Surgical excision of giant primary carcinoma in the medial liver lobe.

OBJECTIVE: To assess the techniques for surgical excision of giant primary carcinoma in the medial liver lobe. METHODS: Operative managements, complications and their causes during and after resection of giant carcinoma in the medial liver lobe were analyzed retrospectively in 166 cases treated from October 1996 through December 2001. RESULTS: Of the 166 patients, 123 (74.1 %) underwent tumor resection and 43 (25.9 %) regular lobectomy, including left trilobectomy (8, 4.8 %), medial lobectomy (21, 12.7 %), right anterior lobectomy (11, 6.6 %), and hemihepatectomy (3, 1.8 %). All patients were subjected to surgery with intermittent interruption of the first porta hepatis under normothermia. The total interruption time was 7-68 minutes and average time was 24.5 minutes. The maximum single interruption time was 41 minutes. Intraoperative blood loss was 50-4000 ml, averaging 726 ml. The maximum blood transfusion was 5200 ml, averaging was 811 ml, and transfusion was not needed in 54 patients. Postoperative complications occurred in 9 patients (5.4%), of whom, 2 (1.2%) died of liver failure and acute respiratory distress syndrome respectively. CONCLUSIONS: An adequate reserve of liver function is a prerequisite for a smooth recovery after operation. Careful intraoperative management is crucial to decrease postoperative complications.

Expression of AFPmRNA in blood of patients with recurrent hepatocellular carcinoma.

OBJECTIVES: To detect the expression of human alpha-fetoprotein mRNA (AFPmRNA) in peripheral blood (PBL) of recurrent hepatocellular carcinoma (HCC) by using nested reverse transcriptase polymerase chain reaction (nested RT-PCR) and to discuss relations between AFPmRNA and recurrent HCC. METHODS: Five ml peripheral blood samples were obtained from 19 patients with recurrent HCC. To identify HCC cells in PBL, liver-specific AFPmRNA was amplified from the total RNA extracted from the whole blood by nested RT-PCR. RESULTS: Seven patients (36.8%) with recurrent HCC were detected AFPmRNA, and 12 patients with recurrent HCC were AFPmRNA negative (63.2%). There was no significant relation between patients with AFPmRNA positive and AFPmRNA negative (P>0.05) in judging the origin of recurrent HCC. CONCLUSION: AFPmRNA may not be used as a marker of origin of recurrent HCC.

Percutaneous cryoablation after chemoembolization of liver carcinoma: report of 34 cases.

OBJECTIVE: To assess the indications, efficacy and clinical significance of percutaneous cryoablation for liver carcinoma after transcatheter liver artery chemoembolization (TACE). METHODS: Thirty-four patients with histologically or clinically confirmed primary or metastatic carcinomas were treated with TACE. One week to 1 month later, they were treated percutaneously under ultrasound guidance using cryosurgical system in the period of July 2001-June 2002. All patients were followed up to determine serum tumor marker, CT scans, MRI images or ultrasound images. RESULTS: This therapy was performed in 34 patients including 32 patients with Child A liver reserve, 2 patients with Child B and no patient with Child C. There were 28 patients with primary liver cancer and 6 patients with metastatic liver cancer. During the follow-up period (3 to 15 months), 41.1% patients were recognized clinically cured because the serum tumor markers became normal, or CT scans and MRI images revealed that the lesion became completely necrotic. 44.1% patients were recognized effectively treated. CONCLUSIONS: Percutaneous cryoablation combined with TACE is a choice of treatment for liver carcinoma. It is minimally invasive, safe and effective for those patients with liver cancer unsuitable for surgery.

[The study of peptide vaccine HDS from Streptococcus mutans glucosyltransferase].

