Changes in myocardial collagen content before and after left ventricular assist device application in dilated cardiomyopathy.

BACKGROUND: The purposes of this study were to confirm the changes in myocardial collagen level after left ventricular assist device (LVAD) support in dilated cardiomyopathy (DCM), find the relation between these changes and prognosis, and test a practical method to assess the level of myocardial collagen. METHODS: Left ventricular samples were collected from DCM patients with different prognosis (transplanted group n = 8, weaning group n = 10) at the time when the LVADs were implanted and again during cardiac transplantation (n = 8). The level of neutral salt soluble collagen (NSC) and acid soluble collagen (ASC) was measured by Sircol collagen assay, and that of total collagen and insoluble collagen (ISC) by quantification of hydroxyproline (Hyp). Serum samples were collected from a portion of these patients (transplanted group, n = 6; weaning group n = 7) at the time the LVADs were implanted, 1 month after implantation and on explantation. Circulating concentration of carboxy-terminal propeptide of type I procollagen (P I CP), amino-terminal propeptide of type I procollagen (P I NP), amino-terminal propeptide of type III procollagen (P III NP) and type I collagen telopeptide (I CTP) were measured by the equilibrium type radioimmunoassay. RESULTS: Before LVAD implantation the level of NSC and ISC in the weaning group was higher but ASC in the transplanted group was lower than in the controls (P < 0.05). After LVAD support, the level of total collagen was higher, but ASC was also lower in the transplanted group than in the controls (P < 0.05). In comparison of the pre- and post-LVAD subgroups of the transplanted and weaning groups, all collagen fraction levels before LVAD implantation were lower in the transplanted group than in the weaning group (P < 0.05); but this difference disappeared after LVAD support. Comparison of the pre- and post-LVAD subgroups of the transplanted group showed increased level of NSC and total collagen after LVAD support. The changes of serum peptide concentration showed that P III NP increased constantly in the transplanted group, but P I CP and P I NP increased in the weaning group after LVAD implantation. CONCLUSIONS: The changes in myocardial collagen level as a sign of myocardial interstitial remodeling in DCM are not involved with total collagen but involved with collagen fractions, and they are related to prognosis. The changes of myocardial collagen content and serum procollagen peptide after LVAD support can be regarded as an expression of the reverse of maladaptive myocardial interstitial remodeling.

[Current attitudes and knowledge about suicide in community members: a qualitative study].

OBJECTIVE: Understand the public's current attitudes and knowledge about suicide and, thus, provide essential information to the development of targeted public education programs-important components of the suicide prevention effort. METHODS: Seventeen mental health professionals who were extensively trained in the methods of conducting focus groups used a pre-tested focus group outline on attitudes and knowledge about suicide to conduct 101 focus groups and 18 individual in-depth interviews with a total of 842 community respondents from 6 regions in northern China. The focus groups and in-depth interviews were audio-taped, transcribed and analyzed using the QSR Nvivo text analysis software. RESULTS: Most respondents believed that suicide was a greater problem in rural areas and among women and identified physical illnesses, economic problems and interpersonal conflicts (particularly family conflicts) as the main causes of suicide. Rural residents and women were believed to exhibit impulsive suicidal behavior because of their personal limitations and over-sensitiveness. Most thought that suicide was understandable and a small proportion felt that it was acceptable behavior in certain circumstances. Almost all felt that suicide resulted in the stigmatization and a loss of 'face' for the family. Most believed that one should show concern for persons who have suicidal behavior and their family members and expressed a willingness to have superficial social relationships with them but were unwilling to establish close personal relations with them. The vast majority believed that suicide was either very difficult or impossible to prevent. CONCLUSIONS: In China the community is tolerant, sympathetic and, in some cases, accepting of suicide but there remains a substantial underlying stigmatization of suicide. Community members have some misunderstandings about suicide; the most obvious misunderstanding is the underestimation of the importance of mental illness as a cause of suicide. The content of public health messages used in suicide prevention programs should be developed by combining findings from both qualitative and quantitative research.

Single photon emission computer tomography of dopamine transporters in monkeys and humans with 99mTc-TRODAT-1.

