[Chronic alveolar hypoventilation in obstructive sleep apnea syndrome].

UNLABELLED: Among 30 patients with obstructive sleep (OSA) syndrome in 15% (50%) chronic alveolar hypoventilation developed; chronic hypercapnia was found in patients with mildly (7), moderately (2) and severely (6 patients) impaired lung ventilatory function. All patients with chronic hypercapnia were morbidly obese (BMI 44 +/- 12). Nocturnal polysomnography showed similar mean apnea duration in the groups of hypercapnic and eucapnic patients, longer duration of the longest apneas in hypercapnia (88 +/- 42 s) than in eucapnia (38 +/- 19 s), p < 0.01 and more severe desaturation during sleep (the mean of the lowest SaO2 during apneas 0.46 +/- 0.17 mol/mol) in hypercapnic and 0.71 +/- 0.11 mol/mol in eucapnic patients, p < 0.001). Polycythemia was found in 14 patients, exclusively in hypercapnia. CONCLUSIONS: Chronic alveolar hypoventilation develops in some obese patients with impaired-even mildly or moderately-lung ventilatory function and with the most severe OSA syndrome, in which apneas may occasionally be extremely long and lead to very low saturation during sleep. Polycythemia occurs in most patients with chronic alveolar hypoventilation and OSA syndrome.

[Early results of chemotherapy in patients with non-small cell lung cancer].

59 patients with non-small cell lung cancer were treated with cisplatin (60 mg/m2) on day 1 and etoposide (60 mg/m2) on days 1-5. The results of the treatment were evaluated after 4th course. Remissions were seen in 27 patients, 17 patients had stabilization and 15-progression. The response rate in stages I and II was 70%, in stage III-40% and in stage IV-23%. In squamous cell carcinoma the response rate was 55%, in large cell carcinoma-25% and in adenocarcinoma-12%.

[Two cases of tuberculous pericarditis--diagnostic difficulties].

Some difficulties etiological diagnosis were presented in an example of two clinical cases. In the first patient with exudative pericarditis tuberculosis was determined in histopathological examination of lymph node (caseous necrosis). After antituberculotic treatment the majority of clinical symptoms was disappeared. In the second patient with chronic constrictive pericarditis the tuberculous etiology was accepted according to highly positive tuberculin reaction and signs of pericardial calcification. Patient was qualified to pericardiectomy.

[Castleman tumor].

A case of a mediastinal benign tumor in a 56 year old male is presented. The tumor was detected during a routine chest radiographic examination. The patient was asymptomatic. During thoracotomy two tumors were found. One was localized in the middle mediastinum the other in the posterior mediastinum. Both tumors were removed completely. Basing on the results of the clinical examination, of the laboratory data and the histopathological examination of the resected tumors a diagnosis of Castleman's tumor of the plasma-cell type was made.

[Diagnosis of obstructive sleep apnea syndrome].

Symptoms and signs in 12 patients with severe obstructive sleep apnea (OSA) syndrome have been presented. The most common symptoms were snoring , increased motor activity during sleep and excessive daytime somnolence. The factors predisposing to OSA syndrome were obesity and anatomic abnormalities of the upper airway structure. In some cases the signs of OSA syndrome included hypertension, right heart failure, chronic alveolar hypoventilation and polycythemia. Polysomnography showed sleep fragmentation and the prevalence of light sleep stages. Obstructive sleep apneas repeated 73 +/- 23 times per hour of sleep. The mean apnea duration was 19 +/- 8 s. The mean arterial oxygen saturation during apnea was 72 +/- 14%.
