[Postmortem diagnosis of hypertension].

The postmortem diagnosis of hypertension is of importance both for the determination of basic disease or terminal cause of death and for statistical investigations based on autopsy material or for comparisons between clinical and epidemiological data as well. The analysis of our autopsy records revealed: 1. The most important macroscopical findings are an increased weight of the heart, an arteriosclerosis of the peripheral extracerebral arteries of the brain, and an adenomatous hyperplasia or adenomas of the suprarenal cortex, they should be looked for at autopsy. 2. A microscopical finding of a hypertension is in the first place the arteriolosclerosis of kidneys followed by that of the pancreas, myocardium, liver, and brain. In practice, a specimen of kidney and pancreas should be investigated histologically in each case of a possible hypertension.

[Pathogenesis, clinical course and morphology of peliosis].

On the basis of two cases and a survey of literature the syndrome of the peliosis of liver, spleen and bone marrow is described and its pathogenetic factors are demonstrated. For peliosis is a syndrome which is characterized by multiple cavities filled with blood in the parenchyma. It is established that the peliosis has changed from very rare autopsy findings in a prognostically unfavorable basic disease has changed to a clinically significant and therapeutically controllable complication of the hormone therapy.

Muscular changes in carcinomas of the breast.

14 samples of the Musculus pectoralis major from patients with early stages of breast cancer and 3 samples from patients with nonmalignant mammary diseases were stored in liquid nitrogen and investigated histologically, histochemically and histometrically. Main changes are lipomatosis, fibre necrosis, occasionally perifascicular atrophy, metabolic changes (e.g. accumulation of lipids), and alterations of the activity of oxidative enzymes. Histometrically we found an atrophy of type 2-fibres. There may be relations between the degree of the muscle changes and the severity of the tumour disease. The paraneoplastic muscle disease is a "concomitant myopathy" and may be regarded as a "functional myopathy". In its development we can differentiate a phase of compensation and a phase of decompensation.

[The evaluation of senile fractures of the proximal femur as cause of death (author's transl)].

The analysis of 159 patients who had died of senile fractures of the proximal femur revealed, that the fracture must not be regarded as fatal basic disease (cause of death) in all cases, but only in about half of them. Various basic diseases, deuteropathies and complications are dealt with providing a certain "mosaic" of fatal factors. In cases with senile fractures it is the polypathy ("multiple pathology") that has to be taken into consideration when determining the cause of death.

[The consequences of fibrosis of spleen capsule and perisplenitis cartilaginea for the postmortem diagnosis of hypertension and cor pulmonale (author's transl)].

The statistical investigation of fibrosis of the spleen capsule and of perisplenitis cartilaginea in 444 consecutive autopsy cases revealed a significant correlation between the changes of the spleen capsule and cor pulmonale, but no significant correlation to hypertension was detected. Therefore, fibrosis of the spleen capsule and perisplenitis cartilaginea can be used as a criterion for the postmortem diagnosis of cor pulmonale.

[Experience with the one-way aspiration curette (Keradenta)].

An aspiration-curette sucking endometrium from the uterine cavity for diagnostic purposes is described. The curette is available as sterilised embaled one-way set, consisting of metal probe, tissue collecting basin, label and transportation bag. By means of this curette in 100 cases of various gynecological diagnoses aspirations from the uterine cavity were performed. The tissue was investigated histologically and compared with that gained by traditional curettage. It was found that by means of suction-curettage an exact histological diagnosis is possible. The one-way curette offers several advantages, especially an intervention without anaesthesia and under ambulant conditions. Complications are not to be expected.

[The evaluation of postomortem heart weight (author's transl)].

In 1974 we published the "method of differentiated relative heart weight" for the evaluation of the postmortem heart weight. It is not the simple relation of heart to body weight of 1/200 that is used, but--depending on the body weight--1/175 for normal weight, 1/150 for underweight and 1/200 for overweight. This method is now submitted a test by means of 218 exactly diagnosed cases and the heart weights are compared with the mean values of Linzbach. If the weight of a heart is lower than the weight estimated by our method one can conclude, that the patient did not suffer from hypertension and/or from cor pulmonale. If the heart weight is higher than the estimated value a pathological heart weight is to be supposed, but normotension cannot be excluded, especially in women. In those cases the comparison of the heart weight with the mean values of Linzbach is recommended.

[The evaluation of organ weights by means of multiple regression equations (author's transl)].

The investigation is based on the weights of 570 livers, 648 spleens, 200 right and left lungs, 559 right and 556 left kidneys of 364 male and 360 female persons over 20 years of age being autopsied in our Institute from 1969 to 1972. In both sexes these weights were set in relation to age, body weight, body height, breast width, breast depth and circumference of chest. By the application of higher statistical methods and under computerusage it was made possible to state, that there is a marked relationship between the organ weight on one hand and the body weight and in women the body height on the other. The relationship between the organ weight and various constitutional sizes and age is less marked. In practice these relations should be considered. This can be done by multiple regression equations, which enables us to evaluate the weight of spleen, lungs, kidneys and female livers, but not the weight of the male liver. It must be considered, that the estimated weight of spleen and kidney of both sexes is more inaccurate than that of the lungs of both sexes and of the female livers. The multiple regression equations are tabulated for both sexes (table 4).

[The evaluation of the postmortem body weight with special reference to the optimal weight (author's transl)].

The investigation was based on 405 men and 482 women. In all cases the optimal weight depending on the metric index and the body weight estimated by means of the Brocaindex was calculated. The comparison of all data revealed the following: 1. Persons deceased have always a lower body weight than living persons of equal age, constitution and sex independant of the method used. That means that diagnosis of generalized adipositas is more difficult in persons autopsied than in persons living. 2. The calculation of the optimal weight and of the Brocaindex does not yield the same results. In the majority of the cases the optimal weight is lower. Therefore, the pathologist must know which method of evaluation of body weight the clinician used. 3. The optimal weight depends on the constitution. This applies to persons deceased as well as living. 4. There is a statistical correlation between postmortem optimal weight and basic disease. For the dicision whether generalized adipositas or extreme cachexia being the cause of death the optimal weight should be considered.

[Postmortem evaluation of constitution types in relation to the metric index (author's transl)].

The comparison of the metric index (by Strömgren) and the Brugsch-index revealed the following: 1. In the present time the frequency of the various constitution types differs from that during the 2nd world war (by Selberg). In men as well as in women pyknics are most frequent. 2. The frequency of the constitution types of living persons - determined by the metric index - corresponds to that of persons autopsied. Therefore the metric index may be applied to persons deceased. 3. A linear statistical correlation exists between the Brugsch- and the metric index. 4. Both indices show correlations to the basic disease: Patients with a malignant tumour are much more often of athletic than of leptosome or pyknic constitution. Persons with arteriosclerotic and hypertensive diseases as a cause of death are more often of pyknic constitution. In practice not only one but several indices should be considered.
