[Ozena and allergic rhinitis in ichthyosis vulgaris].

The clinical course of an ozaena in a patient with an autosomal dominant ichthyosis vulgaris was complicated by the skin disease related atopic disposition with allergic rhinitis. Electron microscopic studies of the pathologically keratinized mucosa of the nasopharynx revealed a similar defect of the mucosal keratohyalin caused by the absence of the protein filaggrin.

[Adenoid cystic carcinoma of the lacrimal gland].

A 38-year-old female presented with a left-sided peripheral N. VI-paresis and negative neuroradiologic work-up. After one year she developed an incomplete N. III paresis, numbness of her face correlating to N. V1 and V2, reduced lacrimal secretion as well as a palpable mass at the lateral orbital rim. Magnetic resonance imaging revealed an orbital tumor extending into the cavernous sinus. Biopsy disclosed an adenoid cystic carcinoma of the lacrimal gland with basaloid, cribriform and tubular pattern. Therefore, an exenteration with postoperative radiation therapy was performed. Immunohistochemical findings included positive reactions for keratin (KL-1 and AE 1 + 3), Vimentin and Egp 34. By determination of Ki 67 the proliferative activity of the tumor was found to be 15%. Our case report demonstrates that even in absence of an exophthalmus or pain a malignant orbital tumor may be assumed as the underlying cause for a combined monolateral oculomotor and trigeminal paresis. Immunohistochemical results support the notion, that the adenoid cystic carcinoma of the lacrimal gland may arise from precursor cells of the terminal duct system.

[Nodular panniculitis: a manifestation of Lyme borreliosis?].

Infection with Borrelia burgdorferi can induce various skin manifestations. The type of skin manifestation and the histopathological picture depend on the stage of infection and vary from local inflammatory infiltrates to chronic atrophic skin disease. Involvement of subcutaneous tissue has been observed only very rarely. We report on two patients suffering from nodular panniculitis (Pfeifer-Weber-Christian) and present evidence that the disease was caused by Borrelia burgdorferi. In one of the cases Borrelia burgdorferi was repeatedly isolated from skin and subcutaneous tissue biopsies in spite of repeated high-dose therapy with ceftriaxone, Doxycycline and cefotaxime.

[Septic granulomatosis (chronic granulomatous disease = CGD) in the jaw area. A case report].

A rare case of chronic granulomatous disease with manifestation in the mandible is presented. A 10-year-old boy showed the clinical picture of osteomyelitis without paresthesia (Vincent syndrome). Diagnosis was done by the nitroblue tetrazolium test (NBT) and by demonstration of a complete cytochrome-b deficiency of the granulocytes. In septic granulomatosis a continuous application of Sulfamethoxazol-Trimetoprim is recommended for prophylaxis of infection. For treatment of infection further antibiotics who pass into the granulocytes like rifampicin and fosfomycin are necessary. Oral surgical procedures request a careful tissue handling and a tight wound closure.

[Effects of acute nicotine administration on the function of the human parotid gland].

The effect of acute administration of nicotine on the secretory function of the human parotid gland was investigated in 4 male volunteers. After intravenous infusion of 20 micrograms nicotine/kg b.w. within 10 min in all subjects an increased salivary amylase activity and protein concentration was observed. This phenomenon is believed to be caused by a stimulation of parotid beta-adrenoceptors secondary to a nicotine-induced release of catecholamines from the adrenals.

[Sialadenosis of the parotid gland after chronic nicotine use].

The effect of chronic nicotine consumption on the morphology of the rat parotid gland was investigated. After nicotine-loading for 90 days with an average serum nicotine concentration of 78 +/- 10 ng/ml a significant increase of acinar cell volume was observed. The acinar cells contained an increased number of enlarged light, immature secretory granules. These findings were confirmed by morphometric analysis. Further an increase of the granular endoplasmatic reticulum, an enlargement of the Golgi complexes and an oedematous swelling of intraglandular autonomous nerves were found. Similar findings have been observed in the parotid gland of animals and also of humans chronically treated with the beta-adrenergic drugs aludrin and isoproterenol, and have been termed "sialadenosis". It seems likely that the observed morphological alterations are caused by a stimulation of glandular beta-adrenoceptors via a nicotine-induced release of catecholamines from the adrenals.

[Primary extracranial meningioma of the eyelid].

