[Visual-oculomotor and vestibular-oculomotor abnormalities in Parkinson's disease].

In the study oculomotor abnormalities were investigated in 35 patients with idiopathic Parkinson's disease. Vestibular-oculomotor reflexes, pursuit eye movement, saccades and optokinetic nystagmus were evaluated using videonystagmographic registration. Saccadis, smooth pursuit and optokinetic reflexes were significantly disturbed in the majority of cases. Pathological results of caloric and rotatory tests were observed rarely. In the patients after stereotactic thalamotomy--14 cases, there were no changes in visual-oculomotor reflexes pathology. The results show that visual reflexes are often disturbed in Parkinson's disease and suggest the possibility that subcortical neurons, not thalamus are engaged in reflex eye movements.

[Rotation suction-knife (shaver) in otorhinolaryngological surgery].

INTRODUCTION: Rotation suction knife (shaver or microdebridor) was developed in beginning 1985 in orthopedic surgery. In the field of otolaryngology it was used in 1992-1994. Shaver is a combination of suction and cutting round knife working together. Knives of different shapes are put into handpiece. They have outer sheath with window, which protect the inner rotating blade. The blade is connected to suction and cutting tissue is removed from the operation field. In otolaryngology surgery we mostly used the oscillation cutting mode which is the most sufficient. Shaver surgery is precise in soft tissue resection, so the most advantages to powered the system is in rhinosurgery, polypectomy with extension to pansinus surgery. Microdebridor can be also used for nasopharynx-adenoidectomy, chonanal atresia surgery, turbinoplasty, dacryocystorhinostomy, sella turcica tumor resection or laryngeal surgery. MATERIAL AND METHODS: We used the Storz's shaver system with a handpiece with a motor in the vertical part of the T-shaped handle. Suction and transport of rejected material does not pass through the motor system. The suction channel is straight and clogging is minimized. We performed shaver surgery during functional endonasal endoscopic surgery (FESS) for polypectomy in 15 cases, medial antrostomy (uncinectomy for "swinging door" procedure) in 10 cases, nasopharyngeal-adenoidectomy in 3 cases and turbinoplasty (inferior turbinectomy) in 10 cases. RESULTS: In our material we did not observe any complications during and after surgery. The healing process is very good without obstruction and crusting. The shaver system has an excellent protection for surrounding tissue and minimalized the tissue destruction. CONCLUSIONS: Shaver is an excellent powered instruments for rhinosurgery especially massive polyps of nose and sinuses. It is very sufficient to prevent normal tissue and remove pathological one, this can be achieved in no bleeding area.

[Directional preponderance of nystagmus].

The presence of directional preponderance in the peripheral and central vestibular disorders and the relation of its phenomenon to the definitive etiological factors, spontaneous nystagmus, positional nystagmus and the role of lateralization of directional preponderance were described. Directional preponderance is helpful to distinguish between peripheral and central lesions. The direction of this phenomenon is significant in the supratentorial CNS disturbances. In the peripheral lesions it is possible to estimate the progression of the disease. The coexistence of the directional preponderance, spontaneous nystagmus and positional nystagmus has the diagnostic value in neurootology.

The value of the examination of visuooculomotor reflexes in diagnosis of posterior cranial fossa lesions.

The aim of this study was to compare the frequency of pathological types of smooth-pursuit and saccadic movements in different localizations of vestibular lesions. We tested 112 patients using videonystagmography. The smooth pursuit was appreciated qualitatively on the basis of Malecki's patterns. We analyzed the saccadic movements taking three parameters into consideration: latency, velocity, and accuracy. The patients suffered from posterior cranial fossa lesions, supratentorial damage, and peripheral vestibular disorders. We discovered that testing of smooth pursuit and saccades was very helpful in pointing to the localization of the damage in the posterior fossa. The frequency of the pathological saccadic and eye-tracking movements was similar for the different sites of deficit inside the posterior fossa, so recognizing the precise localization of lesion in this anatomical region was difficult.

[Cervical vertigo--etiology, diagnosis and therapy].

The aim of the study was to analyze the reasons and actual methods helpful in diagnostic procedures of cervical vertigo. The vascular, proprioceptive and autonomic etiologies were underlined as responsible for cervical vestibular syndrome. The electrophysiologic methods were introduced together with the visual ones. The treatment was suggested to be directed to the etiology. Rehabilitation, pharmacotherapy, magnetic stimulation and surgical procedures must be taken into consideration in treatment patients with cervical vertigo.

[Select neuro-otologic procedures in vertigo cases].

The aim of the study was evaluation of central vestibular disorders in the scope of vestibular ocular reflexes disturbances in three planes: roll, pitch and yaw. The topo-diagnostic value of ocular tilt reaction, vertical nystagmus, rotational and horizontal nystagmus in central lesions was discussed.

Saccadic and smooth-pursuit eye movement in neurootological diagnostic procedures.

