[Clinical significance of antineutrophil cytoplasmic antibodies in patients with lupus nephritis].

OBJECTIVE: To analyze the clinical features of patients with lupus nephritis positive for antineutrophil cytoplasmic antibodies (ANCA) and explore the clinical implications of ANCA detection. METHODS: Totally 261 patients with lupus nephritis were enrolled in this study, including 53 ANCA-positive and 208 ANCA-negative ones. The clinical data of the patients pertaining to the disease history, physical examination, laboratory examinations and pathological inspection were retrospectively analyzed. RESULTS: Compared with patients negative for ANCA, the ANCA-positive patients had significantly higher incidence of serositis (75.5%), acute renal failure (64.2%), myocarditis (30.2%), neuropsychiatric involvement (26.4%) and lung hemorrhage (7.5%)(P<0.05). Significant differences were also found between the two groups in SLE disease active index (SLE-DAI), number of the diagnostic criteria, erythrocyte sedimentation rate (ESR), anemia, anti-Sm antibodies, and serum complement C(3). Most patients positive for ANCA (67.9%) had type IV lupus nephritis with more crescent formation, renal tubular atrophy, hyaline thrombi, and higher mortality rate as well than the negative patients. CONCLUSION: ANCA detection may benefit the estimation of the disease severity and prognostic evaluation of lupus nephritis.

Level of asymmetric dimethylarginine and carotid atherosclerosis in patients with chronic kidney disease.

OBJECTIVE: To determine the association between asymmetric dimethylarginine (ADMA), an endogenous inhibitor of nitric oxide (NO) synthase, with atherosclerosis in patients with chronic kidney disease (CKD). METHODS: One hundred thirty-eight CKD patients were enrolled in this study. Serum levels of L-arginine, ADMA, and SDMA were measured by high-performance liquid chromatography (HPLC). Common carotid arteries intimae-medial thickness (CCA-IMT), cross-sectional calculated intimae-medial thickness (cIM area) and atherosclerotic plaque were detected by noninvasive high-resolution B-mode ultrasonography. RESULTS: Serum levels of ADMA and SDMA were significantly increased in CKD patients (n=138) compared with age matched healthy subjects (n=42, P<0.01). ADMA and SDMA levels increased with the progression of renal dysfunction and were negatively related to creatinine clearance (Ccr) in pre-dialysis patients (r=-0.315, P<0.05; r=-0.426, P<0.01). Serum levels of ADMA and SDMA in dialysis patients (n=74) were significantly higher than those in pre-dialysis patients (P<0.05). Patients with carotid artery plaques showed significantly higher levels of ADMA compared with those without plaques. Serum levels of ADMA closely correlated with the mean IMT (r=0.471, P<0.01) and cIM area value (r=0.430, P<0.01). These correlations remained significant even after adjusting GFR, age, gender ,and other risk factors for atherosclerosis in the multiple regression analysis. CONCLUSION: Serum levels of ADMA increased with the progression of CKD and may play a role in the pathogenesis of accelerated atherosclerosis in CKD patients.

[Effects of hydraulic retention time (HRT) on nitrification performance and microbial community of conventional activated sludge (CAS)].

A conventional activated sludge system was used to treat the ammonium-bearing inorganic wastewater for 260 days under decreased HRTs. When treating 500 mg x L(-1) NH4+ -N wastewater, over 98% ammonia removal could be obtained at HRT > or =20 h. Sharp washout of biomass occurred when HRT was further decreased, resulting in the accumulation of ammonium and nitrite. Changes in the community structure were monitored by microbial quinone and conventional analytical methods. Quinone profiles indicated that ammonia-oxidizing bacteria (AOB) in beta-Protevbacteria and Nitrobacteria in alpha-Proteobacteria were the predominant species to oxidize ammonia and nitrite, respectively. The number of AOB decreased with the reduction of biomass in bioreactor, while that of nitrite-oxidizing bacteria (NOB) changed little and fluctuated at 10(5) CFU x L(-1)

[Comparison of esomeprazole- and omeprazole-based triple therapy regimens for duodenal ulcer with Helicobacter pylori infection].

