Treatment of multi-resistant urinary tract infections in patients with multiple sclerosis.

Multi-resistant nosocomial urinary tract infections occurring in thirty patients with neurogenic bladder dysfunction were treated with ciprofloxacin 100 mg twice daily Early bacteriological eradication was obtained in 48.3%, persistence in 34.5% and superinfection in 17.2%. Recurrence, 30 days after initial eradication occurred in 50% while late eradication was maintained in 35% and reinfection with another germ was noted in 15%. Most recurring and reinfecting germs were still susceptible to ciprofloxacin. The drug was well tolerated. The overall outcome was mainly determined by the vesico-urethral dysfunction itself and by the presence of indwelling bladder catheters.

[Ileo-ureteral reflux after urinary diversion through an ileal conduit].

The authors made a comparative study (intravenous urography, loopography and loopomanometry) between patients with inverted antireflux anastomosis, as proposed by Melchior, and 12 patients with classical uretero-ileostomy (Bricker bladder). Inverting the anastomosis results in greater incidence of obstruction, while less complications were seen using the classical technique. The importance of reflux in ileal loops and the validity of the classical and manometry loopogram is discussed. Antireflux procedure is only indicated in cases with dilated ureters.

Serum tissue polypeptide antigen (TPA) as tumor marker for bladder cancer.

Tissue polypeptide antigen is a differentiation and proliferation marker of non-squamous epithelium and derived neoplasms. No reliable tumor markers are available for bladder cancer. The value of tissue polypeptide antigen was therefore prospectively investigated. The serum tissue polypeptide antigen samples were obtained from 144 newly diagnosed transitional cell carcinoma patients and from 92 patients that were followed after treatment. The normal cut off value was defined at 95 units per liter. Nearly all TaT1 patients had normal TPA values, and 80% of the muscle invasive cancers had normal TPA levels. In those patients where TPA was elevated before treatment its monitoring proved to be a reliable predictor of tumor progression. Tissue polypeptide antigen is a useful marker not for the early detection of bladder cancer but for the monitoring of the efficacy of a treatment.
