[Diagnosis of vascular compression of the trigeminal and glossopharyngeal nerve roots].

Sixty-four patients with trigeminal and 12 with glossopharyngeal neuralgia were examined. Use was made of trigeminal and glossopharyngeal somatosensory evoked potentials (TSEP and GSEP). These results were compared to the finding during surgery in exploration of the cerebellopontile angle. Recording of TSEP and GSEP was found to be of high diagnostic value for the establishment of vascular compression of the nervous radicle in the cerebellopontile angle to determine indications for microvascular decompression.

Extraintracranial anastomosis performed by means of biological gluing materials: experimental and clinical study.

A new surgical technique of extraintracranial anastomosis (EIA) is reported. End-to-side microvascular anastomosis is performed by applying four crossed-fixing sutures and by mantling a hemostatic sponge muff (eg, Spongostan) impregnated with fibrinogen-thrombin glue. Histopathological studies of microvascular anastomosis in animals and clinical results of 70 EIA operations showed that this surgical technique of anastomosis has certain advantages, namely that it reduces the number of sutures needed, is less traumatic to the vascular wall, ensures elastic vascular junction, and the glue and Spongostan used possess high biological inertness that provides a better physiological connection of vessels than conventional suturing. The highly hermetic vascular junction and absence of blood flow at the anastomosis site are achieved without additional suturing. The duration of the operation is significantly reduced. A full EIA function sets in comparatively early.

[The diagnosis of the nature of the lesion of the trigeminal nerve root in trigeminal neuralgia].

The authors examined 64 patients with unilateral trigeminal neuralgia by computed tomography and registration of trigeminal somatosensory evoked potentials. The values of the evoked potentials on the involved side differed essentially from those on the healthy side. This allows vascular compression of the trigeminal nerve root in the posterior cranial fossa to be identified authentically and the indications for surgical treatment of trigeminal neuralgia by microvascular decompression thus determined. The diagnosis of vascular compression of the root of the trigeminal nerve was established in 33 patients, who were then operated on. The diagnosis was confirmed during the operation: pain was relieved completely in all patients after surgical treatment.

[Combined method of creating extra-intracranial microvascular anastomoses using biological adhesives (experimental-clinical study)].

The article deals with the experimental results and the technique of the establishment of end to side microvascular anastomosis with the application of 4 crossed sutures and the use of fibrinogen-thrombin glue and hemostatic sponge. Analysis of the clinical results of the establishment of an extra-intracranial microvascular anastomosis by the suggested method in 54 patients showed its good function in a relatively early post-operative period, comparative simplicity and accessibility of performance, and a gain in the duration of the operation. The operation may be used in the surgical treatment of patients with ischemic lesions of the brain as well as a "prophylactic manipulation" immediately before direct intervention on the cerebral vessels for giant aneurysm, carotido-cavernous anastomosis, etc.

[Microvascular decompression in facial hemispasm].

Microvascular decompression in facial hemispasm is an operation rather infrequently performed in the CIS, while patients with facial hemispasms are great in number. The author obtained satisfactory results of the surgical intervention. Of special interest is the site of compression of the nervous root where the surgical intervention should be made.

[Microvascular compression of the accessory nerve in torticollis spastica].

The paper deals with the pathogenesis of torticollis spastica and potential modes of their treatment. The findings indicate that changes in the root of nerve XI are essential in the pathogenesis of this abnormality. Despite comparatively small materials, much higher efficiency of micro-decompression of the accessory nerve is shown.
