[Evaluation of relationship between early brain response and neurodevelopment in newborns by using near infrared spectroscopy].

OBJECTIVE: To study the relationship between early brain response to extrinsic stimulation and neurodevelopment in preterm infants, assess the brain function of preterm infants in the early stage, and thereby to provide objective evidence for the degree of neurodevelopment in preterm infants and to evaluate prognosis. METHODS: Using near infrared spectroscopy (NIRS), the brain response to sound stimulation of 90 preterm infants at different gestational age was observed and compared with the result obtained from 20 full term infants. The neonatal behavioral neurological assessment (NBNA) was performed at corrected age of 40 weeks, and the infants were followed up for 2 years. The effect of gestational age and brain damage on preterm infants, the relation between early brain response in preterm infants and their neurodevelopment was evaluated. RESULTS: All the preterm infants responded to different degrees to auditory stimulation after birth. The time to beginning to react and the time to appearance of the peak reaction were attained after auditory stimulation and the time to beginning to resume when the auditory stimulation was stopped was (278 +/- 94) s, (446 +/- 67) s and (199 +/- 52) s, respectively, which were significantly longer than those observed in the full term infants (107 +/- 30) s, (264 +/- 51) s and (131 +/- 46) s, respectively. The maximum reactions of hemoglobin, oxyhemoglobin and regional oxygen saturation in the infants after gestational age 32 weeks was (0.3 +/- 0.3)%, (0.7 +/- 0.5)% and (0.3 +/- 0.3)%, respectively, which were significantly lower than those in the full term infants (1.7 +/- 0.7)%, (1.7 +/- 0.8)% and (1.6 +/- 0.7)%, respectively. When the brain response of preterm infants was compared with that in infants without brain damage, the speed of the reaction was slow, the maximum reaction was low. The brain response in preterm infant was correlated with NBNA at corrected age of 40 weeks. It was found during the following-up that abnormal neurodevelopment was associated with poor brain reaction. CONCLUSIONS: NIRS can be used to evaluate brain response of infants. Preterm infants display brain response to auditory stimulation. Early brain response is correlated with neurodevelopment.

Charge couple device-based system for 3-dimensional real time positioning on the assessment of segmental range of motion of lumbar spine.

OBJECTIVE: To observe the tested results of the segmental range of motion (ROM) of lumbar spine by charge couple device (CCD)-based system for 3-dimensional real-time positioning (CCD system), and to analyze its clinical significance. METHODS: Seven patients with lumbar joint dysfunction and 8 healthy subjects were tested twice by the CCD-based system with an interval of 10 min. RESULTS: The ROM of the patients was obviously lesser than that of the healthy subjects. The measuring data of segmental ROM of lumbar spine by CCD system is correlated significantly to the same data checked later on the same subjects in every direction of the movements. The differences between two checks are usually less than 1 degree. CONCLUSION: Specially designed CCD based system for 3-dimensional real-time positioning could objectively reflect the segmental ROM of lumbar spine. The system would be of great clinical significance in the assessment of the biomechanical dysfunction of lumbar spine and the effect of the treatment applied.

[Application of near-infrared spectroscopy to postoperative monitoring of flap in plastic surgery].

As a non-invasive technique for the measurement of blood and oxygen in tissue, the near-infrared spectroscopy (NIRS) has an increasing application to the postoperative monitoring of plastic surgery. In authorial research, a set of NIRS oximeter has been used in 6 successful flap-transplantation operations to monitor and contrast the oxygen saturation in the free flap side and in the normal opposite side. It was found in the research that there is a notable difference in the oxygen saturation between those two sides. Another research has been done in an unsuccessful operation. In this experiment, several points in the mandible were measured and the result shows that the oxygen saturation is at an obviously lower level in the anoxic position than in the normal ones. In the above several researches, near infrared spectroscopy showed a high sensitivity to detect the dynamic changes in flaps induced by inhalation of pure oxygen. Therefore, NIRS can be a valuable aid in the post-operative monitoring of free flap after the operation, and must have a great practical future in this field.

[Simultaneous measurement of muscle energy metabolism by MRS and frequency-domain NIR spectroscopy].

The frequency-domain near-infrared spectrometry (NIRS) is capable of measuring the absolute absorption and reduced scattering coefficient of tissue noninvasively. This allows the quantitation of tissue hemoglobin concentration which reflects the balance between oxygen delivery and oxygen utilization of the skeletal muscle. Phosphorus magnetic resonance spectroscopy (31P-MRS) has become a gold standard of noninvasive measurement of human skeletal muscle metabolism. The rate of phosphocreatine (PCr) resynthesis during recovery is an indicator of the rate of oxidative metabolism. The purpose of the present study was the effect of lower intracellular pH (pHi) on PCr and oxygenation recovery. The preliminary results of plantar flexion experiment on a healthy male subject show that both PCr resynthesis and reoxygenation were very much prolonged by severe acidosis (pHi = 6.42).

[Spectral colorimetric correction of full view color images in a real scene measurement system].

