[Transforaminal lumbar interbody fusion in treatment of upper lumbar disc herniation: analysis of 18 cases].

OBJECTIVE: To study the effectiveness and advantages of transforaminal lumbar interbody fusion (TLIF) in the treatment of upper lumbar disc herniation. METHODS: Eighteen cases with upper lumbar disc herniation, 12 males and 6 females, aged 21 - 67, underwent TLIF. Follow-up was conducted for 19.8 months (12 - 54 months). The surgical process, outcomes, and complications were reviewed retrospectively. In TLIF operation, pedicle screw fixation was performed first, unilateral or bilateral facet joints were then excised. Disc removal and titanium mesh or fusion cage insertion were completed via transforaminal approach. RESULTS: Unilateral TLIF was performed in 14 cases and bilateral TLIF was accomplished in 4 cases. The mean operation time was 82.4 minutes and the intraoperative blood loss was 323 ml. No injury of spinal cord or nerve roots happened during the operation. The follow-up showed that the operation results were excellent in 11 cases, good in 4 cases, and fair in 3 cases. No worsening or relapse of preoperative symptoms was observed. Disc spaces and lumbar lordosis were restored satisfactorily. No internal fixation failure was found. CONCLUSION: Able to excise disc completely, restore physiological lumbar curve effectively and prevent postoperative instability, TLIF is one of effective surgical treatments for upper lumbar disc herniation for selected patients.

[Experience in reoperation of lumbar spinal surgery using posterior lumbar interbody fusion cage].

OBJECTIVE: To investigate the indications and surgical techniques of reoperative lumbar spinal surgery using posterior lumbar interbody fusion cage. METHODS: Seventeen cases underwent reoperative lumbar spinal surgery using posterior lumbar interbody fusion cage. The surgical methods included decompression of lumbar canal, dural and radicular conglutination release, discectomy, posterior insertion of lumbar interbody fusion cage, pedicle screw instrumentation, posterolateral lumbar fusion. Periodical follow-up was conducted for 18 months (12 - 43 months). The clinical effect was evaluated by the Macnab's criterion, and the outcome of lumbar interbody fusion was evaluated according to the Brantigan's criterion. RESULTS: Twenty-seven cages were implanted into twenty-three disc spaces (cage at L(3 - 4) in 5 cases, cage at L(4 - 5) in 8 cases, and cage at L(5)-S(1) in 4 cases). All postoperative patients ambulated after two weeks. No wound healing problems and nerve injury were found. Dural rupture occurred in 5 cases, but without occurrence of cerebrospinal fluid leakage. According to the Macnab's criterion, the preoperative lower extremity radicular symptoms and neurogenic claudication were effectively relieved in 15 cases (88.2%), and the outcomes were good or excellent in 15 cases (88.2%). According to the Brantigan's criterion no evidence of implant failure was found, and interbody fusion could be achieved about 6 months in all patients. No additional operation was needed for involved segments, and the surgical outcomes could be maintained. CONCLUSION: A good and effective method, use of posterior lumbar interbody fusion cage in reoperative lumbar spinal surgery eliminates neural compression and the discogenic pain, restore the lumbar alignment, and accomplish the internal fixation and anterior column fusion of lumbar vertebrae simultaneously.

[Comparative study on recruitment maneuvers in acute respiratory distress syndrome with pulmonary and extrapulmonary origin].

OBJECTIVE: To study the effects of lung recruitment maneuvers (RM) in acute respiratory distress syndrome (ARDS) with pulmonary and extrapulmonary origin (ARDSp/ARDSexp). METHODS: Thirty-seven ARDS patients with a forced expiratory volume were selected (16 with ARDSp, 21 with ARDSexp). Without using any sedative and neuromuscular blocking agents, with continuous positive airway pressure (CPAP) up to 30 cm H(2)O (1 cm H(2)O=0.098 kPa), the patients were asked to hold the breath for 30 seconds, and then CPAP was lowered to the level of that before the treatment within 5-10 seconds. RESULTS: Compared with the state before recruitment, changes in heart rate (HR) were not obvious in two groups during RM, mean arterial pressure was elevated evidently after 2 minutes in two groups (both P<0.05), and central venous pressure (CVP) rose after 30 seconds in two groups (both P<0.05). Peak inspiratory pressure, platform pressure and mean airway pressure increased clearly after 30 seconds (all P<0.05), and pulmonary compliance decreased obviously 30 seconds after re-inflation recruitment in two groups (both P<0.05), and increased obviously in ARDSexp group 5, 15, 30 minutes after recruitment (all P<0.05). In the group with ARDSp oxygenation index (OI) changed distinctly 10, 30 minutes and 1 hour after recruitment compared with the state before recruitment (all P<0.05). In patients with ARDSexp OI was significantly improved 10 minutes, 30 minutes, 1 hour, 2 hours after RM. CONCLUSION: In ARDSexp, with pulmonary interstitial edema as the main pathology, responded better to RM than ARDSp with pulmonary consolidation.

