Pulmonary exposure to diesel exhaust particles enhances fatty change of the liver in obese diabetic mice.

In epidemiological studies, exposure to ambient particulate matter (PM) has been reported to be positively associated with mortality in subjects with diabetes mellitus. Diesel exhaust particles (DEP) are major constituents of atmospheric PM. However, there is no experimental evidence for the relation of DEP to diabetes mellitus and its complications. We investigated the effects of DEP inoculated intratracheally on diabetic changes and nonalcoholic fatty liver disease (NAFLD) in diabetic obese and control mice. db/db mice and the corresponding nondiabetic db/+m mice received exposure to vehicle or DEP every two weeks. Animals were examined with biochemistry, histology, and immunohistochemistry for hexanoyl-lysine (HEL) in the liver. In the db/+m mice, pulmonary exposure to DEP did not increase levels of aspartate aminotransferase (AST) or alanine aminotransferase (ALT) compared to that to vehicle. In the db/db mice, however, the exposure to DEP increased the levels of AST and ALT compared to that to vehicle. Only in the db/db mice, DEP enhanced the magnitude of steatosis and formation of HEL, a marker of oxidative stress, in the liver compared to vehicle. These results suggest that pulmonary exposure to DEP, PM, enhances steatosis in the liver of obese diabetic subjects possibly via enhanced oxidative stress.

Lack of ghrelin secretion in response to fasting in cholecystokinin-A (-1), -B (-2) receptor-deficient mice.

Cholecystokinin receptors (CCK-Rs) have been classified into two subtypes: CCK-AR (1R) and -BR (2R). We generated CCK-AR(-/-), CCK-BR(-/-), and CCK-AR(-/-)BR(-/-) mice and found that the gastric emptying of a liquid meal was increased in CCK-BR(-/-) and AR(-/-)BR(-/-) mice, compared with wild-type and CCK-AR(-/-) mice. Given that enhanced gastric emptying leads to eating, food intake after overnight fasting was examined, as was the effect of CCK-8S on food intake. Male mice 6-8 months of age were deprived of food for 16 h with free access to water, after which they were injected intraperitoneally (0.1 ml/mouse) with either vehicle or CCK-8 (0.3, 1.0, or 3.0 nmol/mouse), and their food intake was monitored for 4 h. CCK-8S inhibited food intake in wild-type and CCK-BR(-/-) mice, but not in CCK-AR(-/-) or AR(-/-)BR(-/-) mice. Unexpectedly, we observed a lower food intake in CCK-AR(-/-)BR (-/-) mice treated with vehicle than in mice of the other genotypes. To examine the mechanism of decrease in food intake in CCK-AR(-/-)BR(-/-) mice, the involvement of ghrelin was determined in wild-type and CCK-AR(-/-)BR(-/-) mice. Fasting plasma ghrelin levels were significantly lower in CCK-AR (-/-)BR(-/-) mice than in wild-type mice, and no increase in response to fasting was observed in CCK-AR(-/-)BR(-/-) mice. An administration of acyl-ghrelin produced a small increase in food intake in CCK-AR(-/-)BR(-/-) mice, but not to the levels of wild-type mice. In conclusion, CCK-AR(-/-)BR(-/-) mice showed lower food intake as well as lower response to exogenous ghrelin, and a lower plasma ghrelin level after fasting, though which receptor is more important is unknown.

Age- and cell cycle-dependent changes in EPC-1/PEDF promoter activity in human diploid fibroblast-like (HDF) cells.

The changes in gene expression during senescence are very interesting. Early population doubling cDNA-1 (EPC-1, also known as pigment epithelial derived factor, PEDF) is one of the genes whose expression decreases dramatically during cellular aging. We examined whether or not EPC-1/PEDF promoter activity was affected by the cellular ageing using human diploid lung fibroblast cells in culture. Here we showed the promoter/enhancer region of EPC-1/PEDF existed at more than 1760 bp upstream from the transcriptional initiation site of the gene, and was regulated by both aging and cell cycle. These findings suggest that the expression of the EPC-1/PEDF gene is, at least in part, regulated transcriptionally in the cells. The analysis of the promoter region of the EPC-1/PEDF gene in this paper suggests the age- and cell cycle-dependent expression of specific transcriptional factor(s).

Relationship of cariogenic bacteria levels with periodontal status and root surface caries in elderly Japanese.

