[Clinical and functional characteristics of patients late after pneumonectomy with complicated course of postoperative period].

Results of examination of 65 patients, to whom twenty years before pneumonectomy was performed for pulmonary tuberculosis and chronic nonspecific pulmonary diseases, were analyzed. Peculiarities of postoperative period course were determined in patients with early complications, including the tuberculosis reactivation. Methods of examination: clinical investigation, standard laboratory tests, bacteriologic investigation of phlegm, roentgenography, multiradial roentgenoscopy of thoracic cage, bronchography, computeric tomography, roentgenokymography, spirography, pneumotachometry. Specific and nonspecific changes in lungs, which resided, were revealed accordingly in 20 and 23.3% patients. The tuberculosis reactivation is possible twenty years after the operation. Pneumonectomy performance permits to eliminate an active tuberculosis in 70-80% patients. The tuberculosis changes in residual lung worsen the patients quality of life after pulmonectomy performance due to progress of respiratory insufficiency.

[The evaluation of the external respiratory function in patients in late follow-up period after pneumonectomy].

The external respiration function (ERF) indexes were studied in 215 patients in 25-40 years after performance of pneumonectomy. In 55.3% patients ventilation indexes have exceeded the due values by 50% for two lungs, in 8.4%--significant disorders of the ERF indexes were noted in connection with an early and late complications occurrence and with presence of specific and metaphthisic changes in a single lung as well.

[Substantiation of indication to performance of diagnostic thoracoscopy and thoracotomy in patients with pleuropulmonary diseases using method of oncological testing].

New approach for decision making concerning conduction of surgical treatment of the patients with pleuropulmonary diseases is proposed. This approach especially important in differential diagnosis of dissemination of obscure etiology. He is based on usage of a new screening diagnostic method "Oncotest", which intends setting of photohistochemical reaction in the blood smears, sections and serum for detection of biomolecular markers of cancer. Results of oncotesting of 172 patients with various pleuropulmonary diseases are analyzed. Application of a new approach permits to administer to the patient surgical or endoscopic intervention timely.

[The characteristic of the initial hemostatic status of patients requiring surgical treatment for lung diseases].

Overall 164 patients were examined, who had been assigned to surgical treatment for nonspecific pyo-inflammatory diseases, tuberculosis, and pulmonary neoplasia, with the purpose of assessing the baseline hemostatic abnormalities. The presence of hypercoagulation and of hyperfibrinogenemia was proved to be the case against the background of low activity of fibrinolysis in all patients but the ratio of the above components and the mechanism of origination of abnormalities differed depending on the kind of the underlying disorder. A differentiated approach is recommended towards selecting corrective means for dealing with hemostatic inadequacies in treating the above-named conditions.

[Omentopexy as the operation of choice in treating recurrent postpneumonectomy pleural empyema with a bronchopleural fistula].

In 6 patients with recurrent postpneumonectomy pleural empyema and bronchopleural fistula the transdiaphragmatic omentopexy for the pleural cavity remnant was performed. In 5 patients the complete remission was noted, one patient died, the death's cause was not related directly with the operation performed. The control group of 20 patients had similar pathological findings and to them conventional methods of treatment were applied. In 10 patients the remission was achieved, in 5--the bronchial fistula recurrence was noted, 5 patients died. Thus, the total clinical efficacy was 83.3 and 50% accordingly.

[Pulmonary alveolar microlithiasis].

The literary data and own observation of pulmonary microlithiasis, an extremely rare disease, are presented. The results of investigation of local immunity in patients were depicted.

[Use of non-medicamentous methods of treatment with the aim to improve the immune status of patients with suppurative diseases and malignant neoformations of the lungs].

A test group of 40 patients and a control group of 20 matched controls with pulmonary cancer and suppuration were exposed to preoperative and postoperative electromyostimulation (EMS) with adjuvant intravascular laser radiation of the blood (ILRB) to prevent thromboembolism versus conventional chemotherapy, respectively. At the preoperative examination all the patients exhibited secondary immunodeficiency. It was found that introduction of EMS and ILRB corrects immunity and activates fibrinolysis. A positive trend in coagulation is also attributed to immunomodulating effects of EMS and ILRB.
