Transformation of ameloblastic fibroma to fibrosarcoma.

The direct transformation of an ameloblastic fibroma into a fibrosarcoma in a 16-year-old Caucasian male is reported. Although no ameloblastic epithelium was found in the recurrent tumor, the odontogenic origin of the fibrosarcoma was evident. The ameloblastic fibrosarcoma and the fibrosarcoma of identical odontogenic origin represent an entity which should be distinguished from conventional fibrosarcoma as these tumors demonstrate different clinical behaviors.

[Cytological detection of a myeloic metaplasia in punctates of transplanted kidneys and their diagnostic importance (author's transl)].

In 30 patients who had undergone renal transplantations, 45 renal biopsies were carried through for a suspected acute immuno-incompatibility (rejection) during the period from 1974-1975. Of all bioptic material taken swab-preparations were made at the same time, aiming at clarifying the cytomorphological criteria of the acute transplant rejection. In 14 out of 27 cases where an acute transplant rejection was histologically diagnosed, myelocytes and normoblasts were found in the smears. In one out of two chronic cases of transplant rejection the detection of metaplasias was negative. Thus, the detection of myeloic metaplasias in renal punctates is an important criterium for the cytodiagnostics of a transplant rejection crisis.

[Early lesions of the human kidney in paraproteinemie an electron microscopic study (author's transl)].

INTRODUCTION: In paraproteinemia, especially in multiple myeloma, the origin and morphology of lesions in the kidneys still remain controversial. In many studies mostly of tissue obtained post mortem and examined with light microscopy, a discrepancy between impairment of renal function and minimal pathomorphological findings is often striking. The few studies done with the electron microscope up to now present a wide variety of morphological findings probably due to the different stages of kidney lesions examined, generally in patients with renal insufficiency and nephrotic syndrome. We have therefore tested the question whether early lesions are detectable in the nephron of patients with paraproteinemia who do not show any impairment of renal function. MATERIAL AND METHOD: Kidney biopsies were obtained from 8 patients with an average age of 58 years. 6 had multiple myeloma (3 IgG-, I IgA-, 2 Bence-Jones plasmacytoma), 1 Waldenström's disease and 1 an idiopathic paraproteinemia. Confirmation of the diagnoses was obtained by immunoelectrophoresis, bone marrow biopsy, and sternal puncture. In all cases creatinine clearance was normal and proteinuria was not increased substantially even in patients with a slight Bence-Jones-proteinuria. For electron microscopy small pieces from the renal biopsies were fixed in aldehydes followed by osmium tetroxide and embedded in Epon. [Obliterative intimofibrosis of the renal arteries under the influence of hemodialysis in patients with chronic renal insufficiency (author's transl)].

Histologic and morphologic methods were employed to study the influence of chronic hemodialysis on kidney vessels in chronic renal insufficiency. Arteries of contracted kidneys from patients with and without hemodialysis treatment were investigated. The dialysis group was made up of 33 patients, 28 having undergone bilateral nephrectomy and 5 having died. The control group consisted of 21 patients with chronic renal insufficiency, who died in uremic coma without prior hemodialysis. A statistical evaluation was done by comparing measurements from corresponding arteries in the dialysis- and control groups. The correlation pattern from a BMD 03D-program, in which each group was separately assessed for the possible influence of various clinical findings, was determined. Clinical influences taken into account included the course of the kidney disease, grade of renal insufficiency, duration and degree of hypertension as affecting the renal arteries. The statistical results showed that hemodialysis treatment, even taking clinical data into consideration, influenced the development of intimal fibrosis in the arteries of contracted kidneys in an increasing positive manner. Decreased perfusion of the kidneys during hemodialysis suggested as a possible cause. The examination of early lesions in renal arteries following short-term dialysis treatment lends support to this possibility. Here edema and proliferation of the intimal cells in the arteries, similar to that in vessels having a reduced blood flow, is observed.

[Diagnostic value of cytological investigations in metastatic tumours of the liver: comparison of laparoscopy, histology, and cytology (author's transl)].

Fine needle aspiration biopsy is generally a harmless method if contraindications are considered and provides sufficient tissue for cytological evaluation. The diagnostic value is very high: in 27 cases of metastatic tumours of the liver diagnosed by laparoscopy the diagnosis was confirmed in 26 cases and revised in one case. In a control group of 200 patients with various diffuse liver diseases or morphologically normal findings no false positive results were found. In no case were post-biopsy haemorrhage or other complications observed. Needle biopsy and cytological evaluation should be used with an increased frequency when a malignant tumour of the liver is suspected clinically. After primary localization of suspicious areas by ultrasound or scanning the pick-up rate is high. Laparoscopy with precisely aimed fine needle biopsy is the method for verifying the diagnosis in cases with cytologically negative results which is up to 100% accurate.

[Cytodiagnosis of central nervous systems tumors: comparative study of cytological (CSF) and histological results (author's transl)].

From a group of 52 tumours of the CNS it was possible to make a study of 35 (10 primary and 25 metastatic) to see if the nature of the neoplasm could be established from an examination of the cells of the CSF, comparing the results with those obtained by histological examination of the tumour tissues. There are very few dependable criteria to be found in the cells sedimented from the CSF (secretory activity; degree of differentiation) so, generally, only a determination as to whether the tumour is primary or metastatic, and if epithelial or mesodermal, can be made. Organ specificity can be identified only as regards the metastatic hypernephromas and thyroid carcinomas.

Studies of liver function and structure in patients with gallstones before and during treatment with chenodeoxycholic acid.

Twelve patients with gallstones were treated with chenodeoxycholic acid (CDCA) for 3 to 22 months. Liver function and structure were studied before and during treatment. No signs of hepatotoxicity were observed. Activities of several enzymes, including transaminases, GLDH, and gamma-GT were frequently abnormal in this group of patients. But there was no statistical difference between treatment and pre-treatment levels. Serum bilirubin and protein electrophoresis remained normal throughout the study. The BSP retention test was not significantly altered by CDCA treatment. Light microscopic examination in 8 patients revealed no changes of liver structure during 3 to 19 months of CDCA administration except, however, for a decrease of fatty changes in 3 patients. The ultrastructure of liver cells was found to be normal in 5 patients, in whom electron microscopy was done as well.
