[Intravascular photohemotherapy in the complex prophylaxis of purulent-septic complications after coronary shunting].

The article presents results of treatment of 78 patients with the history of coronary bypass operations for ischemic heart disease. There were 48 patients with high risk factors of the development of pyo-septic complications. Among them there were 27 patients treated by complex postoperative therapy including intravascular photomodification of blood. The intravascular irradiation of blood was shown to have a positive influence upon the development of the early postoperative period in this category of patients. The immunomodulating effect was noted due to a greater amount of activated lymphocytes and T-helpers in blood. The positive dynamics of immunological indices were in correspondence with the data of the clinical state, pyo-inflammatory complications were not registered in this group of patients. Early (on the 3rd-5th days) inclusion of the intravascular irradiation of blood in the complex postoperative therapy is thought to be expedient for the patients with high risk of pyo-inflammatory complications.

Analysis of ethanol-glucose mixtures by two microbial sensors: application of chemometrics and artificial neural networks for data processing.

Although biosensors based on whole microbial cells have many advantages in terms of convenience, cost and durability, a major limitation of these sensors is often their inability to distinguish between different substrates of interest. This paper demonstrates that it is possible to use sensors entirely based upon whole microbial cells to selectively measure ethanol and glucose in mixtures. Amperometric sensors were constructed using immobilized cells of either Gluconobacter oxydans or Pichia methanolica. The bacterial cells of G. oxydans were sensitive to both substrates, while the yeast cells of P. methanolica oxidized only ethanol. Using chemometric principles of polynomial approximation, data from both of these sensors were processed to provide accurate estimates of glucose and ethanol over a concentration range of 1.0-8.0 mM (coefficients of determination, R(2)=0.99 for ethanol and 0.98 for glucose). When data were processed using an artificial neural network, glucose and ethanol were accurately estimated over a range of 1.0-10.0 mM (R(2)=0.99 for both substrates). The described methodology extends the sphere of utility for microbial sensors.

[Renal lesions in elderly patients with diabetes mellitus].

AIM: To elucidate the role of some renal diseases, pyelonephritis in particular, in elderly patients with type 2 diabetes mellitus (DM). MATERIALS AND METHODS: The trial included 150 patients with diabetes mellitus type 2 aged over 60 years with DM duration 16.6 +/- 7.8 years. All the patients underwent general clinical examination, Reberg-Tareev test, urine seeding, renal ultrasonography. RESULTS: The diagnoses associated with renal pathology were made in 75% of patients, the specific renal parenchymal damage (diabetic nephropathy-DN) proper was much less common (73 patients with type 2 DM). 16 of them were found to have chronic renal failure. The concurrent abnormalities included pyelonephritis (21%), urolithiasis (14%), renal cysts (19%), their combination (43%). Concomitant pyelonephritis had the most significant impact on renal function. Essential hypertension occurred in 65% of patients. CONCLUSION: In old age, renal damage specific for DM develops in the presence of pyelonephritis, cystic degeneration of the parenchyma, involutional processes, atherosclerotic and hypertensive arteriolosclerosis.

[The surgical treatment of postinfarct aneurysms of the left ventricle].

Operations for ischemic heart disease complicated by the appearance of postinfarction left ventricular aneurysm were fulfilled on 94 patients. The modified operation after V. Dor used in 55 patients allowed fulfilling interventions on the damaged parts of the interventricular septum excluding the application of foreign patches. Intrahospital lethality was 7.6%.

[A postinfarct heart aneurysm: its removal or revascularization?].

A conclusion about the hemodynamic significance of aneurysm having the area of more than 30% of the left ventricle surface is made on the basis of an analysis of the pre- and post-operative parameters of the myocardium contractility and hemodynamic indices in 49 patients with postinfarction aneurysm of the left ventricle (main group) and 26 patients with ischemic heart disease without this complication (control group). Formation of the syndrome of chronic insufficiency of blood circulation in hemodynamically insignificant aneurysm results from ischemia of the myocardium, rather than the aneurysm as such. Improvement of blood supply to the myocardium as a result of its revascularization in such patients led to normalization of the left ventricle geometry.

[Sinus node function, sinoatrial and atrial conduction and the nature of the course of supraventricular heart rate disorders in IHD patients before and after aortocoronary bypass].

Selective coronarography, assessment of hemodynamics, sinus node function, sinoatrial and intraatrial conduction were performed in 53 coronary heart disease (CHD) patients with supraventricular arrhythmia. The severity of cardiac rhythm paroxysms was evaluated at transesophageal pacing by measurements of arrhythmia induction frequency threshold. 29 patients underwent aortocoronary shunting. No relationships were found between the pattern of arrhythmia, degree of stenosis, number of coronary lesions. Aortocoronary bypass changed neither pattern nor severity of supraventricular arrhythmia.

[The age-related aspects of gouty nephropathy].

The incidence of podagric nephropathy in different age groups was studied on a large populational material (41,452 persons over 30 years). The number of patients suffering from podagra was shown to increase with each decade of life. In 122 patients with podagric nephropathy, deviations in urinalyses were recorded in 80 cases (65.5%). Of these, 53 patients (72.6%) were ot 60 years of age and older. In patients over 60, derangement of nitrogen-excretory renal function was seen more frequently (in 64.4%) than in other age groups. The greater incidence of podagra and podagric nephropathy over the recent decade may be related not to the changes in the nutrition pattern and hypodynamia but also, and possibly to a larger measure, with deterioration of the environmental conditions and alterations in radiation background.

[Chronic renal failure in middle-aged and elderly persons].

The authors analyzed the frequency of chronic renal insufficiency (CRI) and the level of serum creatinine (200 mumol/l and more) in 10,350 outpatients aged over 60 (I group), 2400 persons of the same age in need of episodic hospitalization (II group), and in 1840 patients who died in hospital (III group). In the 1st group CRI frequency was 0.98%, in the 2nd group--6.9%, in the 3rd group--14.6%. Pyelonephritis (76.1%) and myelonephropathy (7.5%) were among CRI main causes. The authors underlined the multitude of factors causing CRI in middle-aged patients and the prospects of adequate therapy even in terminal states; CRI syndromes were considered for persons of this age group.