A case of a primarily extracranial meningioma of the eyelid, so far not reported in literature, is discussed. As mentioned in literature, this case of an extracranial meningioma has also irritated both the physician and the pathologist, due to its uncharacteristic clinical, radiological and histological manifestation. The possibility of an extracranial meningioma as a cause of a head and neck mass should be borne in mind.

[Thrombosis of the internal jugular vein in therapy with hematopoietic growth factors. A case report].

Thrombosis of the internal jugular and subclavian vein is often caused by central venous catheters. We report on a 39-year-old patient suffering from clinically suspected thrombosis of the internal jugular and subclavian veins. Thrombosis was confirmed by B-mode-sonography. The development of thrombosis in this case as a consequence of a central venous catheter and of an additional continuous i.v. treatment with human granulocyte/macrophage colony-stimulating factor (GM-CSF) is discussed.

Inhibition of a cell surface protease after cisplatin chemotherapy.

The epithelial cells in squamous carcinoma and leukoplakia of the oral cavity possess the cell surface protease, guanidinobenzoatase (GB), in an active form. GB is closely similar to plasminogen activator, a protease associated with both transformed cells and tumour cells. The active centre of GB binds the fluorescent probe 9-aminoacridine (9-AA) enabling cells containing active GB to be visualised by fluorescent microscopy. It was observed that chemotherapy with cisplatin resulted in a marked decrease in cell surface GB activity and this decrease was due to the formation of an enzyme-inhibitor complex. One of the results of chemotherapy was shown to be the suppression of a cell surface protease which is known to be associated with migration and malignancy of cells in vivo.

Evidence for the induction of protease activity on cultured tumour cells as a consequence of implantation into nude mice.

We have studied the enzymic status of a tumour associated protease on human colonic tumour cells cultured in vitro and grown in vivo in nude mice. The cultured tumour cells lack this protease. The cells of the tumour colonies in the mice possess active enzyme although the tissue contains an inhibitor capable of inactivating this tumour cell protease. The evidence indicates that the induction of this protease on the tumour cells is a consequence of implantation of the cultured cells into the nude mouse.

Antigenic profile of mammary fibroadenoma and cystosarcoma phyllodes. A study using antibodies to estrogen- and progesterone receptors and to a panel of cell surface molecules.

Using serial frozen sections, monoclonal antibodies and an indirect immunoperoxidase method, 13 fibroadenomas (FA) and 3 cystosarcomas phyllodes (CSP) were analyzed for the expression of Egp34, HEA319-antigen, leucocyte differentiation antigens CD10, CD30, CD57, CD72, CDw75, and CD77, epidermal growth factor receptor (EGFR), estrogen (ER) and progesterone receptor (PR), and transferrin receptor (CD71). Egp34, CDw75, HEA319 antigen, CD10, and CD30 turned out to be consistently expressed in different cell types constituting FA and CSP and revealed that in malignant CSP the myoepithelial compartment acquires the ability to invade the stroma. Phenomenologically, the variable mode of expression of CD57 in myoepithelial cells, of CD77 in ductal epithelium, and of CD72 in both epithelial and stromal cells is suggestive for reflecting differences in their functional state but cannot be further interpreted at present. Expression of PR and ER was restricted to duct cells and was relatively independent, non-systematical. However, expression of ER and EGFR was inverse. This was also true for EGFR and CD71 in both duct cells and myoepithelial cells of FA. In contrast, stromal cells of FA were able to co-express EGFR and CD71 in the absence of PR and ER. This suggests a hormone-independent stimulation of the stromal cell compartment, possibly leading to local proliferation as the primary event in tumorigenesis of FA. In malignant CSP, however, the main proliferating cell is an abnormally mobile, HEA319 antigen-, CD10- and CD30-positive myoepithelial cell found to co-express ERFR and CD71 which is abnormal for this cell type but encountered in (myo-)fibroblasts of FA.

[Use of B-mode sonography in the diagnosis of laryngocele].

A case of a combined laryngocele is presented, including clinical features, the histopathological findings, and the treatment of the disease. The article stresses the value of imaging methods such as CT-scan and high-resolution real-time sonography for preoperative diagnosis. Especially note-worthy is the fact that the internal and external parts of laryngoceles, filled with fluid and with air, can be shown by sonography.

[Lipomatous atrophy of the parotid gland in chronic alcohol consumption].