BACKGROUND: The diagnosis of Parkinson's disease is presently based on non-specific symptoms. However, radionuclide dopamine transporters imaging can provide specific diagnostic tool for Parkinson's disease. This study was designed to investigate the effects of imaging of dopamine transporters with 99mTc-TRODAT-1 in early diagnosis or differential diagnosis of Parkinson's disease. METHODS: Nine normal monkeys were used to establish N-methyl-4-phenyl-1, 2, 3, 6-tetra-hydropyridine (MPTP) hemi-Parkinsonian animal models, and they were subjected to imaging. Twenty-nine patients with Parkinson's disease, 12 age-matched healthy volunteers, and 18 age-matched patients with Parkinson's syndrome were investigated. Single photon emission computer tomography (SPECT) was performed 3 hours after intravenous injection of 740 MBq 99mTc-TRODAT-1. Striatum specific uptake of 99mTc-TRODAT-1 was calculated according to the ratio of striatum (ST) to cerebellum (CB) in dopamine transporters uptake. RESULTS: In normal monkeys, bilateral ratio of ST/CB was 2.34 +/- 0.41. After the injection of MPTP, uptake rate of 99mTc-TRODAT-1 at damaged region was much lower than that at the contralateral region, resulting in a significant difference in the ratio of ST/CB (right: ST/CB = 1.73 +/- 0.35; left: ST/CB = 1.90 +/- 0.30), especially in hemi-Parkinsonian model monkeys (right: ST/CB = 1.29 +/- 0.17; left: ST/CB = 1.80 +/- 0.33). The ratios of ST/CB were 1.57 +/- 0.17 and 1.61 +/- 0.14 for the right and left respectively in the healthy volunteers, 1.04 +/- 0.29 and 1.06 +/- 0.30 in the age-matched patients with Parkinson's disease, and 1.56 +/- 0.17 and 1.59 +/- 0.18 in the age-matched patients with Parkinson's disease syndrome. A significant difference was noted between group of Parkinson's disease, normal controls and Parkinson's disease syndrome. CONCLUSION: The results suggest that 99mTc-TRODAT-1 dopamine transporters SPECT has clinical application value in early diagnosis or differential diagnosis of Parkinson's disease.

[Clinical and molecular-biological study of a May-Hegglin anomaly family].

OBJECTIVE: To study the changes of platelet in May-Hegglin anomaly (MHA) and the molecular pathogenesis mechanism. METHODS: Peripheral blood was drawn from the MHA proband, her father and her uncle. Platelet count and morphology were examined by automatic blood cell counter and microscopy, respectively. The platelet membrane protein was examined by flow cytometry. Membrane antibodies were determined by ELISA. PCR was used to amplify the exons 25, 31 approximately 32, 38 and 40 of the MYH 9 gene in the MHA patient and her diseased father. Furthermore, PCR products were sequenced, a specific point mutation was identified and inclusions (Dohle's body) in the neutrophil was detected by indirect immunofluorescence technique. RESULTS: It was proved that in MHA patients, platelet count was higher by cell counter than by microscope (P < 0.01). Giant platelet was 94% but platelet membrane proteins (CD41, CD61, CD42A, CD42b) were in normal range. Membrane antibodies was undetectable. An A5521G mutation (GAG-->AAG) in the exon 38 was found in the proband and her diseased father, resulting in a characteristic change of NMMHC-A1841 (Glutamic acid-->Arginine), which was not found in other members of the family and in normal controls. Spindle-like inclusions with fluorescence were clearly displayed in neutrophil cytoplasm. CONCLUSION: The molecular pathogenesis mechanism of May-Hegglin anomaly is the mutation in MYH 9 gene.

Studies on the interaction between Ag(+) and human serum albumin.