[The community and structure of nitrogen-fixing microorganism in Sanjiangyuan natural reserve].

Research on the diversity of microorganism community in natural environment has been concerned hot spot using the newly molecular biotechnology in the world now. To understand the composition and structure of nitrogen-fixing bacteria communities in the Qingzang plateau, the molecular diversity and phylogenetic of nifH genes of Sangjiangyuan natural reserve were examined by using the PCR-RFLP based cloning approach. The 3 samples were come from different sites and different plant types, and their biogeochemical parameters were diverse. DNA was directly extracted from the soil microorganism and amplified the nifH gene fragment using PCR by the primers of nifH-34F 5'-AAAGG(C/T)GG(A/T) ATCGG(C/T)AA(A/G) TCCACCAC-3' and nifH-491R 5'-TFGTT(G/C)GC(G/C)GC(A/G)TACAT(G/C)GCCATCAT-3'. For the nifH gene segment, diverse PCR products were characterized by cloning, restriction fragment length polymorphism (RFLP) analysis and sequencing. A total of 233 clones and 99 operational taxonomic units (OTUs) which were digested the clones by the restriction enzymes MspI and RsaI were obtained from all samples. YS-1 had 63 clones and 24 OTUs, ZD-1 had 75 clones and 28 OTUs, and NQ-1 had 95 clones and 47 OTUs, respectively. They were found 1-2 significant domain groups of clones and shared 4 OTUs in all samples. A wide range of sequence divergence was observed in the 26 nifH clones that were sequenced from all samples. Sequence comparison showed that the nifH clones were 66% to 98% similar. The phylogenetic tree was constructed by the Clustal W and Mega software. 26 sequences could be subdivided into 4 clusters in the phylogenetic tree, and some of them had the closely similar to Proteobacteria, but The majority of the clones were not closely related to any known cultivated nitrogen-fixing bacteria, Therefore, most of them are unique and may represent novel sequences of nitrogen-fixing bacteria.

[Molecular diversity and phylogenetic analysis of nitrogen-fixing (nifH) genes in alp prairie soil of Sanjiangyuan natural reserve].

Research on the diversity of microorganism community in natural environment has been concerned hot spot using the newly molecular biotechnology in the world now. This was the first description of the molecular diversity and phylogenetic analysis of nitrogen-fixing (nifH) genes in alp prairie soil of Sanjiangyuan natural reserve. DNA was directly extracted from the soil microorganism and amplified the nifH gene fragment using PCR by the primers of nifH-34F 5'-AAAGG(C/T)GG(A/T) ATCGG(C/T)AA(A/G) TCCACCAC-3' and nifH-491R 5'-TYGTT(G/C)GC(G/C)GC(A/G)TACAT(G/C)G CCATCAT-3'. For the gene fragment, diverse PCR products were characterized by cloning, restriction fragment length polymorphism (RFLP) analysis and sequencing. 143 clones and 35 different RFLP patterns were received in two samples by the restriction enzymes MspI and RsaI digested. ZD sample had 82 clones and 21 different RFLP patterns, and YS sample had 61 clones and 19 different RFLP patterns. There were shared 5 RFLP patterns in two samples. The analysis result found a significant dominant group of clones occurring in both samples which account for 29.3% and 32.8%, respectively, and several minor groups were also detected. 21 clones were sequenced, and their levels of nucleotide identity were from 71% to 98%. None of the sequenced nifH gene was completely identical to any deposited in the data banks, and therefore each of them belong to a noncharacterized bacterium. Finally, the phylogenetic tree was constructed by the Clustal W and Mega software. 21 sequences can be subdivided into 4 clusters in the phylogenetic tree, and most of them had the closely similar toalpha- , beta-, and gamma-Proteobacteria . The significant dominant group in YS sample and ZD sample had the closely related with Rhodobacter sphaeroides and Delftia tsuruhatensis, respectively. The YS-nifH-11 was the only sequence which had highly similar to Cyanobacteria .

