Tigroid background in fine-needle aspiration cytology of clear cell sarcoma.

Clear-Cell Sarcoma (CCS) is a rare malignant soft tissue tumor of young adults that often metastasizes to the lymph nodes. The fine needle aspirate cytology (FNAC) findings of CCS have been described previously and it is included in the cytological category of polygonal soft tissue tumors. We describe the cytology of a case of CCS, with emphasis on the so called "tigroid background." Though this tigroid background is a nonspecific feature and has been described in various other tumors, to the best of our knowledge, it has not been described in CCS. This feature may be useful in the recognition of CCS.

Interobserver variation in breast cancer grading: a statistical modeling approach.

OBJECTIVE: To study random and the systematic error in breast cancer grading, to find the source of disagreements and measure the reliability of graders so that appropriate corrective action can be taken. STUDY DESIGN: Five independent observers graded 50 breast carcinoma slides from 50 consecutive breast cancer specimens according to the Nottingham criteria. The polychoric correlation was used to measure association. Stuart-Maxwell and McNemar tests were used to measure equality of thresholds. RESULTS: The polychoric correlation among observers was high (mean = 0.803, 0.712, 0.797 and 0.602 for the final grade, tubule formation, nuclear pleomorphism and mitotic figures, respectively). However, there were significant differences in thresholds (6, 7, 7 and 9 pairs of 10 showing significant differences in classification of grades/scores for final grade, tubule formation, nuclear pleomorphism and mitotic counts, respectively). CONCLUSION: The high polychoric correlations suggest that random error in grading breast cancers in this study was low, confirming the underlying reliability of grading and graders. However, significant differences in the thresholds lowers raw agreement. Such a scenario may be rectified by increased intradepartmental discussion.

Tissue effects of fine needle aspiration on salivary gland tumours.

Fine needle aspiration cytology (FNAC) is a widely used diagnostic procedure to evaluate lesions in the salivary glands. Though regarded as a safe and reliable procedure ,a variety of histological changes following FNAC have been reported. We studied the FNAC related tissue changes in 50 neoplasms of the salivary gland and the impact on subsequent histological evaluation. FNAC induced changes were found in 34% (17/50) with needle tract (10/50)and infarction (7/50) as the commonest changes . These changes did not interfere with the subsequent histological evaluation of the tumours. We conclude that knowledge of a previous FNAC and awareness of its effects on histology may avoid potential misdiagnosis, though the incidence of significant changes is small.

Cytomorphology of subcutaneous panniculitic T-cell lymphoma (SPTCL)--a case report.

T cell lymphomas account for approximately 60% of cutaneous lymphomas. The annual incidence of cutaneous lymphoma is estimated to be from 0.5 to 1 per 1,00,000 persons per year. We present one case of cutaneous lymphoma, an eighteen year old male who presented with multiple swellings all over the body of one month duration. On examination, multiple, subcutaneous, mobile, non tender nodules were seen ranging from 0.5 cm to 5 cm in diameter. FNAC revealed non-Hodgkin's lymphoma morphologically in favour of cutaneous T-cell lymphoma. A biopsy and immunohistochemistry confirmed SPTCL.

Myxoid liposarcoma of the breast--report of a rare case.

Breast sarcomas are very rare. A 40 year old lady presented with a well-defined hard lump, 6 cm in size, in the left breast. Patient underwent simple mastectomy with axillary clearance following a diagnosis of phylloides tumour on fine needle aspiration cytology. Grossly, it was a circumscribed myxoid tumour. Histopathologically, it fulfilled all the criteria for a diagnosis of myxoid liposarcoma. The absence of epithelial component excluded phylloides tumour.

Case reports of basaloid squamous cell carcinoma--an aggressive variant of squamous cell carcinoma.

Basaloid squamous cell carcinoma (BSCC) is a high grade variant of Squamous Cell Carcinoma with a predilection for the aero-digestive tract. The typical microscopic features of carcinoma with a basaloid pattern in intimate association with a squamous component helps in diagnosis of this tumour. There are few reports in the Indian literature and we report three cases, including one in the sinonasal region. This study discusses the differential diagnosis of BSCC, as recognition of this tumour may have therapeutic implications.

Fibrosarcomatous Bednar tumor with distant metastases--a case report.

In July 1994, a 27 year-old man presented with a twice recurrent, pigmented tumor on his left wrist. Wide excision and histological examination showed a pigmented dermatofibrosarcoma protuberans (PDFSP) or Bednar tumor with foci of fibrosarcomatous (FS) change. Immunohistochemistry helped to distinguish it from cellular blue nevus and malignant melanoma. In January 1997, he presented with 3 swellings on his thighs which showed features of fibrosarcoma. In March 1998, he developed multiple tumors on his thighs, chest wall, supraclavicular region, parapharyngeal region and two lung nodules. To the best of our knowledge, there are only 3 previously published reports of metastases in patients with Bednar tumor. We present the fourth such case.

Solitary parotid metastasis from columnar cell carcinoma of the thyroid: a diagnostic dilemma.

A 65-yr-old woman presented in May 1998 with a left parotid mass of 2 yr duration. The cytology and histopathology of the parotid lesion showed features of a columnar cell carcinoma. In November, 1967, she was diagnosed to have a classic papillary thyroid carcinoma (PTC). Ultrasound examination revealed a persistent thyroid nodule, the aspiration cytology of which confirmed columnar cell variant of papillary thyroid carcinoma (CCV-PTC). CCV-PTC is a recently described rare tumor of the thyroid that has been associated with an aggressive clinical course. Our case was peculiar in that the patient presented with an isolated metastasis in the parotid gland. Second, after an indolent course following the initial diagnosis of usual PTC 31 yr ago, she had now developed a more aggressive variant of the tumor.

Fine needle aspiration diagnosis in HIV-related lymphadenopathy in Mangalore, India.

OBJECTIVE: To evaluate the usefulness of fine needle aspiration (FNA) study of lymph nodes in HIV-positive patients. STUDY DESIGN: The study was conducted at Kasturba Medical College, Mangalore, India. Samples from lymph nodes of 48 HIV-positive patients were taken and air dried, and wet smears were made. After staining with routine cytologic stains and special stains, detailed cytomorphologic study was conducted. RESULTS: Tuberculosis accounted for nearly half (48%) the cases, followed by HIV lymphadenitis (36%), lymphoma (10%), suppurative lymphadenitis (2%), Mycobacterium avium-intracellulare lymphadenitis (2%) and metastases (2%), in descending order of their frequency. A suppurative picture, which was found in 13% of cases of tuberculous lymphadenitis in AIDS patients, occasionally was misleading without the help of acid fast bacilli stain. CONCLUSION: FNA is a useful tool in the diagnosis of lymphadenopathy in HIV-positive patients provided that proper safety measures are taken to avoid contracting the infection.

Second primary lung cancer with tuberculosis.

We report a rare case of second primary lung cancer presenting metachronoulsy in a patient with laryngeal cancer. Though regional recurrence and second primary neoplasm of the upper aerodigestive tract are common following laryngeal cancers, second primary cancers of the lung are uncommon. Adding to the rarity of the case was the presence of concurrent active pulmonary tuberculosis with second primary neoplasm of the lung.