OBJECTIVE: To investigate the antigenicity of the peptide vaccine HDS from Streptococcus mutans glucosyltransferase and its ability to induce protective immune responses in an experimental rat model of dental caries. METHODS: Artificial antigen HDS-KLH, peptide HDS, glycosyltransferase were injected to immunize rats. Measurement of the specific anti-HDS, GTF IgG or IgA concentration in saliva and serum were undertaken by ELISA among the experimental groups. Gnotobiotic rat model was developed when challenged S. mutans and a caries promoting diet. The jaws of the rats were selected and dyed. The Keyes caries score for each jaw were counted. RESULTS: The level of serum and salivary specific anti-HDS IgG and IgA in the group immunized by HDS-KLH was significantly higher than that in control group (P < 0.05). The Keyes caries score of GTF, HDS and HDS-KLH immunized group were significantly lower than that of control group, especially lower in smooth tooth surface. CONCLUSION: Artificial antigen HDS-KLH could induce immune response. As a peptide vaccine, HDS-KLH could reduce the caries incidence in experimental rat model.

[Optimization of T-dNA insertional mutagenesis and analysis of mutants of Magnaporthe grisea].

The rice blast fungus Magnaporthe grisea causes one of the most destructive diseases of rice around the world. Significant progresses have been made recently in genomics studies of the fungus, opening new era of the functional genomics which requires to generate a large scale of gene knockout mutants. It has been demonstrated that T-DNA insertional mutagenesis is a powerful tool of functional genomics not only for plants but also for fungi. In this paper, we optimized the conditions for T-DNA insertional mutagenesis of M. grisea using Agrobacterium tumefaciens-mediated transformation (ATMT) approach. We employed the binary vector pBHtl constructed by Dr. S. Kang's laboratory at the Pennsylvania State University, which carries the bacterial hygromycin B phosphotransferase gene (hph) under the control of the Aspergillus nidulans trpC promoter as a selectable marker to transform the conidia of M. grisea. We optimized the conditions for T-DNA insertional mutagenesis including the medium, dosage of hygromycin B, cefotaxime and carbenicillin to select the transformants and inhibit the growth of A. tumefaciens after co-culturing. The dosage to inhibit non-transformants could vary from 200-600microg/mL among different M. grisea isolates so that the optimal dosage of the antibiotics should be decided according to isolates. Rice polished agar medium was found the best selection medium which would facilitate the mutant sporulation and minimize the contamination chance. In average, about 500 transformants could be obtained when transforming 1 x 10(6) spores at the optimum condition, among which 85% had T-DNA insertion detected by polymerase chain reaction (PCR) and thermal asymmetric interlaced polymerase chain reaction (TAIL-PCR). Fifteen out of 1520 transformants showed mutation in colony morphology. Within 58 randomly selected mutants, it was found that there were 4 sporulation-decreased mutants, 8 less germination mutants and 9 appressorium defective mutants. Several virulent mutants to C101LAC(Pi-1)and 75-1-127(Pi-9)were also obtained which would facilitate cloning the corresponding avirulence genes.

[Curative effect of percutaneous microwave coagulation therapy for hepatocellular carcinoma].

OBJECTIVE: To observe the curative effect of percutaneous microwave coagulation therapy (PMCT) with 2450 MHz microwave antenna for hepatocellular carcinoma. METHODS: Under local or epidural anesthesia, a thin percutaneous microwave antenna was introduced with ultrasound guidance into the tumor in the liver for thermo-coagulation. RESULTS: Among the 97 hepatic cancer lesions in 52 patients, 61(62.9%) with phi < 3 cm were coagulated once. In follow-up of 6-12 months of these lesions, 57(93.4%) showed no recurrence by CT or MRI. Thirty-six (37.1%) with 3 cm < phi < 5 cm coagulated twice showed that 27 (75.0%) gave CR and 9 (25.0%) gave PR by CT or MRI in follow up of 6 months. There were no serious clinical side effects or complications in the PMCT patients. CONCLUSION: Percutaneous microwave coagulation therapy gives good curative effect on liver tumor with phi < 3 cm. It is partly effective on lesions 3 cm < phi < 5 cm.

