[Juvenile angiofibroma: the value of CT and MRI for treatment planning and follow-up].

Juvenile angiofibroma is a rare, benign, hypervascular, nasopharyngeal tumour. The aim of the study was to assess the value of CT and MR in determining therapeutic approach and follow-up in patients with this condition. There were 40 patients with juvenile angiofibroma. All patients underwent angiography. In 24 patients CT and 5 patients MRI was performed. Most patients (67%) presented with tumour stage III according to Chandler. Preoperative tumour staging based on CT was in accordance with surgical evaluation in 92% of cases. In 4 (17%) of 24 patients CT and MRI showed a recurrence after surgery. On the bases of radiological findings 2 patients were referred to irradiation instead of surgery. CT allows correct tumour staging in 92% of patients. It well demonstrates the presence and extension of the recurrence.

[Transglabellar, transorbital approach for anterior skull base tumors].

Transglabellar, transorbital approach through the bottom of the frontal sinus for tumours invading anterior skull base in presented. Exposure was improved by rhinectomy and lateral swing of the nose. The most common tumours in the group of 11 patients were squamous cell carcinoma (4) and olfactory neuroblastoma (4). Skull base was excised en block with partial maxillectomy in 8 patients and with total maxillectomy in 3 patients. Orbitectomy was performed in 6 cases. Dura defect was closed with fascia lata and fibrin glue. No serious complications after surgery developed. Rhinoscopy and computed tomography demonstrated clear cavity and no dura prolapse. Transglabellar, transorbital approach provides good exposure for anterior skull base tumours excision and repair.

[Renal cell carcinoma metastases to nose and paranasal sinuses presenting as recurrent epistaxis].

Distant metastases in head and neck region are very uncommon. Nasal bleeding was the first symptom in a patient with metastasis from renal cell carcinoma. Nasal tumour biopsy revealed renal carcinoma to be the primary lesion. Initially the patient refused surgery and nephrectomy was performed 8 months later. MR scans demonstrated diffuse intracranial invasion which made complete removal impossible. Potential routes of renal carcinoma spread are presented. Renal cell carcinoma should be considered in the diagnosis of nose and paranasal tumours. Epistaxis can be the first symptom of renal carcinoma metastasis to the nose and paranasal sinuses.

[Cat-scratch disease of the parotid gland].

Cat-scratch disease is a mild disease associated with regional lymph node enlargement. We present a case of cat-scratch disease of the parotid gland in 10-year-old girl. She presented with painful parotid mass, after unsuccessful treatment with antibiotics. Computed tomography showed tumour like lesions in parotid gland. Subtotal parotidectomy with facial nerve preservation was performed as neoplastic disease was suspected. Histopathology examination of the sample revealed typical changes for cat-scratch disease. History of continuous contacts with cats was confirmed by the parents. Cat-scratch disease should be taken into account in differential diagnosis of parotid masses, especially in children.

[Methods of closure for pharyngocutaneous fistula after laryngectomy].

The majority of salivary fistulas after laryngectomy or after laryngo-pharyngectomy close spontaneously within 2-3 weeks, and the remaining require surgical reconstruction. In the years 1987-2000 the pharyngo-cutaneous fistula was closed in 19 patients. The fistula developed after laryngectomy because of larynx cancer T3 and T4. 13 patients were irradiated before. In all the patients laryngectomy was combined with unilateral or bilateral neck dissection. Salivary fistula was closed in one stage operation using pectoralis major myocutaneous flap: 1) skin island of the flap to replace defect of the neck skin and mobilized mucosa from inside, 2) skin island for lining and deltopectoral skin flap from outside, 3) skin island for lining and free split skin flap for external cover, 4) skin island of the flap divided in two paddeles, one for lining and another for external cover. Indications for each method were discussed. Very good result of total fistula closure was obtained in 15 out of 19 patients. The fistula occurred again in 4 patients, in two of them the failure was caused by cancer recurrence.

[Glasgow Benefit Plot for evaluation of stapedectomy result].

In a group of 153 patients, the result of unilateral stapedectomy was evaluated using Glasgow Benefit Plot. Pure tone average at frequencies 0.5; 1; 2; and 3 kHz of operated and non-operated ear were used to distribute patients to pre- and post-operative groups. In 26 (79%) of 33 patients with unilateral hearing loss, bilateral normal hearing was achieved. Thirty one (46%) of 68 patients with asymmetric bilateral hearing loss and 37 (71%) of 52 patients with symmetric bilateral hearing loss had unilateral normal hearing after the operation. Twenty (29%) patients of the group III had bilateral symmetrical hearing loss after surgery. Stapedectomy was less beneficial for 17 (25%) of 68 patients with asymmetric hearing bilateral loss and 15 (29%) 52 of patients with symmetric bilateral hearing loss, who still had asymmetric hearing loss after the operation. Evaluation of hearing tests using the Glasgow Benefit Plot enables to evaluate patient's hearing disability and to predict possible benefit from surgery for individual cases.

Bone and air conduction after stapedectomy.

