity of the symptoms. However, when the placebo-treated patients received indomethacin drops, the symptoms were significantly decreased (P less than .002). This study suggests that topical administration of indomethacin 1% may reduce ocular symptoms in patients with corneal scars, edema, or erosions.

Assessment of intraocular lens power on the basis of precataract refractive measurements.

Assessment of intraocular lens (IOL) power may rely, when biometric measurements of the eye are not available, on refraction previous to cataract development. Such an assessment, however, is generally considered unreliable. To improve the predictability of this method, we correlated refraction measurements with predictions of IOL power. In contrast to previous studies which used retrospective assessment of precataract refraction, this study included only noncataractous eyes and correlated the direct refractive measurements obtained with hypothetical IOL power values. These values were calculated by placing axial length and corneal power measurements of the same eyes in the SRK formula. We conclude that the regression formula obtained may improve the clinical judgment required for predicting lens implant power in cataractous eyes, on the basis of precataract refractive measurements.

[Age-related macular degeneration after retinal detachment surgery].

We studied a group of 157 patients who had retinal detachment surgery in one eye with successful anatomical reattachment and a fellow eye without a retinal detachment. In a group of 7 out of 8 patients who had a retinal detachment that included the macula and who underwent surgery, we found that the development of macular degenerative changes was much less pronounced in the operated eye than in the fellow, control eye. In a group of 3 patients who had retinal detachment without macular involvement, the appearance of macular degenerative changes appeared to be rather symmetric in the operated and in the fellow eye. The presence of a circling buckle did not appear to have an influence on the ARMD changes. It appears that the presence of a macular detachment is a factor in modifying the natural history of age-related macular degeneration. The diminution of signs of ARMD could be related to a degree of atrophy in the retinal pigment epithelium occurring after a retinal detachment with the macula off.

Oculographic automatic perimetry in glaucoma visual field screening: a clinical study--preliminary results in glaucoma patients.

An automatic method for visual field screening using oculographic potentials have been evaluated on 12 eyes of 65 glaucoma patients. The oculographic perimeter records, plotting the central 22 degrees of the visual field, have been compared with Armaly's manual selective perimetry performed by the Goldman Perimeter. In spite of different testing standards of both methods, equivalent pathological field defects were demonstrated in 72% of the eyes following the first examination. Successive plottings with the automatic method improved the results considerably. The method has proved to be a reliable objective automatic test. Further development is suggested through which the procedure might be useful for glaucoma screening in the future.

Diurnal changes of the intraocular pressure of patients with angle-closure glaucoma.

The diurnal curves of 21 patients (40 eyes) with angle-closure glaucoma and of 21 normotensive subjects (42 eyes) were studied. The patients with angle-closure glaucoma had a V-shaped diurnal curve that was completely different from the diurnal curve of the normotensive subjects. Most of the eyes of the patients with angle-closure glaucoma had a diurnal change of the intraocular pressure of 8 mmHg or more, while in the normotensive subjects only 1 eye had a diurnal change as large as 8 mmHg.

Automatic testing of the visual field using electro-oculographic potentials.

The central visual field is tested using static perimetry on a tangent screen located 1 m from the patient. During the test electro-oculographic (EOG) potentials, associated with eye movements, are recorded. For this purpose two pairs of electrodes are placed in such a way that one pair records the vertical and the other the horizontal component of the EOG potential. The EOG signals that are produced when the tested eye moves toward the light target on the screen are digitized and fed into a computer programmed to relate each pair of signals to the specific lamp on the tangent screen. The results of the test appear on a computer-printed chart that shows the distribution of the lamps on the testing board together with the information whether the lamp was seen by the subject. This method of visual field testing is both objective and automatic.

Fluorescein pupillary flow in aphakics with intact and spontaneous openings of the vitreous face.

The passage of fluorescein dye, injected into the general circulation, from the posterior to the anterior chamber was studied in 60 selected aphakic eyes between 2 days and 4 weeks after an uneventful cataract extraction. In a group of 22 eyes with intact anterior hyaloid, fluorescein was seen to pass only through the pupillary margin, mostly within 20 seconds of the beginning of the eye injection. In 24 eyes with rents in the anterior hyaloid the dye was seen percolating only through the vitreous face openings. No fluorescein passed through the pupillary margin or the peripheral iridectomy in this group of eyes. The appearance time of the dye was delayed to about twice the normal appearance time. In 8 eyes with rents in the anterior hyaloid the dye passed only through the pupillary margin. In 4 eyes with anterior vitreous rents the fluorescein appeared through the anterior vitreous face opening. From the analysis of the data it appears that the spontaneous breaks in the anterior vitreous face may represent a 'self-c,re' of pupillary and iridectomy block by the vitreous in aphakics in a high proportion of cases. It is our impression that 3 peripheral iridectomies and a very tight closure of the wound can prevent the complication of spontaneous breaks of the vitreous face.

Cataract Extraction

Argon laser application to the angle structures in the glaucomas.

The argon laser beams' effect on angle structures was studied in 20 uncontrolled glaucomatous eyes. Nineteen eyes became hypotensive quickly, but in 15, the elevated tension returned during the 20 to 40 day follow-up period. Four eyes sustained low pressures throughout the ten-month follow-up period. Two were of babies with congenital glaucoma, one had simple glaucoma following an unsuccessful filtering operation, and one was aphakic following congenital cataract extraction. Ten eyes were medically controlled postoperatively, and six were failures that had to undergo further surgery. Outflow facility measurements revealed an increase in C values during the hypotensive period. The C values were reduced later, with the decrease corresponding to the increase in intraocular pressures. The hypotensive effect together with the increase in outflow facility was probably related to laser-induced openings in the angle structures. The varying results at the later stages depended on whether or not these openings remained opened, or became partially or completely closed during healing.

Experimental cleavage of chorioretinal scars by traction and subretinal fluid.

The resistance to separation by localized traction and exudative retinal detachment of experimental chorioretinal scars produced with argon laser photocoagulation, cryothermy and diathermy was tested in rabbits. In vivo, the area of retinal separation over the scar was similar to that obtained by traction on normal retina. Moreover, the retinal cleavage occurred at the level of the scar regardless of the scar inducing modality, indicating that the scar is the weakest point in the retina-pigment epithelium joint. However, spaced and dense chorioretinal scars produced by argon laser photocoagulation, diathermy and cryothermy usually remained attached after experimental production of exudative retinal detachments with penicillin G intravitreal injections. This study indicates that the resistance to detachment of the normal retina by traction may be as strong as that of the scarred tissue and that the chorioretinal scars' effect is related to lesser fluid penetration into the treated area.

Identification of copper ions in aqueous and vitreous of eyes containing copper and iron foreign bodies.

The reliability of aqueous analysis by the atomic absorption spectrophotometer for copper ion content in samples from eyes containing intravitreal pure copper particles was investigated. It was demonstrated that values of copper ions rose well above the normal in the aqueous as well as in the vitreous of such eyes. The standard deviations of the mean values were generally high. It was also found that a similar copper ion content increase occurred in eyes containing iron particles. The possible clinical implications of these findings are discussed.
