[Cementing of small osteochondral fragments in hand surgery using a fibrin glue--clinical experiences].

A short historical review of the development of a new fibrinogen adhesive system, consisting of highly concentrated fibrinogen, thrombin, and factor XIII, is given. Small osteochondral fragments are well fixed with this system. This was demonstrated in five cases with good success, the fragments being early revascularized. Some of these patients have had severe damage to the finger joint surface. The problems of a prematured fibrinolysis were discussed.

[Results of the surgical treatment of bridge callus through silastic membrane interposition].

A report on 5 cases with successful treatment of a posttraumatic synostosis of radius and ulna by resection and interposition of a silastic-membrane is presented. This membrane has to ensheath one of the forearm bones for at least half of its circumference and it is fixed partially with Dexon sutures. During operation it is necessary to confirm this fixation of the membrane, which must be unchanged after rotation of the forearm. The range of movement reached 4 years after operation was about 120 degrees (100-130 degrees) on an average. A better result can only be expected by an osteotomy of the radius and correction of its position.

[Corrective osteotomies of the head of the tibia (indications, technique and results) (author's transl)].

The best indication for corrective osteotomy of the head of the tibia is unilateral osteoarthrosis of the knee joint in which the apical angle of the deformity lies in the head of the tibia or in the joint. Full-length radiographs of the leg are indispensable to determine the level of the osteotomy and the angle of correction required. The indications for high openwedge osteotomy of the tibial condyles and high osteotomy of the head of the tibia are discussed, and some of the technical details of the osteotomy and its fixation are described. Between 1970 and 1977, 139 corrective osteotomies of the head of the tibia were carried out in our clinic. Seventy nine adult patients with 100 operated knees were followed up approximately 3.5 years after operation. In 80% there was an improvement with regard to pain and walking distance when compared with the praeoperative state. The results depend on the kind of deformity (varus deformities generally show a better result) and the seriousness and duration of pre-existing osteoarthrosis and laxity of the ligaments.

[Fibrin glue and bone regeneration].

The osteoinductive property of fibrin glue with and without admixture of aprotinin was proven in animal model. Aprotinin as an inhibitor of the fibrinolysis is supposed to be an inhibitor of the osteogenesis, too. Three holes of 4 mm diameter and 2 mm depth were placed into the diaphysis of both femura in 12 adults dogs. The defects were filled with either pure fibrin glue or with glue containing aprotinin (3000 KIE), or with nothing (vacant). After 8 weeks the quantity of the new built woven-bone was examined by plane geometry and the "Bone Metabolising Unit (BMU)" (Frost) which are crossing the border of lamellar bone and new woven bone were counted out. There was seen no statistical significantly between the three groups neither in the quantity of new bone nor in the BMU. Therefore the fibrin glue has no osteoinductive property.

[Mechanics of the perilunate carpal bones and their injury patterns including post-traumatic instability with reference to rare clinical cases].

There is given a survey of recent findings of the biomechanics of the carpal bones of which the complex of the os scaphoideum, lunatum and capitatum has the key role. The mechanism of injury and posttraumatic instability of the wrist are presented. Rare injuries like subluxation of the os scaphoideum with DISI instability, "scapho-capitate fracture" syndrome, and perilunar dislocation associated with scaphoid fracture (de Quervain) with a proximal fragment of the scaphoid which is turned over about 180 and 90 degrees, are reported in form of clinical cases. In case of subluxation of the os scaphoideum the operation described by Dobyns is recommended, using a split of ECRB tendon for dorsal ligament reconstruction.