[The effect of postoperative transcatheter hepatic arterial chemoembolization on disease-free survival after hepatectomy for hepatocellular carcinoma].

OBJECTIVE: To evaluate postoperative transcatheter hepatic arterial chemoembolization (TACE) for improving the disease-free survival of HCC patients after hepatectomy. METHODS: 1 725 HCC patients were followed up after hepatectomy retrospectively. Of 1 457 patients who were followed-up completely, 209 had postoperative TACE. The 1 457 patients were divided into ten groups according to tumor thrombus, small HCC, capsular invasion, vascular invasion and cirrhosis. The disease-free survival was analyzed between subgroups of weather postoperative TACE was performed in every group. Software SAS 6.12 and EGRET package were used. Kaplan-Meier estimation was used to calculate the disease-free survival rates. RESULTS: There were no difference of the disease-free survival between the subgroups in no capsular invasion and in no hepatic cirrhosis groups. CONCLUSION: Postoperative TACE is helpful in improving the disease-free survival of HCC patients after hepatectomy except those with integrated tumor envelope or no hepatocirrhosis. It is important to prolong the long-term results of operation.

[Percutaneous radiofrequency ablation combined with transcatheter arterial chemoembolization for hepatocellular carcinoma].

OBJECTIVE: To assess the significance of the method of percutaneous radiofrequency ablation (PRFA) combined with transcatheter arterial chemoembolization for hepatocellular carcinoma. METHODS: Thirty patients with hepatocellular carcinoma were divided into PRFA group and TACE + PRFA group between January 2000 and July 2001. All patients were followed up to examine the value of AFP, MRI or CT. Kaplan-Meier estimation was used for the analysis of disease-free survival and the cumulative survival rate. RESULTS: The complete necrosis rates were 86.7% (13/15) and 26.7% (4/15) in the TACE + PRFA group and group PRFA group respectively. The rates of AFP positive down to negative were 66.7% (6/9) for the former and 20% (2/10) for of the latter, and the six-month disease-free survival rates were 100% (13/13) and 75% (3/4) in the two groups. The 1-, 1.5- and 2-year survival rates of the group TACE + PRFA were 100%, 100% and 66.7% respectively. The survival rates of 1 and 1.5 years of the group PRFA only were 80% and 40%. CONCLUSIONS: For those hepatocellular carcinomas over 3 cm in size, located in the porta hepatis, or with indistinct boundary or the presence of foci, TACE can be performed first and then followed by PRFA in suitable time. This method can enlarge the necrosis range and increase the rate of complete necrosis of tumors, thereby decrease the recurrence and improve the disease-free survival and total survival of patients.

[Transarterial embolization for hemorrhage due to spontaneous rupture in hepatocellular carcinoma].

OBJECTIVE: To evaluate the efficacy of transarterial embolization (TAE) for intraperitoneal hemorrhage due to spontaneous rupture in hepatocellular carcinoma (HCC). METHODS: Fourty-two patients with ruptured HCC were divided into 4 groups according to the type of their previous treatment: Group A-TAE followed by elective hepatectomy 15, Group B- TAE alone 11, Group C-emergency operation 6 and group D-medical conservative management 10. RESULTS: Celiac arteriography done before the present treatment showed extravasation of contrast material in 7 (26.9%) of the 26 patients in group A and B, and hypervascular tumor was observed in the rest. The hemostasis success rate of group A, B and C were 100%, which were much higher than that of group D (40%) (P < 0.05). The in-hospital mortality rates of group A, B and C were 0, 3.8% and 16.7% (P > 0.05), which were much lower than that of group D (80%) (P < 0.01). The 1-year survival rate of group A (76.3%) was higher than those in groups B (47.5%) and C (43.7%) (P < 0.05). There was no 1-year survivor in group D. CONCLUSION: Transarterial embolization is safe and effective for hemorrhage due to spontaneous rupture in hepatocellular carcinoma. For resectable lesions, TAE is a preferential treatment to be given first, then followed by elective hepatectomy.

