A retrospective study was carried out on 386 consecutive patients admitted during the period 1975 to 1981 with an abdominal aortic aneurysm; 110 patients (39%) were treated by means of a straight tube graft instead of the more commonly employed bifurcation prosthesis. Its use reduced the operation time by 26 to 31% (p less than 0.001) in both non-ruptured and ruptured aneurysms. Operative blood loss was significantly reduced in ruptured aneurysms only (29%, p less than 0.05). Secondary operations for iliac aneurysm repair were not required in any patient with a straight tube graft during the follow-up period (6 to 60 months). Our experience suggests that, in the absence of significant iliac artery disease, the use of a straight tube graft is the procedure of choice in the treatment of an abdominal aortic aneurysm.
The occurrence of a hepatocellular carcinoma is described in a young woman who had been taking oral contraceptives for 2 1/2 years. She was treated by wedge excision of the tumor. At the time of operation, lymph node metastasis was present. After the operation oral contraceptives were discontinued. No evidence of residual or recurrent tumor could be demonstrated during a follow-up period of 6 years. This is the first report on long-term survival after nonradical resection of metastasized hepatocellular carcinoma associated with the use of birth control pills.
Chronic intestinal ischemia is rare because of the great capacity to form abundant collaterals. The experience with ten patients is discussed and the literature is reviewed. Diagnosis is easily missed because of the lack of specific symptoms. All but one of the patients showed the classical triad (postprandial pain, weight loss, epigastric bruit). The diagnostic value of endoscopy based on remarkable ischemic mucosal findings is stressed. These findings have not been described before. Most of the patients suffered from generalized atherosclerosis and therefore had a high operative risk. There was a considerable postoperative morbidity, but no postoperative mortality. In all patients long-term results were satisfactory. Vascular reconstruction is strictly indicated in case of intestinal angina, not only to diminish severe complaints, but also to prevent acute ischemic infarction. In our opinion there is no indication for prophylactic surgery in case of asymptomatic arterial stenosis or occlusion.

A 37-year-old woman who presented with systemic lupus erythematosus was found to have a membranous obstruction of the inferior vena cava above the hepatic veins, with an extensive collateral circulation. The obstruction was relieved by a renal vein-right atrial interposition graft, and this was followed by disappearance of the anti-nuclear and anti-DNA antibodies. We postulate that the systemic lupus erythematosus might have been provoked by life-long immune stimulation due to bowel-derived antigens bypassing the hepatic reticuloendothelial system and reaching the systemic circulation, or by immune hyperreactivity associated with the abnormal abdominal venous circulation.
