Quantitative determination of salivary components in the pellicle on PMMA denture base material.

The formation of a salivary pellicle is a protective mechanism of the body for all surfaces in the oral cavity. The nature of the substrate may influence the composition of the pellicle. The aim of this study was to investigate the quantitative composition and individual variation of the salivary pellicle formed on denture base material (PMMA). Cylindrical specimens of PMMA were carried in the mouth and then desorbed with a 0.5-M sodium chloride solution. The solution was analysed for total protein, alpha-amylase, total proteases, protease inhibitors, secretory immunoglobulin A, immunoglobulin G, peroxidases, thiocyanate, lysozyme, and calcium content. All investigated salivary components could be found unequivocally in the desorption solution, indicating that a salivary pellicle had formed on the surface of the PMMA. Large coefficients of variation indicate large individual variations in the adsorbed amounts. The data also point to large intraindividual variations for the bound salivary components. Only the protease inhibitors revealed a strong positive correlation of the bound activity to the salivary activity. It is hypothesised that differences in the bound amounts of antimicrobial components might influence the microbial colonisation of denture bases and that protease inhibitors could be meaningful for the spread of the yeast Candida albicans from denture base material to the oral mucosa and thus might be an explanation for different susceptibility to denture base stomatitis.

Optimization of the neck region of dental implants with a bioactive, resorbable coating.

With the goal of shortening the postoperative healing phase and the minimization of the risk of peri-implantitis around dental implants, a polymer coating, with improved surface biocompatibility in the region of soft tissue penetration by the implant, was investigated. The polymer used was the relatively slowly resorbable poly-beta-hydroxybuterate (PHB) whose surface was further activated in a NH3--plasma. The influence of surface roughness of the substrate (commercially pure titanium) as well as the influence of the edge radii on the test samples was determined in prior studies. These studies formed the basis for an optimised coating process. In-vitro biocompatibility was determined using a human gingival primary cell culture. Surface morphology was determined with SEM and AFM to complement the cell culture studies.

An adjuvant afterloading brachytherapy device for use after orbital exenteration in patients with orbital malignancies.

In cases of malignant diseases situated within the orbit and threaten to extend beyond it, removal of the orbit and periorbital contents may be indicated, followed by adjuvant chemotherapy or irradiation. An effective radiotherapy device for treatment of residual or suspicious cancer in the enucleated orbit consists of an external-beam radiation source (60Co) and an intraoperatively fitted mould as a guide for the flexible afterloading tubes using a 192Ir source. Considering the physical dose distribution and the local situation, this therapy permits a high dose with homogeneous dose distribution to the target area of the orbit but also allows significant dose reduction to adjunctive critical structures. Four patients with different orbital malignancies were treated by means of a combination of percutaneous radiotherapy with afterloading brachytherapy following orbital exenteration. The individual mould of the orbital cavity is modeled at the end of the operation. The technique of radiotherapy, dose distribution, and follow-up of the disease are demonstrated. Orbital malignancies with an extremely poor prognosis may profit from the application of this combined radiotherapy to avoid supraradical surgical intervention.

Long-term results of prosthetic therapy in adult cleft lip alveolus and palate patients.

In the framework of the complex rehabilitation of cleft lip and palate patients the final phase of the dental therapy is attained with the definitive prosthetic treatment. This study comprises 148 prosthetically treated patients in the period 1973 to 1986. The results show that there are good preconditions for an undisturbed professional and social development of the cleft lip and palate patients through intensive dental care.

[Intracavitary afterloading-therapy of cervicofacial tumors--results and experiences to date].

Since 1984 we have used the intracavitary afterloading brachytherapy also in therapy of cervicofacial tumors. We report on 21 patients, 12 of them with therapy of recurrences and 9 with a primary therapy. The patients represent an inhomogeneous selected group. With recurrences also long tumor-free survival periods (till now 44, 52, 53 months) can be attained by means of contact therapy alone or in combination with percutaneous irradiation. Also after primary intensive irradiation curative radiation doses are applicable again as therapy of recurrences. Procedure can likewise be applied in primary therapy of cervicofacial tumors as a boost-irradiation in percutaneous radiotherapy. It should be reserved to limited tumor situations in compliance with irradiation of small volumes.
