An ontological approach to support the description of nursing practice in Japan with the ICNP.

BACKGROUND: With increasing computerization of nursing records in Japan, standardization of nursing terminology is becoming imperative. Although some efforts have been made to formalize description of nursing practice in Japan with the International Classification of Nursing Practice (ICNP), lack of effective description tools has impacted negatively on the initiatives. PURPOSE: To develop and evaluate an ontological approach that could be used to facilitate the description of nursing practice in Japan with the ICNP. METHODOLOGY: An ontology-based support system was developed using Protégé-2000, mainly by the following three steps: (1) representing a standard classification of nursing practice (the Nursing Master) in Japan; (2) representing a Japanese version of the ICNP; (3) designing an ontology-based framework. A heuristic matching algorithm was developed to automatically match the action labels in the Nursing Master with the terms of the eight axes of the ICNP Nursing Actions Classification. A preliminary evaluation was performed to examine the usefulness of the system. RESULTS: High hit rate was shown on the ICNP axes ActionType, Target, and Location. The evaluation indicated that 51.7+/-5.8% (mean+/-S.D.) of the action labels with only one action type were properly matched, and that in 80+/-4% (mean+/-S.D.) of action labels with more than one action type, at least one valid action type was matched correctly. CONCLUSION: The ontology-based approach using a frame-based knowledge representation system (e.g., Protégé-2000) is useful for supporting the formal description of nursing practice in Japan with the ICNP.

[Interpersonal relationships experienced in college life and development of social skills].

The purpose of this study is to clarify the relation between the development of social skills and students' experience in interpersonal relationships during their college life, focusing on what factors are most influential. The study was conducted based on a questionnaire regarding social skills, attitudes toward college life, clinical practice, lectures and interpersonal relationships, etc., distributed to 72 senior students from a 4-year nursing college as subjects. As a result, it was found that when students had to meet hardships regarding interpersonal relations during their college life, those who had two or more coping tools were able to develop more social skills than those who had to cope with the difficulty on their own. Moreover, students who had part-time jobs, took part in two or more extracurricular activities or overcame the difficulty of interpersonal relations during school festivals were reported to have improved their social skills. From these findings, one can speculate that the more quantity and the higher quality of interpersonal relationships students experience in their college life, the more and the higher-level social skills they can develop. It was also shown that the negative attitude toward interpersonal relationships as well as the difficulty students felt, during clinical practices worked as negative factors that might arrest the development of their social skills. It is therefore suggested that during clinical practices teachers should commit themselves positively to helping students cope with difficult situations or mend their negative attitudes toward interpersonal relations.

Extraction of specific nursing terms using corpora comparison.

The purpose of the study is to develop and evaluate a bottom-up approach to extract the specific nursing terms from textual nursing records using corpora comparison. A nursing records corpus was developed as the target corpus, and a newspaper corpus and a medical literature abstracts corpus were developed as the reference corpora. Two filters were established to extract the technical terms and the relative frequency ratio was used for corpora comparison. The issues related to the improvement of both the algorithms and the evaluation methods were discussed.

[Evaluation of students' learning through simulation experience study in gerontological nursing education--understanding the elderly person and the role of the caregiver].

The purpose of this study was to clarify the learning of the students who participated in an elderly person simulation experience study. We analyzed the aspects of the students' understanding of an elderly person and the role of a caregiver using the records maintained by the students during their simulation experience study. Based on the results, the students' understanding of an elderly person was classified under six categories, that is, 1) the physical phenomena of aging, 2) the effect of physical phenomena on the activities of daily living (ADL), 3) influence of the vital functions on an elderly person's physical function and health, 4) the effect on the mental state, 5) the constraint on the other persons, and 6) the change in the social status and relationships. The role of a caregiver was classified under two categories; that is, 1) the activities and role of the caregiver, and 2) the adjustment to the environment. In this study, the students perceived not only the physical difficulties but also the emotional and sensitivity levels of the aged. In conclusion, it is thought that the elderly simulation program is effective in developing a better understanding of the physical state of the elderly as well as the importance of providing them with support.

Preliminary clinical evaluation of a video transmission system for home visits.

A portable video transmission system based on a hand-held computer and cellular telephone was developed for use with patients undergoing rehabilitation at home. It was designed to be simple to operate, for use by nurses on home care visits. The whole system can be held in one hand, and video pictures can be recorded and transmitted with the touch of a few buttons. The quality of the transmitted pictures and the feasibility of the system were clinically evaluated by physical therapists and nurses. Three image transmission scenes were used to evaluate the system: turning over in bed; walking; and raising and lowering a shoulder. Nurses reported that the system was easy to carry and simple to use. Physiotherapists found the system to be useful in making a judgement about the patient's condition. The patients were all enthusiastic about the system. The increased interaction had the effect of improving the patients' desire to continue their in-home rehabilitation.

Extracellular proteinase and phospholipase activity of three genotypic strains of a human pathogenic yeast, Candida albicans.

Strains of a human pathogenic yeast, Candida albicans, have (A) intronless, (B) intron-containing, and (C) a mixture of intron-containing and intronless 26S rRNA genes. To elucidate the significance of these three genotypes in pathogenesis, we measured two major virulence factors, extracellular proteinase and phospholipase activity, in 56 clinical isolates of C. albicans, and investigated the relationship between genotype and enzymatic activity. The genotype B strains had significantly higher proteinase and phospholipase activities than genotypes A or C. These results suggest that to understand the pathogenesis of C. albicans, the genotypes should be considered.
