The effect of hormonal contraception on the adequacy of colposcopic examination of the cervix.

OBJECTIVE: This study was undertaken to determine whether the use of hormonal contraception affects the adequacy of colposcopic examination of the cervix. STUDY DESIGN: A retrospective chart review of patients seen in the Vanderbilt University Medical Center Colposcopy Clinic from 2000-2002. Patient age, use of hormonal contraception, and adequacy of colposcopic examination were recorded for analysis. RESULTS: Of all the colposcopic records reviewed, 526 of 1156 met inclusion criteria. Study patients ranged in age from 14 to 51 years. The prevalence of the visible squamocolumnar junction ranged from 75% to 91%. The multivariate logistic regression analysis showed that both age and use of hormonal contraception significantly influence adequacy of colposcopic examination (P < .001, P = .001, respectively). CONCLUSION: The use of combination oral contraceptives is associated with an increased likelihood of adequate colposcopy.

A big first step.

The singleton, term gestation, live birth rate per cycle initiated (BESST) endpoint proposed at the beginning of 2004 is a first big step which should be added to by the consideration of multiple pregnancy rates in relation to singleton rates, by recording of fetal reductions and of pregnancies resulting from cryopreserved material. After three or more steps we may have an accurate reporting system which helps patients to distinguish the pros and cons for singleton term delivery.

Hom study course: winter 2004.

OBJECTIVE: The Home Study Course is intended for the practicing colposcopist or practitioner who is seeking to develop or enhance his or her colposcopic skills. The goal of the course is to present colposcopic cases that are unusual or instructive in terms of appearance, presentation, or management or that demonstrate new and important knowledge in the area of colposcopy or pathology. Participants may benefit from reading and studying the material or from testing their knowledge by answering the questions. ACCME ACCREDITATION: The American Society for Colposcopy and Cervical Pathology (ASCCP) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to sponsor continuing medical education for physicians. The ASCCP designates this continuing medical education activity for 1 credit hour Category I of the Physician's Recognition Award of the American Medical Association. Credit is available for those who choose to apply. The Home Study Course is planned and produced in accordance with the ACCME's Essential Areas and Elements.

Timing of initiation of the preovulatory luteinizing hormone surge and its relationship with the circadian cortisol rhythm in the human.

The relationship between the hypothalamo-pituitary-gonadal (HPG) axis and the hypothalamo-pituitary-adrenal (HPA) axis has been well documented in the rat. In most cases, a negative coupling was observed and an inhibitory effect of the HPA axis upon the HPG was shown. In the female rat, a marked circadian rhythm of corticosterone plasma values is observed during each day of the estrous cycle, with maximal values around 08:00 p.m. The preovulatory luteinizing hormone (LH) surge also occurs at 08:00 p.m. on the day of proestrus. Here we measured circadian variations of plasma cortisol in humans in relation with the time of initiation of the preovulatory LH surge. Blood samples were taken at 08:00 a.m., 12:00 a.m., 04:00 p.m., 08:00 p.m., 12:00 p.m., and 04:00 a.m. from 19 subjects for 4 consecutive days, once 17beta-estradiol (E(2)) values reached 125 pg/ml (days 7-10 of the menstrual cycle). Serum E(2) and LH determinations were performed by microparticle enzyme immunoassays. Serum progesterone and plasma cortisol determinations were made using RIA methods. For plasma cortisol values, a marked circadian rhythm, with 2- to 3-fold higher values during the morning than during the afternoon, was almost identical before, during and after the LH surge. However, values were generally higher during the follicular phase than during the luteal phase. Maximum cortisol values occurred between 04:00 and 08:00 a.m. and minimal cortisol values between 04:00 and 08:00 p.m. Initiation of the LH surge (50% over the mean of previous values) occurred at 04:00 a.m. (20% of the cases) or at 08:00 a.m. (80% of the cases). There was a strong coupling between the onset of the surge and the acrophase of the cortisol circadian rhythm: maximal cortisol plasma values were seen at 04:00 a.m. when the LH preovulatory surge started at 04:00 a.m. and 08:00 a.m. when it started at 08:00 a.m. The present results show that the positive coupling documented in the female rat between the HPA and the HPG axis at the time of preovulatory LH surge is also present during the menstrual cycle in the human.

Reflections on the usefulness of embryo cloning.

Questions are raised about the potential of embryo cloning, or blastomere separation, both to increase the rate of pregnancy following in vitro fertilization and to provide sufficient material for preimplantation genetic diagnosis. Nevertheless, it is concluded that research on blastomere separation in the human should continue.

IVF: past and future.

The introduction of IVF into the New World had its roots in Robert Edwards' six-week fellowship at Johns Hopkins in 1965, when he and I made a systematic attempt to fertilize human oocytes in vitro. While fertilization was not claimed in the publication of the work done in 1965, a retrospective examination of published photos indicate that human fertilization was obtained at that time. Edwards and Steptoe achieved a term birth with IVF in 1978, and this stimulated the establishment of an IVF clinic in Norfolk, Virginia. Using ovarian stimulation in 1981, the first delivery in the New World took place. This led to a series of studies on the influence of ovarian stimulation on the normal menstrual cycle and resulted in the finding that with ovarian stimulation there are three response patterns: high, normal and low. It was shown that an LH surge does not occur in ovarian stimulation. This latter observation led to the discovery of the LH surge inhibiting factor. The Norfolk programme became involved in the societal impact of IVF through an invitation to the Vatican, the Ethical Committee of the American Fertility Society (later the American Society for Reproductive Medicine), a lawsuit for libel against the local newspaper, and other activities.