The effect of chronic ethanol consumption on the morphology of the rat parotid gland was investigated. After feeding an ethanol diet containing 36% of total calories as ethanol for 90 days light microscopic and electron microscopic investigations of the parotid gland tissue showed a striking fat accumulation compared to control rats. Besides interstitial fat tissue dispositions, fat droplets of various size occurred in most of the acinar cells. Additional morphometric investigations revealed a decrease in mean acinar cell volume (-21.9% p greater than 0.001). The latter finding reflects an atrophy of the secretory parenchyma and appears to be responsible for the impaired saliva secretion after chronic ethanol consumption which has been observed in former studies.

[Detection of muscle, vein and artery infiltrations of cervical lymphatic metastases].

In a prospective study, 15 patients with squamous cell carcinomas of head and neck and clinical fixed cervical lymph node metastases were examined to determine the value of computed tomography, magnetic resonance imaging and high resolution real time ultrasonography in preoperative recognition of muscle and vessel infiltration. The preoperative findings were compared with the microscopic results of neck specimens and the operative reports. For each diagnostic method sensitivity, specificity, and effectivity were calculated. Ultrasonography, magnetic resonance imaging and computed tomography can assess muscle infiltrations as well. Metastatic infiltrations of carotid arteries are best recognised by ultrasonography and magnetic resonance imaging. Both methods are more valuable than computed tomography. All methods are unsuccessful in detecting infiltrations of the jugular vein. In conclusion, metastatic infiltrations of surrounding tissue, as described by imaging methods, are rarely confirmed by histological results.

[Mesectodermal leiomyoma of the ciliary body--clinical aspects, surgery and immunohistochemistry].

The clinical, surgical and light microscopical features of a mesectodermal leiomyoma of the ciliary body are presented. Slit-lamp examination of a 63-year old woman strongly suggested a malignant lesion of the ciliary body and the peripheral choroid. Following cryoretinopexy and argonlaser-photocoagulation, the tumor and the adjacent portion of the cornea, angle structures, iris, ciliary body, choroid and sclera were removed en bloc. The defect was covered by a corneosclerotransplant 14 mm in diameter. Histological examination revealed a well circumscribed tumor of the ciliary body, consisting of sheets of fusiform cells. Intracytoplasmatic filaments gave a positive reaction to an actin antibody (Actin HHF 35 MA 931), demonstrating the smooth muscle origin of the ciliary body tumor by immunocytochemical means.

[Realtime sonography].

Between March 1985 and March 1988 109 patients with tumours of the oral cavity and the oropharyngeal region underwent 123 examinations by orthopantomography and realtime sonography for primary staging and/or diagnosis of a recurrent disease. In 105 cases the preoperative results could be compared to intraoperative and histopathological findings. Realtime sonography as a noninvasive, inexpensive and efficient method offered the ideal diagnostic adjunct to the orthopantomogram. Not only size, shape and structure of the soft tissue tumour could be accurately defined; whatsmore it was possible to demonstrate its relation to bony structures and to a high degree of sensitivity cortical invasion and periosteal reaction. The limitations of the method especially the risk of false positive findings after invasive diagnostic and therapeutic procedures are pointed out.

Correlation of DNA flow cytometric results and other prognostic factors in primary breast cancer.

The percentage of cells in S-phase and DNA-ploidy have been measured in 300 primary mammary carcinomas by means of DNA-flow cytometry (FCM). The data were compared with the age and menopausal status of the patients as well as with the size, regional lymph-node involvement, histologic type, grade and concentration of estrogen (ER) and progesterone (PR) receptors of the tumors. A DNA-diploid distribution of the G0/1-peak was found in 37.6% of the cases. The mean percentage of S-phase fractions was 4.83. DNA-aneuploid tumors had significantly higher amounts of S-phase fractions (6.12%) than DNA-diploid tumors (2.66%). There was also a significant correlation between the DNA measurement data (DNA-ploidy and S-phase fractions) and histologic grade, as well as the content of ER and PR, but not between DNA-ploidy, S-phase fractions, tumor size (T) and evidence of axillary lymph-node metastases. DNA-FCM gives a biological characterization of the tumor in addition to the histopathologic examination. The method can be used as a routine procedure because of the reliability and reproducibility of the results as well as the short time needed for the measurements.