Hepatectomy in the treatment of very big primary liver cancer: report of 86 cases.

OBJECTIVE: To study the indications for resection of very big primary liver cancer and the operative results. METHODS: From January 1985 to June 1996, 86 patients with very big primary liver cancer (>/= 15 cm in diameter) underwent hepatectomy in our hospital. The volume of bleeding and blood transfusion was recorded during the operation. After the operation, the draining quantity from their abdominal cavities, and the days of transfusion and hospitalization were recorded. The occurrence of complications and survival time of the patient were followed up. RESULTS: The postoperative mortality was 3.48% and the occurrence rate of complications was 31.40%, which was significantly correlated with preoperative lower level of serum albumin or the elevated gamma-globulin level and the amount of resected liver tissue. But their liver function before operation was fairly good, the 1-, 3-and 5-year survival rates after hepatectomy were 58.2%, 35.7% and 17.64%. CONCLUSIONS: patients with very big primary liver cancer, should be subjected to hepatectomy if their liver function before operation are normal and the margins are distinct between the tumor and liver tissues. After the operation, other treatments are suitable for good effects.

E-selectin and its ligand-sLeX in the metastasis of hepatocellular carcinoma.

OBJECTIVE: To study the significance of E-selectin and its ligand-sLeX in the metastasis of hepatocellular carcinoma (HCC). METHODS: Flow cytometry and immunohistochemistry were used to detect the expression of E-selectin and its ligand-sLeX in both HCC cell lines and human HCC tissues. RESULTS: The positive rate of E-selectin in vascular endothelial cells adjacent to cancer was 67.9% (19/28). The expression of E-selectin in tumors accompanied with emboli or satellite foci was significantly higher than that in tumors without emboli or satellite foci (P<0.05), and it was not related to tumor size, tumor capsule, AFP content, and the degree of differentiation. The positive expression of sLeX in SMMU-7721, PLF/PRF/5 and HepGII cell lines was 7.03%, 63.35% and 97.29% respectively. The positive cells of sLeX mainly distributed in the margin of tumor tissues. The positive expression of sLeX in HCC cells in emboli or invasive tumor tissues was much higher than in Primary foci. CONCLUSION: E-selectin and its ligand-sLeX are closely correlated with the metastasis of HCC.

Influence of preoperative transcatheter arterial chemoembolization on liver resection in patients with resectable hepatocellular carcinoma.

OBJECTIVE: To assess the influence of preoperative transcatheter arterial chemoembolization (TACE) on liver resection in patients with resectable hepatocellular carcinoma (HCC). METHODS: Of 126 patients with resectable HCC, 62 received preoperative TACE (TACE group) and the remaining 64 patients were selected as controls (non-TACE group). Perioperative risk factors including liver function alteration, mean blood loss during operation, mean time of clamping the porta hepatis, length of operation, postoperative abdominal drainage at day 1, 2 and 3, morbidity and mortality were compared between the two groups. RESULTS: Neither significant difference in liver function alteration nor mortality was observed between the two groups. More severe hepatic cirrhosis, longer operation time, more blood loss and postoperative abdominal drainage were noted in the TACE group than in the non-TACE group. There was no significant difference in postoperative morbidity between the two groups. CONCLUSIONS: Preoperative TACE for resectable HCC increases surgical difficulty and risk, and therefore should be considered prudently according to the individuality of patients.

Pancreaticoduodenectomy with Roux-Y anastomosis in reconstructing the digestive tract: report of 26 patients.