UNLABELLED: Otosclerosis causes loss of air conduction (AC) but also bone conduction (BC) may be deteriorated. It is reported that also BC may improve after surgical treatment of otosclerosis. The investigation was carried out in a group of 50 patients treated due to otosclerosis in our institution. The group included 34 women and 16 men, at the age 24 to 55 years. Pure-tone audiometry for bone and air conduction threshold was performed in all the patients. Hearing tests were carried out before and 2-4 months after the operation by the same technician Partial. Only patients with air-bone gap closure were included in the study. Stapedectomy was performed under local anaesthesia. RESULTS: Significant improvement of AC was obtained after stapedectomy. There was also significant improvement of BC at all the frequencies examined of 6-12 dB and it depended on BC threshold before surgery. In the group of 37 patients with BC threshold before surgery > or = 30 dB a significant improvement of BC after stapedectomy developed. In the group of 14 patients with BC threshold before surgery < 30 dB the change of BC after stapedectomy was not significant. Successful stapedectomy improves bone conduction. It means that in patients with otosclerosis there is a reversible BC pseudo hearing loss and it is related to BC threshold before the operation.

Incus subluxation and luxation during stapedectomy.

Stapedectomy is a safe surgical procedure used in patients with otosclerosis. However, complications may occur and decrease hearing gain after the operation. The aim of the work was to analyze hearing results in patients with incus subluxation during stapedectomy. In 15 (5%) of 292 patients with otosclerosis, the incus was unintentionally luxated during surgery. In one patient the incus was completely dislocated and malleovestibulopexy with autoincus had to be applied. In 14 stapedectomies the incus was subluxated and though it was pathologically mobile it was held in position by its ligaments. This situation allowed delicate reposition of the incus and insertion of the piston prosthesis on the long incus process. Mean AC threshold improved by 24 dB. Mean ABG improved from 34.6+/-8.3 dB before to 13.1+/-6.3 dB 8 months after stapedectomy (t=9.7; p<0.0001). Ten years after surgery average ABG was 10.6+/-4.4, which means stable hearing result. In 6 patients including the individual with complete incus luxation, postoperative ABG was less than 10 dB. In 9 patients postoperative ABG was between 10 and 15 dB. When the incus is subluxated good hearing results can be expected after insertion of the prosthesis on the repositioned incus.

[Results of stapedectomy in osteogenesis imperfecta].

Osteogenesis imperfecta is a connective tissue disease connected with improper synthesis of collagen type I. Bone fragility, blue sclera and hearing loss are the main symptoms of classic type of the disease. Early (6 months) and late (10 years) results of stapedectomy in 3 patients with osteogenesis imperfecta syndrome are presented. Six months postoperatively air-bone gap was smaller than 10 dB in 2 patients and 17 dB in one patient. In all patients, bone conduction pure tone threshold improved after the operation. Audiometric tests performed 10 years after stapedectomy were similar to those obtained 6 months after the operation. Stapedectomy in patients with osteogenesis imperfecta syndrome provides good and stable hearing results.

[Parotid metastases from breast cancer].

Metastatic tumours in parotid gland are very uncommon, and they usually reflect the lymphatic drainage from the head and neck region. The most frequent types of the metastatic tumours are squamous cell carcinoma and malignant melanoma. Metastases to parotid gland from subclavicular area are extremely rare. Two patients with breast cancer and metastases to parotid region are presented. One patient developed metastases in parotid gland 15 years after surgery and radiotherapy of breast cancer. In the latter patient parotid tumour was the first manifestation of the disease. Both patients underwent parotid surgery. None of the patients survived more than 6 months. Metastases in parotid region carry bad prognosis for the patients.

[Change in otosclerosis presentation over two decades].

Presentation of otosclerosis in eighties and nineties was compared on the basis of analysis of patients' documentation. A group I of 80 patients treated in 1980-1990 and a group II of 148 patients treated in 1991-2000 were compared with regard to sex, age, incidence of oval window obliteration and bilateral disease. Bone conduction and air conduction threshold and air-bone gap were also compared. There was no difference between two groups regarding sex, age and bone conduction threshold. Oval window obliteration was found in 8 (10%) patients of group I and in 7 (4.7%) patients of group II and the difference was not significant. Bilateral otosclerosis was diagnosed in 77 (96.3%) patients of group I and in 121 (81.8%) patients of group II and the difference was significant (p < 0.01). Air conduction threshold and air-bone gap in patients treated in nineties were significantly smaller than in eighties (p < 0.001). In the last decade patients with otosclerosis presented with smaller hearing loss, smaller air-bone gap and had bilateral disease less frequently than patients in eighties.

Labyrinthitis in chronic otitis media.

Labyrinthitis is the most frequent complication of chronic otitis media (COM). Retrospective analysis of 1,210 patients operated due to COM was carried out. Circumscribed labyrinthitis was found in 17.2% and deafness in 1.2% of patients. To evaluate the effect of labyrinthitis on bone conduction (BC) we measured BC threshold in a group of 140 patients with unilateral COM without labyrinthitis and in a group of 42 patients with unilateral COM with labyrinthitis. BC threshold of the healthy ear was subtracted from the BC threshold of the affected ear in the same patient to eliminate presbyacusis. There was no significant difference in bone conduction threshold between the patients with circumscribed labyrinthitis and patients with COM without labyrinthitis. The incidence of labyrinthitis was higher in patients with cholesteatoma than in patients without cholesteatoma. Management of labyrinthine fistula is presented.