[Nitric oxide and nitric oxide synthase in patients with chronic hepatitis B].

OBJECTIVE: To investigate nitric oxide (NO) and nitric oxide synthase (NOS) in patients with chronic hepatitis B. METHODS: Nitric oxide and nitric oxide synthase, including inducible NOS (iNOS) and constitutive NOS (cNOS), were measured in patients and control groups, then were statistically analyzed. RESULTS: NO and iNOS were significantly higher in patients with hepatitis B than in the controls (P < 0.05). NO and iNOS were significantly higher in patients with increased ALT than in the controls and in patients with normal ALT (P < 0.05). NO was significantly higher in patients with normal ALT than in the controls (P < 0.05). cNOS were not significant different among these groups. NO and iNOS significantly correlated with ALT in patients with hepatitis B (r=0.367 and r=0.474). No significant relationship was found among NO, NOS and HBV DNA. Among different genotype groups, NO and NOS had no significant difference. CONCLUSION: NO and NOS were higher in patents with chronic hepatitis B. In patients with increased ALT, NO's damage was severe. In patients with normal ALT, there was no significant damage caused by NO. NO should be detected in patients with hepatitis B in addition to HBV markers.

[Gene mutation in the areas of PreC/C and BCP of HBV DNA].

OBJECTIVE: To investigate the gene mutation in the areas of pre core/core (Pre C/C) and basic core promotor (BCP) of HBV DNA and its clinical significance. METHODS: The nt 1 735-1 965 segment of HBV DNA was amplified with PCR in 54 cases with chronic hepatitis B and 10 cases with post-hepatitis cirrhosis. Then the PCR product was sequenced. RESULTS: There were 168 site mutations in 48.5% (33/68) cases with hepatitis B. The first ten mutation sites were nt 1 764 (58.8%), 1 762 (48.5%), 1 799 (21.0%), 1 766 (14.7%), 1 896 (13.2%), 1 754 (8.8%), 1 899 (8.8%), 1 768 (7.4%), 1 814 (7.4%) and 1 913 (7.4%). Three rare mutations of nt 1907, 1 922 and 1 923 were also detected. The mutations of nt 1 896, 1 764 and 1 762 were found in 16.7%, 35.2% and 35.2% of chronic hepatitis, and in 30.0%, 60.0% and 60.0% respectively of post-hepatitis cirrhosis cases. There was statistical significance between the two groups (P < 0.01). CONCLUSION: The mutations in the areas of Pre C/C and BCP of HBV DNA might possibly be associated with liver fibrosis. There are many mutation sites in HBV DNA and mutation occurs frequently, therefore gene sequencing is helpful to the design of gene chip and to clinical application.

[The effects of glycyrrhizin-18alpha on the expression of NF-kappaB in the renal interstitium of rats with obstructive nephropathy].

AIM: To investigate the effects of glycyrrhizin-18alpha on the expression of nuclear factor-kappaB in the renal interstitium of rats with obstructive nephropathy. METHODS: Obstructive nephropathy model of rats was set up by unilateral ureteral ligation. The rats were sacrificed 7,14, 28 and 56 days after operation. And then specimens were taken from the renal cortex. The morphological changes in renal interstitium were observed under light and electron microscopes. The expression of nuclear factor-kappaB and type III collagen was detected by immunohistochemical staining and analyzed by EIG image analysis system. RESULTS: Progressive fibrosis was observed in the renal interstitium of rats with obstructive nephropathy. The semi-quantitative analysis indicated that the expression level of NF-kappaB and type III collagen in the renal interstitium of glycyrrhizin-18alpha treatment and prophylaxis groups was markedly higher than that of mock operation group (P<0.01) and remarkably lower than that of normal saline control group(P<0.01) 7 days after operation. No significant difference in the expression of NF-kappaB and type III collagen was detected between glycyrrhizin-18alpha treatment group and prophylaxis group(P>0.05). CONCLUSION: Glycyrrhizin-18alpha may prevent renal interstitial fibrosis in obstructive nephropathy by down-regulating the expression of NF-kappaB in renal interstitium.

[Surgical treatment of degenerative scoliosis].