OBJECTIVE: The relationship of the levels of cariogenic bacterial species with periodontal status and decayed root surfaces was investigated in elderly Japanese subjects. METHODS: Three hundred and sixty-eight individuals (each 75 years old) were examined for periodontal status (pocket depth, attachment loss), root surface caries and salivary levels of mutans streptococci (MS) and lactobacilli (LB). RESULTS: Values >4 mm of attachment loss (rAL4) and for average attachment loss (aAL) of sites measured were significantly higher in subjects with LB than those without. Multiple regression analysis also showed a correlation between aAL and rAL4 values with the presence of LB (aAL p = 0.003; rAL4 p = 0.002). Further, multiple regression analysis of interacting factors regarding decayed root surfaces showed that LB carriers had a greater incidence of decayed root surface caries (p = 0.003), while MS and LB levels were correlated to the number of decayed root surfaces (LB p = 0.010; MS p = 0.026). CONCLUSION: Our results indicate that considerable attachment loss elevates the possibility of having LB, thus increasing the risk of root surface caries. It was also found that LB and MS measurements may be useful indicators of decayed root surfaces in elderly individuals with attachment loss.

[Dentistry supports eating function in the aged].

As demographic aging continues even today, geriatric dentistry responsive to the times must be established. The greatest concern of elderly individuals is health, and as dentists, we care for the oral cavity as the gateway to the digestive organs. Stated briefly then, I believe that our primary role as dental health care providers in an elderly society is to maintain and promote health by preventing a decline in eating function. Dentistry up to the present has pursued responses to disease. But in an elderly society, a decline in oral function accompanying aging is a problem. In other words, consistent with changes in the population structure, dentistry in the future should not end with the treatment of tooth decay, periodontal disease, or other such dental diseases; rather, it must pursue a shift to prevention of age-attendant decline in oral function and maintenance of healthy food intake and swallowing functions. Nonetheless, dentistry is gradually declining in hospitals that admit elderly individuals. While urban dental clinics are regarded as too numerous, dentistry in hospitals is disappearing, and the thought of health care settings without dentistry is sobering. I believe we must eliminate a troubling situation in which our connection to dentistry is severed at a time when we most need dentistry. This should be the first step in establishing dentistry for maintenance of eating functions.

Videofluoroscopic kinesiologic analysis of swallowing: defining a standard plane.

PURPOSE: The Videofluorographic Swallowing Study (VFSS) has become the routine method for assessing swallowing dysfunction and accordingly, much research has been conducted on this procedure. However, due to a lack of standardization of the method of analysis of VFSS, it is often difficult to compare the results of such studies. Therefore, we conducted a comparative study of VFSS's spatial measurement using different standard planes used in the past study and Camper's plane and examined which plane was the most preferable. METHOD: VFSS was performed on 20 healthy young volunteers (26.9 +/- 3 years) and 9 healthy elderly (77.3 +/- 3 years). Each subject swallowed 4ml of thin liquid barium. We measured hyoid displacement and opening of the upper esophageal sphincter (UES) by using four different standard planes. RESULTS: In the young group, the correlation between anterior hyoid displacement and UES opening was significant in all standard planes. In the elderly group, the correlation between anterior hyoid displacement and UES opening was significant only in Camper's plane. Moreover, this plane is hard to be affected by morphological change as ageing. CONCLUSIONS: Camper's plane was found to be as the most preferable plane for analyzing swallowing function.

Na+/K+ ATPase and its functional coupling with Na+/Ca2+ exchanger in mouse embryonic stem cells during differentiation into cardiomyocytes.

Cardiomyocytes derived from mouse embryonic stem (mES) cells have been demonstrated to exhibit a time-dependent expression of ion channels and signal transduction pathways in electrophysiological studies. However, ion transporters, such as Na+/K+ ATPase (Na+ pump) or Na+/Ca2+ exchanger, which play crucial roles for cardiac function, have not been well studied in this system. In this study, we investigated the functional expression of Na+/K+ ATPase and Na+/Ca2+ exchanger in mES cells during in vitro differentiation into cardiomyocytes, as well as the functional coupling between the two transporters. By measuring [Na+]i and Na+ pump current (Ip), it was shown that an ouabain-high sensitive Na+/K+ ATPase was expressed functionally in undifferentiated mES cells and these activities increased during a time course of differentiation. Using RT-PCR, the expression of mRNA for alpha1-subunit and alpha3-subunit of the Na+/K+ ATPase could be detected in both undifferentiated mES cells and derived cardiomyocytes. In contrast alpha2-subunit mRNA could be detected only in derived cardiomyocytes but not in undifferentiated mES cells. mRNA for the Na+/Ca2+ exchanger 1 isoform (NCX1) could be detected in undifferentiated mES cells and its expression levels seemed to gradually increase throughout the differentiation accompanied by increasing its Ca2+ extrusion function. At the middle stages of differentiation (after 10-day induction), more than 75% derived cardiomyocytes exhibited [Ca2+]i oscillations by blocking of Na+/K+ ATPase, suggesting the functional coupling with Na+/Ca2+ exchanger. From these results and RT-PCR analysis, we conclude that alpha2-subunit Na+/K+ ATPase mainly contributes to establish the functional coupling with NCX1 at the middle stages of differentiation of cardiomyocytes.