In the three dimensional multiple modalities scanning real scene measurement system, a color linear CCD was used to obtain the one dimensional linear color information of the scene. Successive one dimensional color images of different angles were patched to constitute the full view color image. If the color image is displayed directly on the color cathode ray tube (CRT) monitor of a computer, colorimetric distortion will occur. Colorimetric correction is then needed. Through thespectral colorimetric correction, the colorimage displayed on the color CRT monitor is matched with the original scene in the colorimetric sense.

[Non-invasive determination of the optical properties of neonatal brain].

The absorption andreduced scattering parameters of tissue can provide information on a variety of physiologic processes. In the present paper, the local optical properties of 23 infant foreheads were measured with a two-wavelength (788 and 832 nm) portable frequency-domain NIR spectroscopic techinque, based on a standard reference phantom with known optical properties. The single source-detector separation is 40 mm. The cerebral blood volume and tissue oxygensaturation were also derived from themeasured absorption coefficients.

[Small-invasive photoelectric detection of blood glucose].

A new type of photoelectric small-invasive blood glucose monitor was developed. This instrument is based on a 16 bit microprocessor MSP430F413 which belongs to a low-power microprocessor of the component family of Texas Instruments promotion recently. There are three advantages of this instrument: (1) using the method of two optical beams to decrease the influence of the LED's power fluctuation, (2) using the multiple comparative method to determine the terminative time of enzymatic reaction and the start time of computing and processing, (3) to store a lot of measured data up to 50 samples. The data measured by this instrument were compared with Prestige blood glucose monitor, and the result shows that the accuracy and the reliability of this instrument is better. The specialties of this instrument are easy operation, getting results quickly, low-power and low cost. It is an ideal instrument for diabetic test and control of blood glucose concentration at home.

[Measurement and statistical analysis of the vertical force in normal young people's gait].

OBJECTIVE: To find a method for evaluating the relationship between mechanical data and gait efficiently. METHOD: Gait mechanical data of 27 young men (20-22 years) without history of the kinetic diseases were collected using a force-measuring system. The data were transformed into characteristic two-peak curves in vertical direction (z-axis). Seven characteristic parameters were distilled. The coefficient of variation (CV) and the absolute symmetry index (ASI) of the parameters between left foot and right foot were analyzed. RESULT: It was found that CV of the first temporal parameter of gait (T(Z1)) is equal to 14.49%, which corresponds to larger dispersion. The values of ASI are all lower than 12.5%, which means the symmetry between left and right feet meets general requirement. But the P value (result of t-test) of F(Z2) is 0.0368 (lower than 0.05), which means that there is obvious differentia between F(Z2) of left and right feet, while no such differentiae were found for the other six characteristic parameters. t-test was also used to analyze the differentia between the gait of male and female. It is still found that p value of F(Z2) is lower than 0.01, which corresponds to a great differentia on F(Z2) between male and female. CONCLUSION: Some significant conclusions of the repetition, dispersion, symmetry and differentia of gait data have been obtained by the analysis.

[A gait analysis system based on digital video and digital image processing].

OBJECTIVE: To provide a gait analysis system. METHOD: The system is composed of general video devices and personal computer hardware. The techniques of digital video and digital image processing such as template matching, sub-pixel and motion estimate etc were employed to identify and trace the markers. RESULT: A gait result of a cerebral palsy object, which consists of bar-figure, data, and video was provided. CONCLUSION: The gait analysis system based on digital video and digital image processing is easy to use, and can provide visual result, which is easy to understand.

[The preliminary study on noninvasive detection using NIR diffusion reflectance spectrum for monitoring blood glucose].

The noninvasive detection method using near-infrared diffusion reflectance spectrum for monitoring blood glucose was described. A group of healthy volunteers(2 young male and 2 young female) was tested twenty-six times under changing of both the blood glucose concentration and spectrum region during various states. In order to set up a model, the set was chosen from these samples, the set of prediction was not belong to the set of the model. The results from different selected spectrum regions and reprocessing methods show as follows: 1) In the spectrum region of 9,000-12,000 cm-1 using MIN-MAX normalization reprocessing method to set up a model and predict, the errors of prediction were more than 36 mg.dL-1; the model errors were under 16 mg.dL-1 and predictive errors were less than 25 mg.dL-1 by using smoothing and second derivatives in the same region; in the another region of 4,000-5,000 cm-1 with smoothing and first derivatives both error of model and prediction were more than 25 mg.dL-1. 2) In the region of 4,000-9,000 cm-1 with smoothing and second derivatives, the errors from model were less than 15 mg.dL-1, the errors of prediction were less than 31 mg.dL-1; selecting the spectrum regions of 6,100-7,500 cm-1 and 4,200-4,700 cm-1 by a quantitative software OPU VERSION 3.01 with smoothing, first derivatives and vector normalization, the errors of model were no more than 11 mg.dL-1, the errors of predication were no more than 22 mg.dL-1. 3) Choosing the sample from the subject itself to establish the model in 9,000-12,000 cm-1 with smoothing and second derivatives, the errors of model and predication were less than 15 mg.dL-1 and 11 mg.dL-1 respectively. The results showed that the method was feasible to monitoring the blood glucose concentration by selecting an appropriate spectrum region and reprocessing methods with partial least square algorithm which set up a mathematical model. Excepting the number of sample, selected spectrum region and reprocessing methods, the other influencing factors such as the variant from the subject were discussed also.