OBJECTIVE: To investigate the effect of esomeprazole- and omeprazole-based triple therapy regimens in the treatment of duodenal ulcer with Helicobacter pylori (Hp) infection. METHODS: Totally 104 patients with duodenal ulcer and Hp infection were randomly divided into group A (52 cases) treated with esomeprazole (20 mg), amoxicillin (1000 mg) and clarithromycin (500 mg) twice daily and group B (52 cases) treated with omeprazole (20 mg), amoxicillin (1000 mg) and clarithromycin (500 mg) twice daily. The treatment lasted for 7 days, after which abdominal pain relief rate, ulcer healing rate and eradication rate of Hp as well as adverse effects of the medication were evaluated and compared. RESULTS: On the first and second days of medication, the abdominal pain relief rates were 34.6% and 59.6% respectively in group A, significantly higher than those in group B (17.3% and 38.5%, respectively, P<0.05). Ulcer healing rate and Hp eradication rate were 92.3% and 88.5 % in group A, and 88.5% and 82.7% in group B, showing no significant differences between the two groups (P>0.05). The adverse effects of the medication were rare in both groups without significant differences. CONCLUSION: Esomeprazole-based triple therapy may effectively eradicate Hp infection and promote duodenal ulcer healing with good tolerance, capable of achieving more speedy pain relief than omeprazole-based therapy.

[Synthesis and in vitro antitumor activity of multi-methoxyl carbazole analogues].

AIM: To design and synthesize new methoxyl carbazole analogues as antitumor compounds. METHODS: Methoxyl-nitrobiphenyls (3a-3c) were prepared through the Ullmann reaction of 4,5-dimethoxyl-2-bromonitrobenzene and methoxyl-iodobenzene compounds with the catalysis of copper powder, and then reduced by P(EtO)3 to obtain methoxyl carbazoles 4a-4c. The modification at 9-position of the methoxyl carbazoles (4a-4c) gives 16 carbazole derivatives (5a-5p). These compounds were confirmed by 1HNMR, MS, IR and elemental analysis. RESULT: In vitro antitumor activities evaluation in vitro demonstrated that IC50 value of the target compounds 4c, 5a, 5b, 5g, 5h, 5i, 5l, 5n and 5p against HT-29 cells were 12.1, 10.6, 8.1, 3.1, 4.4, 10.1 and 9.2 micromol x L(-1) respectively, and IC50 value of the target compound 4a against KB was 17.7 micromol x L(-1). CONCLUSION: Some of the target compounds have better inhibitory effects against H-29 and KB cells.

[Activity of nitrifiers and metabolized products in a membrane bioreactor MBR under the condition of non-sludge discharge].

A membrane bioreactor fed with synthetic ammonium bearing inorganic wastewater was operated under the following conditions: HRT, from 30 h to 7 h; influent NH4+ -N, 500 mg/L; non-discharge of sludge. Formation of extra-cellular polymers (ECP) and their influence on the activity of nitrifiers in the membrane bioreactor was investigated. Over the whole experimental period of 200 d, the activity of nitrifiers in terms of overall ammonium-oxidizing rate decreased from 2.1 kg/(L x d) to 1.5 kg/(L x d), and the nitrifier number using INTF absorbency measurement decreased from 0.023 x 10(-3) to 0.01 x 10(-3), although the MLSS increased from 4500 mg/L to 10500 mg/L. The ECP content increased from the initial 300 mg/L to 600 mg/L. Batch tests using the extracted ECP also indicated that the activity of nitrifiers could be inhibited by the existence of ECP.

Identification of ubiquinones and menaquinones in activated sludge by liquid chromatography-atmospheric pressure chemical ionization mass spectrometry.