OBJECTIVE: To explore the way to lower the morbidity and mortality of patients after pancreaticoduodenectomy. METHODS: Between March 1998 and March 2001, 26 patients with periampullary tumors received pancreaticoduodenectomy (PD) with Roux-Y anastomosis to reconstruct the digestive tract. Of these patients, 6 had ductal cell carcinoma at the head of the pancreas, 8 distal common bile duct carcinoma, 5 ampullar adenocarcinoma of the Vater, 6 duodenal adenocarcinoma, and 1 duodenal malignant lynphoma. A 30-40 cm free vascularized segment of the proximal jejunum was taken and pulled up to the bed of the duodenum for end-to-end pancreaticojejunostomy, end-to-side choledocojejunostomy or side-to-side jejunojejunostomy by a single loop. RESULTS: The operative mortality was zero. Postoperative intraabdominal hemorrhage occurred in 2 patients, but no leakage during pancreaticojejunostomy or choledocojejunostomy as well as abdominal infection. The patients were discharged from the hospital on the tenth to fourteenth day after operation. Follow-up for 5 to 36 months (mean 21 months) revealed chronic steatorrhea and malnutrition in one patient (3.85%), and good digestive function and normal nutritional status in 25 (96.15%). No bile reflux gastritis, retrograde infection, anastomotic ulcer, and dumping syndrome were observed. CONCLUSION: Our results show that this procedure can effectively reduce the morbidity and mortality of patients after PD.

Intermittent portal triad clamping in resection of liver tumors involving the hepatocaval confluence.

OBJECTIVE: To review our experience in and the results of resecting liver tumors involving the hepatocaval confluence under intermittent portal triad clamping (PTC). METHODS: Sixty-eight consecutive patients with liver tumors involving the hepatocaval confluence underwent hepatectomies with liver parenchymal transections under intermittent PTC. RESULTS: All the tumors were successfully resected under PTC, except for one in which the infrahepatic inferior vena cava was concomitantly occluded in addition to PTC. There was neither operative death nor uncontrollable massive bleeding or air embolism occurred in our patients. The bleedings from the main and short hepatic veins and right adrenal veins were properly managed during the operation, with a mean intraoperative blood loss of 1400 ml. Of the 68 tumors resected, 65 were hepatocellular carcinomas (HCC). Their 1-, 2-, 3- and 4-year survival rates were 64.11%, 52.82%, 44.90% and 36.98%, respectively, and the patients with HCC with capsules survived significantly longer than those with HCC without capsules. CONCLUSIONS: The liver tumors involving the hepatocaval confluence could be safely resected simply under PTC, without routine use of total hepatic vascular exclusion. As for HCCs in this area, the tumor with capsule is a better indicator for surgical resection than that without capsule.

Expression of perforin and granzyme B mRNA in judgement of immunosuppressive effect in rat liver transplantation.

AIM:To explore the expression of perforin and granzyme B genes mRNAto judge the effect of immunosuppression in acute rejection of liver transplantation.METHODS: The expression of perforin and granzyme B genes mRNA was examined by reverse transcription-polymerase chain reaction (RT-PCR) in hamster to rat liver grafts under the immunosuppression of cyclosporine or/and splenectomy. Histological findings were studied comparatively.RESULTS:Cyclosporine could obviously decrease the cellular infiltration, and completely repress the expression of mRNA for perforin and granzyme B, but could not change severe hepatocyte necrosis and hemorrhage. Splenectomy could significantly lighten hepatocyte necrosis, and completely eliminate hemorrhage, but not affect the cellular infiltration and the expression of perforin and granzyme B genes mRNA. Cyclosporine or splenectomy alone could not prolong the survival time, however, their combination could completely repress the rejection of liver grafts. The survival time of animals were significantly prolonged (37.1 days). The architecture of hepatic lobules was preserved. There were slight cellular infilt-ration in the portal tracts and no expression of perforin and granzyme B genes mRNA could been seen in three weeks after transplantation.CONCLUSION:Perforin and granzyme B genes are valuable in judging the effect of immunosuppression in liver transplantation.