The origin of the generation of saccadic and smooth-pursuit eye movements has not been proved. The goal of the study reported here was to search for the connection of visuooculomotor reactions--saccades and eye-tracking test--with cerebellar damage. Using electronystagmography, we tested 11 patients treated with aminoglycosides because of pneumonia. After a mean of 21 days of pharmacological therapy, a neurological examination revealed cerebellar disability in all cases. On the basis of our previous studies, we suspected cortical Purkinje cell damage. In all tested patients, saccadic movements were disturbed: We observed overshoot, undershoot, and prolonged latencies. In seven patients, eye-tracking test results were not correct: We noted the degradation of the sinusoidal pattern most often. The results pointed to a connection between the generation of these two tested visuooculomotor reactions with cerebellar diseases possibly located in the cortex.

[Influence of the cerebellum on the postural labyrinth reflex--experimental and clinical data].

The subject of the study was to estimate the influence of the cerebellum and the postural labyrinth reflex. The pigeons after hemicerebellectomy or vermis ablation this reflex was analysed during body rotation around sagittal and transversal axis. In the clinical part of observations, stabilometric measure of head motion during balance on the movable platform was performed in patients with supratentorial and cerebellar tumors. The results showed the greatest reflex disturbances in vermis ablation and vermis tumors. In hemispheric tumors of the cerebellum these disorders were slighter, but greater in comparison with supratentorial lesions. The authors stressed the role of the vermis in controlling of the reflex.

Ozone therapy and pressure-pulse therapy in Ménière's disease.

The aim of this study was to evaluate the usefulness of ozone and pressure-pulse therapies in treating Ménière's disease. Using objective otoacoustic emissions and short-increment sensitivity index (SISI) tests together with subjective anamnesis, we tested 15 patients (8 men, 7 women) who had suffered from Ménière's disease for 1-3 years and had permanent sensorineural hypoacusis; we compared results before and after treatment. We performed ozone therapy and pressure-pulse treatments simultaneously for one 10-minute session each day for 10 consecutive days. After treatment, both otoacoustic-spontaneous and transiently evoked emissions and SISI test results exhibited no statistically significant changes. However, the subjective state of the patients was clearly improved. The frequency, severity of attacks, and tinnitus decreased. The mechanism of such an improvement is discussed.

[The influence of the cervical proprioception on the postural reflexes--experimental and clinical study].

The aim of the study was to estimate the effect of the neck proprioception on the postural reflexes in the cases of the cervical pathology in the whiplash injury and degenerative disease of the cervical vertebral column. The investigation using ENG and stabilometry was performed in 81 patients. Experimental observations were done among 10 pigeons with limitation of the neck proprioceptors action by special neck tutor. Their postural reflexes were analysed during balance test on the seesaw. Our conclusion is, that disorders of the proprioceptors impulastion can damage the postural reflexes even in patients with normal labyrinth function.

Betahistine in vertebrobasilar insufficiency.

The aim of this study was to observe the usefulness of betahistine dihydrochloride--Betaserc--in therapy for vestibular disorders in patients with vertebrobasilar insufficiency. Two groups of patients, in each of which were 150 patients (mean age, 52.2 years), were tested on the basis of videonystagmography and stabilometry. Betaserc was administrated in two separate doses: 8 mg three times daily and 16 mg three times daily for 120-180 days (mean, 132 days). In every case before and after therapy, visuo-oculomotor and vestibulo-oculomotor reflexes were tested, and amplitude and velocity of the sway were measured during dynamic posturographic testing. After Betaserc treatment, pathological visuo-oculomotor reactions and pathological cervical test results disappeared in most cases: Smooth pursuit improved in 59.9% of cases and saccadic movements in 55.9% of patients, and cervical nystagmus disappeared in 62.2% of tested people. During stabilometry, mean and maximal platform amplitude and mean head velocity decreased as compared with results from tests performed before treatment. These observations were significant after the greater dose of Betaserc; nonetheless, improvement was noted after both doses. The usefulness of Betaserc in vertebrobasilar insufficiency was proved, 4-6 months' therapy was sufficient, and the effect on central compensation seemed to be most probable.

[Vestibular peripheral organ in chronic otitis media].

The aim of the study was to estimate the function of the vestibular peripheral end organ in patients with chronic otitis media. In the group of 60 cases, 31 female, 29 male, in the age from 27 to 49 years, the electronystagmographic and stabilometric analysis of vestibular-oculomotor and vestibular-spinal reflexes was performed. The results showed that inner ear dysfunction was mainly observed in cholesteatoma cases. The otolith apparatus disturbances were not often present in comparison with the lesion of the ampullar receptor.

[Hyperinsulinemia in vertigo, tinnitus and hearing loss].

48 patients (25 woman, mean age 42 +/- 9.9 years and 23 men, mean age 46.6 +/- 8.3 years) suffering from vertigo, tinnitus and hearing loss of unknown origin were included into this study. The occurrence and coexistence of symptoms was measured. Insulin levels were measured fasting and at the second hour of OGTT. Results were compared with the control group of 31 healthy persons (16 women, mean age 41.3 +/- 5.6 years and 15 men, mean age 47.6 +/- 9.4 years). The character of vertigo, localization of hearing loss and electronystagmographic findings showed the pathology of the inner ear. The occurrence of hyperinsulinemia was significantly more common in the patients group--43.8%, comparing to the control group--22.6%. Also the insulin levels in the second hour of OGTT, were statistically significantly higher in patients that in the control group.
