[The current possibilities for optimizing the diagnosis of acute cholecystitis].

Based on an analysis of the material of clinical observations of 1523 patients with acute cholecystitis the authors make a conclusion that findings of repeated echosonography with photometry and endoscopic intraduodenal electrothermometry of the duodenal mucosa allow reliable diagnosis of acute cholecystitis, estimation of the character of morphological alterations of the gallbladder wall, determination of the morphofunctional state of the liver. These methods allow to reliably control the course of the inflammatory process in acute cholecystitis and to timely change the curative tactics.

[Duodenal motility disorders in calculous cholecystitis].

In acute calculous cholecystitis, the impairment of motor duodenal function was revealed in 81.6% of the patients, in chronic calculous cholecystitis-in 72.2%. Hypotension in the duodenum is fraught with the development of duodenostasis after cholecystectomy. The reliability of intraoperative duodenal manometry in studying the motor duodenal activity in patients with calculous cholecystitis is substantiated.

[Synthetic speech and its perception in complex acoustic conditions].

Perception of synthetic speech in noise conditions has certain specific features which largely determine noise resistance of synthetic verbal communication. On the one hand, noise resistance of synthetic speech is lower than that of natural speech. This has been demonstrated by the study of the masking effect of 90 and 100 dB noise on natural and synthetic speech. On the other hand, there are methods of speech synthesis that seem very promising. Phonetic analysis of natural and synthetic syllables has shown that noise resistance of synthetic syllables is higher than that of natural syllables but perception of blended speech displays an inverse correlation. This demonstrates the fact that perception of verbal signals under noise conditions is based on the sensory relations that differ from those simulated by synthetic communication.

[The morphofunctional characteristics of the normal duodenojejunal flexure and in pathology of the extrahepatic bile ducts].

Results of complex experimental and clinical (146 patients) investigations of the morphofunctional state of the duodeno-intestinal flexure (DIF) are presented. It was established that patients with the pathology of extrahepatic bile ducts had limited mobility of DIF which was responsible for higher intraduodenal pressure and appearance of duodeno-gastric reflux. Mobilization of DIF improves the evacuation of the intestinal contents from the duodenum in patients with the pathology of extrahepatic bile ducts and is thought to be a prophylactic measure against the development of chronic duodenal obstruction after cholecystectomy.