Influences of aging on taste perception and oral somatic sensation.

BACKGROUND: Many elderly persons report reduced taste perception of the foods they eat. Any disturbance of taste and oral somatic sensations can induce this phenomenon. To determine the cause of decreased taste perception in older persons, the authors investigated age-related changes in taste perception and somatic sensations in the anterior tongue. METHODS: Thirty healthy young and elderly persons participated in this study. Recognition thresholds of four basic tastes (sweet, salty, sour, and bitter) and the threshold of irritating sensation induced by capsaicin were measured by local stimulation using filter-paper disks. Two-point discrimination thresholds were measured to evaluate the spatial acuity of touch sensation. All measurements were carried out at the tip of the dorsal tongue. RESULTS: The recognition thresholds of all four basic tastes of elderly participants were significantly higher than those of young participants. On the contrary, age-associated changes were not observed in 2-point discrimination and capsaicin sensation. CONCLUSIONS: Significant age-associated deterioration was observed in taste but not somatic sensations such as touch and burning pain in the tongue, showing that aging affects taste perception and oral somatic sensations differently. This suggests that decreased taste perception of foods in elderly people may be caused primarily by perceptual loss of taste among oral sensations.

[The effect of "Oral Wet" for elderly people with xerostomia--the effect of oral rinse containing hialuronan].

Japanese society is aging rapidly, and the number of elderly people is increasing, hence the number of elderly people who have some diseases or disorders is growing. Therefore, Japanese dentists have to cope with various oral symptoms along with aging. Xerostomia is one of the most common symptoms of the elderly, but there is no established treatment. We tried to use an oral rinse "Oral Wet" that contains hialuronan and xylitol for elderly nursing home residents and examined the effect for Xerostomia. The usage of Oral Wet improved hyposalivation and unpleasant oral complaints of the elderly.

Orexin-A does not stimulate food intake in old rats.

Aging is associated with a progressive decrease in appetite and food intake. Both A and B orexins, expressed in specific neurons of the lateral hypothalamic area, have been implicated in the regulation of sleep and feeding. In this study, the stimulatory effect of intracerebroventricular administration of the orexins on food intake was compared between young (4-mo-old) and old (25- to 27-mo-old) male Wistar rats. A stainless steel cannula was implanted stereotactically into the left lateral ventricle. After a 7-day recovery period, different doses (0-30 nmol) of orexins were injected into the left lateral ventricle without anesthesia. Food and water consumptions were measured at 1, 2, and 4 h after injection. The protein levels of orexin receptors, a specific receptor for orexin-A (OX1R) and a receptor for both orexin-A and -B (OX2R), in the hypothalamus were determined by Western blot analysis and compared between young and old rats. Intracerebroventricular administration of orexin-A stimulated food intake in a dose-dependent manner in young rats. However, no effects were observed at any dose in old rats. The protein level of OX1R in the hypothalamus was significantly lower in old rats than in young rats, although the protein level of OX2R was comparable between groups. Results of the present study indicate that the function of the orexin system is diminished in old rats. The decrease in the OX1R protein level in the hypothalamus could be responsible for orexin-A's lack of stimulation of food intake in old rats.

Regulation of osteoclastogenesis by three human RANKL isoforms expressed in NIH3T3 cells.

Receptor activator of nuclear factor-kappaB ligand (RANKL) induces osteoclastogenesis by binding with the receptor, receptor activator of nuclear factor-kappaB in the presence of macrophage colony-stimulating factor. Three human RANKL isoforms, hRANKL1, hRANKL2, and hRANKL3, were identified. hRANKL1 was identical to previously reported RANKL and possessed intracellular, transmembrane, and extracellular domains, hRANKL2 did not have the intracellular domain, and hRANKL3 did not have the intracellular and transmembrane domains. When bone marrow macrophages were cultured with NIH3T3 cells expressing hRANKL1, osteoclasts were formed, but when cultured with NIH3T3 cells expressing hRANKL2 or hRANKL3, no tartrate resistant acid phosphatase-positive cell was observed. In the coculture system, coexpression of hRANKL3 with hRANKL1 significantly inhibited the formation of osteoclasts by hRANKL1, but coexpression of hRANKL2 with hRANKL1 did not affect the osteoclastogenesis by hRANKL1 significantly. These results suggest that the activity of osteoclastogenesis by hRANKL1 is regulated by the attenuator, hRANKL3.

Functional expression of Ca2+ signaling pathways in mouse embryonic stem cells.