OBJECTIVE: To investigate the techniques and indications of treatment of degenerative scoliosis. METHODS: The clinical data of 45 degenerative scoliosis patients, 18 males and 27 females, aged 41 - 77, all suffering from severe low back pain, were analyzed. Twenty of them underwent posterior lumbar interbody fusion with pedicle screws combined with correction of scoliosis. Sixteen patients underwent depression of vertebral canal and pure internal fixation and fusion of pedicle. Eight patients underwent pure depression, such as laminectomy or incision of intervertebral foramen. The corrective effects of scoliosis, condition of intervertebral fusion, and complications were observed. RESULTS: All operations were performed successfully and all the patients made a remarkable recovery. During the 13 months' follow-up, 88% of the patients showed remarkable relief from pain and claudication and improved life quality. No loss in correction angle and height of disc space was found among the patients who had undergone internal fixation and interbody fusion. The residual symptom of low back pain was more manifest in the patients who had not undergone internal fixation than in those who had. CONCLUSION: Posterior lumbar interbody fusion with pedicle screws is safe and effective in the treatment of severe degenerative scoliosis.

[The study of peri-operative chemotherapy in stage I-IIIa NSCLC].

OBJECTIVE: A number of studies had evaluated the benefit of neoadjuvant chemotherapy combined surgery on stage IIIa-IIIb NSCLC, survival benefit was found in several papers. We attempt to evaluate the survival and prognosis of cisplatinum-based schedule as peri-operative CT for resectable stage I-IIIa NSCLC. METHODS: A prospective, randomized, multicenter study was conducted by Shanghai Lung Cancer Team (supported by Shanghai Branch of Discipline Foundation) since 1995-1997 for 211 cases of stage I-IIIa NSCLC with curative resection (99 stage I, 47 stage II, 65 stage III), age of or= 80, staged by 1997 AJC TNM Criteria. They were randomized to be 103 cases with 1 - 2 cycles of pre-operative CT and 108 cases with no pre-operative CT, 2 - 4 cycles of post-operative CT were used for stage II and stage IIIa NSCLC, it was totally 4 cycles of MVP or MOP CT schedule each case. Follow-up team had been trained, the follow-up rate should be >or= 95%, last follow-up date was March of 2002. Lobectomy was performed for most patients. Accumulated survival, log rank, MST, Cox uni-variance and multi-variance analyses were used as statistics for evaluation. RESULTS: The two arms were well balanced for baseline demographic and clinical characteristics (P > 0.05 for all). Stage I NSCLC had the best year-survival in whole patients. No statistical survival difference was found between the group with pre-op CT and with no pre-op CT, P = 0.074, 0.087 and 0.097, respectively, 5-year survival rates were of 31.98%:36.68%. In various stage, a statistical survival difference was only shown in stage IINSCLC, P = 0.042, 5-year survival rates and MST were worse in the group with pre-operation CT, 20%:65.2% and 24 months:48 months, respectively, but no difference was seen in stage I and stage IIIa NSCLC. Stage and post-operation CT were the only two meaningful parameters with statistical survival difference calculated by multi-variance analyses, P = 0.000 all, but no difference was found in others 4 parameters (age, sex, type and pre-operation CT). The response rate of pre-operation CT was of 50%. Though there was no statistical difference, the responders were with slightly better year-survival rates than MR + NR patients, 38.9% and 33.3%, respectively. In the cases with pathological "T" down stage and "T" unchanged after pre-operation CT had a better yr-survival rates than "T" up-stage, P = 0.03, 5-year survival rates were of 41.67%, 40.51% and 11.76%, respectively, thus, effective chemotherapy might be beneficial to survival. Besides, in the cases with >or= 3 cycles of post-operation CT have better survival rates than less cycles. CONCLUSION: A prospective, randomized, multicenter peri-operation CT study for stage I-IIIa NSCLC conducted in Shanghai, China., it showed there had no benefit in survival between with pre-operation CT arm and with no pre-operation CT arm. In stage II NSCLC, pre-operation CT cases had a worse year-survival than with no pre-operation CT, P = 0.042, but no difference was seen in stage I and stage IIIa NSCLC. The responder of CT and "T" down stage, "T" unchanged had better survival rates than those of not response and "T" up-stage. From multivariate analyses, stage and post-operation CT were the two meaningful parameters to year-survival, >or= 3 - 4 cycles of post-operation CT had a better statistical higher year-survival than less cycles. Nutrition, supportive treatment, immunity status and prevention of toxicity might be the next study worthy to conduct, for CT combined with OP.

[Medial wedged proximal tibial osteotomy for treating osteoarthritis of knee].