The interaction between Ag(+) and human serum albumin (HSA) has been intensively studied by means of equilibrium dialysis, ligand-to-metal charge transition (LMCT) bands, circular dichroism (CD) and Raman spectroscopy. Scatchard analysis of the results of equilibrium dialysis indicates the presence of two types of binding sites for Ag(+) on HSA, and the orders of magnitude of binding stability constants are found to be 10(5) and 10(4), respectively. During the binding process, a gradual increase in absorbance values of LMCT bands is observed with time-scanning UV absorption spectra, implying the Ag(I) centers are continually formed in HSA. The time-scanning CD spectra provide evidence that the binding of Ag(+) induces HSA to undergo a slow rearrangement of tertiary structure, and to change from the original conformation in the absence of Ag(+) (B-state) to conformation binding with Ag(+) (A-state). The rate constants and activation free energy of A-B transition are calculated. The Raman spectrum of Ag(I)-HSA system shows distinct vibration bands at 224 and 246 cm(-1) in the low-frequency region, which significantly reveal the formation of Ag-S and Ag-N bonds. In addition, the electrostatic interaction between Ag(+) and negatively charged oxygen is also detected with Raman spectroscopy.

Using oxidation-reduction potential (ORP) and pH value for process control of shortcut nitrification-denitrification.

A new low cost technology for simultaneous carbon-nitrogen removal from soybean wastewater has been developed in this study. The technology is performed through shortcut nitrification-denitrification. The process operated under realtime control of aeration and mixing time. The shortcut nitrification-denitrification in sequencing batch reactor (SBR) was achieved efficiently and steadily by controlling temperature (28 +/- 0.5 degrees C) and using real-time control strategies. This enabled the prevention of nitrite oxidation, leading to lower operational costs. The feasibility of oxidation-reduction potential (ORP) and pH value as control parameter for shortcut nitrification-denitrification process was also investigated. Results showed that the average removal efficiency of ammonium was more than 95%, and nitrosation rate (NO2(-)-N/NOx(-)-N) was reached to 96%. At the same time, the variation of oxidation--reduction potential (ORP) and pH value was well related to organic matter degradation and ammonium oxidation in SBR. So that judgment on the ending of nitrification and denitrification can be based on the inflection point on the varied curve of ORP and pH throughout each SBR processing cycle, and thus reducing aeration and mixing time for saving energy source. The method saves organic energy up to 40% of chemical oxygen demand (COD) in denitrification process, which should reduce the need for an extra external source of organic carbon. Shorter hydraulic retention time should allow the volume of the reactors to diminish, and thus diminish investment costs. Lower oxygen demand of about 25% gives lower exploitation costs.

A study on prediction of the bio-toxicity of substituted benzene based on artificial neural network.

Quantitative Structure-Activity Relationship (QSAR) between the bio-toxicity of seventy-eight kinds of substituted benzene chemicals to yeast Saccharomyces cerevisiae (1g(1/Cmiz)) and the components of vertex degree autocorrelation vectors (values of A, B, C and D) was studied by using the software of Artificial Neural Network (ANN). The key factors of the autocorrelation descriptors for the 1g (1/Cmiz) value of yeast Saccharomyces cerevisiae, A [0], A [1], C [3], C [5] and D [3] were selected from twenty-four descriptors, and were explained theoretically in this paper. The QSAR-ANN model has been used to predict the bio-toxicity of twenty-three substituted benzene chemicals. The correlation between Cmiz and LC50 was also discussed, and the liner correlation equation between them was established.

[Temperature effects on DO and ORP in the wastewater treatment].

At present, there are some problems in the study of DO and ORP as control parameters, because the researches are usually developed in the invariable temperature. However, environmental temperature is changing in the wastewater treatment. The effect of environmental temperature on DO and ORP as control parameter was studied specially in some experiments carried on a sequencing batch reactor (SBR) fed with soybean wastewater in this paper. Results show that the law of variation in DO during organism removal were influenced with the environmental temperature, at different experiment condition of aeration intensity, MLSS concentration or inflow nutrition concentration, but the variation in ORP were not influenced with it. The reason that the DO was influenced was also discussed. This paper brought forward that the control parameter using ORP was accurate and reliable when environmental temperature changed. In addition, inflow COD concentration and the time that ORP concave value appeared was connected, and the speed that ORP in curve ascended related with the COD concentration during the degradation. So the end point of degradation may be estimated by applying ORP derivative. United ORP concave value and its derivative, it can be made on-line control of aeration intensity and aeration time.