A sensitive analytical method has been developed for identification of ubiquinones (UQ-n(Hx)) and menaquinones (MK-n(Hx)) in activated sludge by liquid chromatography-atmospheric pressure chemical ionization mass spectrometry in negative mode (LC-NI-APCI-MS). Extraction and clean-up of samples were carried out on Sep-Pak Plus Silica solid-phase extraction cartridges. Complete separation of quinones was achieved with an ODS analytical column and using isopropyl ether-methanol (17:83, v/v) as the mobile phase. The compositions of ubiquinones and menaquinones were determined directly using combined information on retention time, the molecular ion mass and fragment ion masses. The lowest instrument quantitative detection limits (LODinst) for UQ-6, UQ-10, and Vitamin K1 were estimated to be 0.4, 4 and 0.12 ng (S/N = 10) using LC-NI-APCI-MS in SIM mode, and the lowest method detection limits (LODmeth) achieved by spiking experiment were estimated to be 0.2, 2 and 0.06 microg/g for UQ-6, UQ-10 and Vitamin K1, respectively. On the other hand, the LODinst for UQ-6, UQ-10, and Vitamin K1 were estimated to be 10, 100 and 2 ng (S/N = 10) using LC-NI-APCI-MS in full-scan mode, and the LODmeth were estimated to be 7, 60 and 1.2 microg/g for UQ-6, UQ-10, and Vitamin K1, respectively. Both LC-NI-APCI-MS and LC-UV/DAD were applied in the analysis of an activated sludge extract. UQ-n (n = 6-10), MK-n (n = 6-10), MK-n(H2) (n = 7-10), MK-n(H4) (n = 8-9) and MK-8(H6) were detected by LC-NI-APCI-MS, while UQ-6, UQ-7, MK-7(H), MK-9 and MK-10(H2) were not found by LC-UV/DAD. These results suggest that LC-NI-APCI-MS is more sensitive than LC-UV/DAD for the analysis of quinones in environmental samples such as sediment, activated sludge and bio-film in biological processes and other aquatic environments.

[Studies on antitumor activity of rhEndostatin].

AIM: To investigate the inhibitory effect of recombinant human endostatin (rhEndostatin) on endothelia cell proliferation and tumor growth. METHODS: MTT assay was applied to examine the anti-proliferation of rhEndostatin on human embryo umbilical cord vascular endothelial cell ECV304 and human cancer cell HCT-8, BGC803 and EJ. Xenotrasplanted nude mice models with human cancer and experimental implanted tumor mice model were used to evaluate rhEndostatin's antitumor activity. RESULTS: rhEndostatin was shown to inhibit the proliferation of ECV304 cells and the IC50 is about 7 x 10(-6) g.L-1. No inhibition was observed in HCT-8, BGC803 and EJ cells at 1 x 10(-4) g.L-1 rhEndostatin. rhEndostatin was shown to inhibit human xenograft in nude mice with human gastric cancer BGC803 and breast cancer B37 when administered subcutaneously at 5, 10, 20 mg.kg-1.d-1 for 24 days in a dose-dependent manner. Mouse hepatoma H22 was also suppressed when given rhEndostatin subcutaneously 20 mg.kg-1.d-1 for 9 days, but it showed no inhibitory effect on Lewis lung carcinoma and B16 melanoma. CONCLUSION: These results indicate that rhEndostatin can inhibit the growth of xenotransplanted human tumors in nude mice and certain murine tumor. The action mechanisms may be that it can inhibit endothelial cell proliferation, thereby inhibiting the formation of new blood vessel in tumor, leading the tumor to stop grow.

[Effect of alpha-difluoromethylornithine on growth characteristics and expression of ALT-04ag gene of human lung carcinoma cells].

OBJECTIVE: To study the effect of polyamine biosynthesis inhibition on growth characteristics of human lung carcinoma cells and its correlation with the expression of human lung carcinoma associated antigen ALT-04ag gene. METHODS: The gene expression was detected by RT-PCR and immunocytochemical tests. The cell growth characteristics were studied by cell growth curves, morphological observation, FCM analysis and DNA electrophoresis. RESULTS: Human lung squamous carcinoma cells L78 treated with 5 mmol/L alpha-difluoromethylornithine (DFMO) for 5 days showed significant growth inhibition and apoptosis induction. The mRNA and protein expressions of ALT-04ag gene in the cells were downregulated, while these changes resulted from DFMO treatment were prevented by provision of DFMO along with exogenous putrescine. CONCLUSION: The effect of polyamine biosynthesis inhibition induced by DFMO restrains the growth characteristics and promotes apoptosis of human lung carcinoma L78 cells, which is associated with down regulation of ALT-04ag gene expression.
