[Effectiveness of the use of proteolytic enzymes in extracapsular cataract extraction].

Experimental and clinical study of lekozim and collalysine proteolytic enzymes efficacies in extracapsular cataract extraction was carried out. Local application of these agents in irrigation aspiration of the cortical mass from the capsule sac equator not only simplified the technique of the most intricate stage of surgery, but also proved to be a sufficiently effective means of secondary cataract prevention. A follow-up of up to 3 years has shown secondary cataract development in 5 percent of patients after extracapsular cataract extraction administered lekozim, in 10 percent of those administered collalysine, and in 17.5 percent of cases with mechanical vacuum extraction. Lekozim and collalysine may be used in all the cases when extracapsular cataract extraction is possible.

[The status of the iridocorneal angle after the implantation of anterior chamber intraocular lenses with flexible supporting elements].

The study was undertaken to examine the status of the iridocorneal angle (ICA) 1 to 4 years after implantation of anterior chamber intraocular lens with flexible supporting devices designed by M. L. Dvali. Correct implantation and gonioscopic monitoring wherein were found to provide reliable lens fixation, as a rule, in the area of the scleral spur and, occasionally, to the anterior part of the ciliary body, if the latter was broad. Additional fixation of the supporting devices was attained by delicate commissures at their small extent at the sites of iris contact, which were detected in the majority of cases. Strangulation of the iris or fixation in its periapical part occurred in small cases and were observed in the upper portion of ICA. Corneal epithelial contact with the supporting devices was revealed in 5.4% of cases, as well as in the upper part of ICA. Only in 1 out of 37 cases, the changes in the drainage area such as iris strangulation at a great extent were sufficient to impair its function and required additional surgical correction. The remaining eyes showed no abnormal intraocular fluid circulation. The visual acuity in 35 eyes was as great as 0.5 or more.

[Secondary implantation of the anterior-chamber intraocular lenses with flexible supportive elements for correction of aphakia].

The authors have designed an anterior-chamber intraocular lens (IOL) with flexible supporting elements and examined the possibility of its application for secondary correction of aphakia. Secondary implantations of such IOLs have been performed in 25 eyes. The implantation has proved to be low-traumatic and effective: the vision acuity has been unchanged or improved in 92% of cases, the loss of endothelial cells has made up 8.5% when the Healon bioprotector has been used and 10.3% without it. Criteria have been developed for the selection of patients for secondary implantation of IOL.
