Fuzzy logic model based on the differential nursing diagnosis of alterations in urinary elimination.

The nursing diagnoses associated with alterations in urinary elimination require different interventions, and nurses who are not specialists need support to diagnose and to manage patients with disturbances of urine elimination. The aim of this study was to present a model based on fuzzy logic for making differential diagnosis of alterations in urinary elimination, considering the nursing diagnosis approved by the North American Nursing Diagnosis Association (NANDA), 2001-2002. The fuzzy maximum-minimum composition was used to develop this model. It was tested with 195 cases from a database of a previous study. The model was able to determine the diagnosis in total accordance with a panel of three experts for 79.5% of the cases. The model diagnosed 19% of the cases with partial concordance with the panel of experts. Only for 3 cases (1.5%) the model showed a different diagnosis. It is concluded that the model proposed here, despite of its simplicity, presents good performance. However, it is recommended more tests before widely used as support for clinical decision.

Proposal for an essential nursing data set to evaluate the health in home care elderly persons.

This paper presents a proposal for a set of computerized data to construct an instrument in initial evaluations of the overall state of health elderly persons. The objective is to establish a common system of language and data-sharing, and to make the data collected available. This is expected to enable better planning and promotion of the quality of the assistance provided to this population.

Nursing documentation in occupational health.

PURPOSE: Traditionally, nursing documentation has been consistent with hospital standards and legal definitions of clinical nursing practice. Identify data and information nurses need to be recorded in order to maintain the continuity and quality of nursing care and the efficiency of nursing performance is a research question that is moving professionals around the world. This study objective is to describe the analysis of nursing documentation in the patient records. METHODS: It is a retrospective study. The study was conducted in the ambulatory occupational health nursing; it was selected 111 patient records. Of these, in 106 we identified a total of 775 nursing records. The nursing records comprise the following dimension: identification, job history, health state, health and safety, psychological e socio- cultural, medical history, physical examination and nursing assessment. RESULTS: In the data set elements found as documented in the subjective data and objective data, there was higher frequency of data elements related to the following nursing dimensions: health state, health and safety, physical examination and nursing assessment. The dimension of job history we found that 25% of the nursing records did not documented information about the current work status of the patient. In addition, the current job activity 20.77% of the records), working day 9.03% of the records), job process 8.13% of the records), worksite exposure 8.0% of the records), environmental works 6.19% of the records), occupation 5.81% of the records), job time 4.39% of the records), before job activity 4.13 % of the records), and work location 3.23% of the records) were not also documented. CONCLUSION: In conclusion, the present study was an attempt to highlight the importance of data to be documented and organized in the existing information systems in the specific area of occupational health care. The adequate data collected can provide the right information to improve nursing care in this care setting and enhance health population.

[Nursing minimum data set: identifying of categories and items for the nursing practice in ambulatory occupational health].

This article describes the categorization of the elements identified from the nursing check-up in ambulatory occupational health. This is a retrospective descriptive study that was conducted in the Nursing Ambulatory Center at Hospital de Clínicas de Porto Alegre. The data sources were the nursing records used in outpatient unit and from the records of the institution's database, collected from August 1998 to August 2003. Of the 106 patient records used, 777 records were identified as first-time encounter and nursing return appointments. The initial results of this study suggest that the demographic elements of the patient/client, nursing care elements, service elements, and occupational health elements comprise a nursing essential data set in the area of occupational health.

[Current perspectives in nursing informatics].

Nursing Informatics is the area of knowledge that studies the application of technological resources in teaching, in practice, in care, and in the management of care. Resources such as voice recognition, knowledge base, genoma project and even Internet have offered to Nursing a gama of possibilities for a better professional performance and better nursing care to the patient/client. This text reports and exemplifies how these resources are impacting and presenting new oportunities for teaching, research and specially for nursing care, still warns for the importance of humanized care in a high-tech scenario.

Nursing phenomena identified in family planning visits with ICNP - Beta version 2.

This descriptive, exploratory, retrospective survey, carried out at a family planning service, aimed to identify nursing phenomena during nursing visits according to the ICNP, Beta version 2. Data were collected based on 52 records of nursing visits, realized from October 2001 to December 2002. To conduct the cross-mapping process, all identified nursing phenomena were joined, organized and compared according to the ICNP's terms. Of the 51 identified nursing phenomena/diagnoses, 46 (90.2%) showed exact and partial concordance. The identified nursing phenomena can be used to assist nurses to provide care for clients in family planning services. The ICNP showed to be a comprehensive program, although some terms need to be reviewed and others enhanced. However, considering that it is an international classification applicable to several countries, the mapping process and cross-references were very satisfactory.

[Nursing on the Web: the creation and validation process of a web site on coronary artery disease].

The World Wide Web is an important health information research source. A challenge for the Brazilian Nursing Informatics area is to use its potential to promote health education. This paper aims to present a developing and validating model used in an educational Web site, named CardioSite, which subject is Coronary Heart Disease. In its creation it was adopted a method with phases of conceptual modeling, development, implementation, and evaluation. In the evaluation phase, the validation was performed through an online informatics and health experts panel. The results demonstrated that information was reliable and valid. Considering that national official systems are not available to that approach, this model demonstrated effectiveness in assessing the quality of the Web site content.