Mouse embryonic stem (mES) cells have the potential to differentiate into all types of cells, but the physiological properties of undifferentiated mES cells, including Ca2+ signaling systems, are not fully understood. In this study, we investigated Ca2+ signaling pathways in mES cells by using confocal Ca2+ imaging systems, patch clamp techniques and RT-PCR. The stimulations with ATP and histamine (His) induced a transient increase of intracellular Ca2+ concentration ([Ca2+]i), which were prevented by the pretreatment of 2-amino-ethoxydiphenyl borate (2-APB), a blocker for inositol-1,4,5-triphosphate receptors (InsP3Rs). The application of caffeine (Caff) or ryanodine (Ry) did not change [Ca2+]i. When stores were depleted with Ca2+ -ATPase blocker, thapsigargin (TG), or histamine, the capacitative Ca2+ entry (CCE) was observed. In whole cell patch clamp mode, store-operated Ca2+ currents could be recorded in cells treated with histamine and thapsigargin. On the other hand, voltage-operated Ca2+ channels (VOCCs) could not be elicited. The application of blockers for plasma membrane Ca2+ pump (PMCAs) (carboxeosin or caloxin2A1) induced a large increase of [Ca2+]i. When the Na+/Ca2+ exchangers (NCXs) were blocked by Na+ free solution or KBR7943, [Ca2+]i was also elevated. Using RT-PCR, mRNAs for InsP3Rs type-1, -2, and -3, PMCA-1 and -4, NCX-1, -2, and -3 could be detected. From these results, we conclude that Ca2+ release from ER is mediated by InsP3Rs in mES cells before differentiation and Ca2+ entry through plasma membrane is mainly mediated by the store-operated Ca2+ channels (SOCs). For the Ca2+ extrusion systems, both NCXs and PMCAs play important roles for maintaining the low level of [Ca2+]i.

Association between dental health behaviours, mental/physical function and self-feeding ability among the elderly: a cross-sectional survey.

OBJECTIVES: The aim of this study was to determine the association between dental health behaviour, mental/physical function and self-feeding ability among the elderly. SUBJECTS: A total of 414 elderly dental patients aged 65 years and older participated in this study. METHODS: A survey was carried out for three years and seven months starting in January 1998 at the Chubu National Hospital. The patients or their carers were examined/interviewed about the severity of senile dementia, dental health behaviour, ability to rinse their mouths, ability to manage dentures, and ability to sit at a table during meals. To assess the association with self-feeding ability among the elderly, cut-offs were given for these variables, and then the odds ratios were calculated. RESULTS: The strongest association to self-feeding ability was marked by inability to rinse their own mouth, followed by inability to manage dentures, inability to sit at a table during meals, severe senile dementia and less frequency of toothbrushing. CONCLUSION: Elderly who have lost the feeding ability often could not maintain their dental health by themselves. Carers must provide not only a feeding service with acknowledgement of aspiration but oral care to prevent dental disease and fatal pneumonia in the elderly.

Improvement of taste sensitivity of the nursed elderly by oral care.

It has been suggested that oral care is essential for improving the quality of life (QOL). The aim of oral care involves not only maintenance of oral health but also enhancement of mastication and appetite by means of improving taste sensitivity. There are, however, few studies that have investigated the effect of oral care on taste sensitivity. In the present study, we focused on tongue brushing among oral care and examined the changes in taste sensitivities by mild tongue brushing in the nursed elderly. Ninety subjects, over 64 years old with ability to communicate, were divided into 2 groups, one was the cared group (subject's tongue was brushed) and the other was the control group (not brushed but rinsed). The thresholds for four primary tastes such as saltiness, sourness, sweetness and bitterness were measured before and after treatments using the whole mouth method. The mean recognition thresholds for salty and sour tastes significantly decreased after tongue brushing in the cared group, while those for all four tastes were not changed after mouth rinsing in the control group. The present result suggests that mild tongue brushing may enhance taste sensitivity of saltiness and sourness in the nursed elderly.

Factors affecting denture use in some institutionalized elderly people.

This study was conducted to assess which factors were related to the ability of elderly residents of nursing homes to use dentures. For 205 patients in two nursing homes in Chuou-ku, Tokyo, dentists and caregivers examined oral status (caries, status of restoration, missing teeth and occlusal support) as well as denture use, Activities of Daily Living (ADL), ability to rinse the mouth, and degree of dementia. Fifty percent of subjects lacked dentures. Seven items were selected for variance in factor analysis. There was a significant relationship between denture use and all analyzed items except age. The items which proved to be significant, included the number of remaining teeth, the ability to rinse, communication, ability to dress/undress, ability to transfer, and eating (Fisher's exact test). A multiple stepwise logistic regression analysis showed that the nonuse of dentures was significantly related to dressing (p<0.01), the number of remaining crowns (p<0.01) and the ability to rinse the mouth (p<0.0001). The nonuse of dentures was also related to the number of remaining teeth and to the disability of hand and oral functions.