OBJECTIVE: To assess the efficacy of medial wedged proximal tibial osteotomy for treating knee osteoarthritis with varus deformity. METHODS: From July 1996 to September 1999, 19 patients with knee osteoarthritis accompanied by varus deformity were treated by medial wedged proximal tibial osteotomy combined with internal fixation. Full-length anterior-posterior radiographs were taken preoperatively, 8 weeks and 2 years postoperatively. The parameters including the femorotibial angle, the tibial angle, the femoral angle, the femoral condyle-tibial plateau angle, and the medial joint space, were measured from these radiographs. The function of knee was evaluated according to the 100-point rating scale standard of knee. RESULTS: The mean postoperative score had been significantly improved from 48.6 +/- 16.6 points to 81.7 +/- 14.8 points after 2 years of operation. The medial joint space had been increased from 2.2 +/- 1.6 mm to 4.9 +/- 1.5 mm and the femoral condyle-tibial angle had been decreased from 7.4 degrees +/- 3.1 degrees to 1.7 degrees +/- 3.1 degrees. There were complications in 3 cases: 2 cases of superficial wound infections and 1 case of intra-articular fracture. There were no delayed union and recurrence of varus deformity. CONCLUSION: Medial wedged proximal tibial osteotomy combined with internal fixation provides an efficacious approach to treat knee osteoarthritis with varus deformity.

[Application of intervertebral carbon fiber cage in degenerated lumbar disorders].

OBJECTIVES: To evaluate the effect of intervertebral carbon fiber cages in the treatment of degenerated lumbar disorders. METHODS: One hundred and twelve patients of degenerated lumbar disease were analyzed retrospectively with regard to the effect of posterior lumbar interbody fusion (PLIF) using intervertebral carbon fiber cage. RESULTS: In the 112 patients, disc height was 7 mm (3 to 11 mm) on average before operation and 11.2 mm (8 - 14 mm) after operation. Cobb angle was 32 degrees (21 degrees to 42 degrees ) on average before operation and 18.9 degrees (13 degrees to 21 degrees ) after operation. After follow-up for 7.8 months (4 to 13 months) on average, all operative disc spaces showed solid arthrodesis through X-ray images. Disc height was 10.9 mm (8 - 14 mm) on average, and Cobb angle was 18.5 degrees (13 degrees to 21 degrees ) on average. During the follow-up, endplate absorbing, pseudo-articulation, cage displacement, subsidence, and breaking were not found. CONCLUSION: Implantation of intervertebral carbon fiber cages is an effective method for the treatment of lumbar degenerative disease. Accurate indications and skilled techniques are required to avoid complication.

[Molecular mechanisms of tumorgenesis and metastasis in nasopharyngeal carcinoma cell sublines].

BACKGROUND & OBJECTIVE: The authors have selected three different biological character sublines, including 5-8F (the highest tumorigenic and metastatic ability), 6-10B(the lowest tumorigenicity and lack of metastatic ability), and 13-9B(lack of tumorigenic ability), from colony lines of nasopharyngeal carcinoma cell line(SUNE-1). The aim of this study was to determine the molecular mechanisms of different biological character of SUNE-1 and its three sublines from two aspects of genetic and virology. METHOD: 1. Amplifying Epstein-Barr virus(EBV) BamHI W fragment by PCR. 2. Determining the expression of latent membrane protein1(LMP1) of EBV in the sublines using in situ hybrization. 3. Molecular cytogenetic analysis of the cell line and sublines by comparative genomic hybridization (CGH). RESULTS: 1. There was EBV BamHI W fragment in SUNE-1 and its three sublines. 2. Expression of LMP1 was observed in the SUNE-1 and its three sublines, and their expression intensity was identical. 3. There were DNA copy number gains on chromosomes 1, 2p, 3, 4, 5p, 6, 7, 9, 10q, 11, 12q, 13q, and 18q as well as loss on 22q in the SUNE-1 cell line; DNA copy number gains on chromosomes 3p, 7q, 8q, 9q, and 10q were observed in 5-8F subline; DNA copy number loss on few chromosomes, but no DNA copy number gain was observed in the 6-10B and 13-9B sublines. CONCLUSION: There were expression of EBV and different genetic changes in the SUNE-1 and its three sublines. It is suggesting that the different biological character tumorgenesis and metastasis of nasopharyngeal carcinoma cell sublines may be due to genetic changes; and EBV infection may play an important role in keeping the malignant phenotypic of nasopharyngeal carcinoma.
