[Predictors of diabetic encephalopathy].

A complex study of neurocognitive traits, depression symptoms, clinical and anamnestic characteristics and metabolic status of patients with diabetes mellitus (DM) with diabetic encephalopathy (DE) of stage I-II has been conducted. Patients with DE had relatively impaired memory and attention as compared to patients without DE. Nonverbal intellect scores were similar in both groups and did not differ from those for "good norm". Development of DE was related to increasing depressive "dissatisfaction", "fatigability", loss of libido and intensification of emotionally-colored complaints about physical health as well as to the relative increase of atherogenic index and glycemia on one of day curve points. An equation of discriminative function that allowed objective assessment of DE diagnosis by clinical and instrumental data was obtained.

[An effect of antioxidant drugs on symptoms of sensomotor polyneuropathy and affective disorders in patients with diabetes mellitus].

An effect of 3-oxypiridine and succinic acid derivates--emoxipine, reamberin and mexidole--on severity of clinical symptoms of distal sensomotor polyneuropathy, anxiety and depression was studied in 120 patients with diabetes mellitus with syndrome of diabetic foot. These medications were used in addition to basic antidiabetic therapy. After 14 days of the treatment an equally pronounced decrease of integral index of "negative" neurological symptoms and reduction of anxiety-depressive disorders were detected.

[Effect of rarely repeated immobilization episodes on hypoxia-tolerance and anxiogenic stress severity].

4 episodes of immobilization stress (60 minutes, once 72 hours) led to diminution of hypoxia tolerance and anxiety-depressive disorders of behavior. The later shifts were associated with decrease of IL-4 concentration and simultaneous increase of IL-6 level in blood serum. The stress-induced behavioral shifts appeared to correlate with IL-4 concentration in blood of stress-sustained rats.

[Influence of anxiogenic stress upon severity of alloxan-induced pancreatic and hepatic damage].

4 episodes of immobilization stress caused anxious and depressive behavioral disorder in rats, accompanied by decreased sensitivity of pancreatic tissue to alloxan-induced damage, and simultaneously increased sensitivity of hepatic tissue to alloxanogenic inflammatory effect. The latter shifts were accompanied by a more pronounced hyperglycemia. The paper discusses a probable role of hepatitis in hyperglycemic response to alloxan challenge associated with stressogenic anxiety.

[Effects of emoxipine, reamberine and mexidol on neuropathic symptoms and a systolic function of the left ventricular myocardium in patients with diabetes mellitus and diabetic foot syndrome].

AIM: To compare efficacy of 3-oxypiridine and succinic acid derivatives (emoxipine, reamberine and mexidol) in distal neuropathy (DN) and left ventricular myocardium systolic function (LVMSF) in diabetic patients with diabetic foot syndrome (DFS). MATERIAL AND METHODS: We studied 120 patients with diabetes mellitus type 1 and 2 (DM1 and DM2). The patients had neuropathic and neuroischemic forms of DFS (stage 0-1 by Wagner, 1990). The patients were randomized into 4 equal groups matched by gender and number of patients with DM1 and DM2. Group 1 received basic therapy plus intravenous placebo (daily 200 ml drops of the polarizing mixture). Group 2 received additional 150 mg emoxipine. Group 3 was given daily intravenous drop infusions of 400 ml reamberin. Group 4 received once daily 300 mg mexidol. The infusion therapy in all the groups lasted for 14 days. Quantitation of neuropathic symptomatic count (NSC), neuropathic dysfunction count (NDC) and echocardiographic examination of LVMSF were made and venous blood samples were obtained before the treatment and after 2 weeks of therapy. RESULTS: Emoxipin, reamberin and mexidol therapy alleviated DM symptoms. In contrast to placebo, the drugs lessened the scores by NSC and NDC. By NSC, the lowest total score was observed after treatment with reamberin. LVMSF changed most significantly in patients on emoxipin, less significantly in those on reamberin and insignificantly on mexidol. The above positive effect did not depend on metabolic compensation of DM. CONCLUSION: The addition of emoxipin, reamberin and mexidol to the scheme of DM patients with DFS relieves symptoms of DM. Moreover, reduction of DM symptoms due to emoxipin and reamberin is associated with better LVMSF.

[Determination of contents of lipid peroxidation products in lipoproteins by using precipitation systems].

The impacts exerted by three different systems of precipitation of apo-B-containing lipoproteins, i.e. phosphotungstic acid-Mg2+ heparin-Mn2+ and heparin-Ca2+, on the lipid peroxidation (LPO) parameters in the fraction of high-density lipoproteins (HDLP) and in the aggregate fraction of low and very low density lipoproteins (LDLP, VLDLP) were comparatively analyzed. The systems of phosphotungstic acid-Mg2+ and of heparin-Ca2+ were found to have the comparable results related with the content of LDLP and VLDLP. While, the use of the precipitation system based on heparin-Mn2+ entails overstated LPO parameters in both lipoprotein fractions. It was demonstrated on the basis of all studied precipitation systems that the aggregate LDLP and VLDLP fraction had the highest content of isopropanol-soluble LPO products. The highest indices for the LPO heptane-soluble products were registered in the HDLP.

[Efficacy of derivatives of 3-hydroxypyridine and succinic acid in diabetic patients with the diabetic foot].

A prospective placebo-controlled study was performed to examine the effects of emoxipine, reamberine, and mexidole on the severity of neuropathic and anxiety-depressive symptom clusters, by simultaneously evaluating the parameters of the lipid peroxidation-antioxidative defense protection (LPO-AOD) system in diabetic patients with the diabetic foot. The 14-day use of all the test drugs was found to cause a significant attenuation of diabetic neuropathy and to abolish concomitant anxiety and depression. The positive clinical effect of emoxipine and reamberine was shown to be associated with the stimulation of AOD and with the limitation of LPO. Mexidole is not inferior to emoxipine and reamberine in its clinical efficacy, but it did not change the parameters of the LPO-AOD system as compared with placebo.

[Fructosamine, high density lipoproteins and degree of dyslipidemia in patients with vascular complications of diabetes mellitus type I].

The contents of products from lipid peroxidation (LP) and from fructosamine oxidation, associated with high-density lipoproteins (HDLP), were examined in the blood of patients with diabetes mellitus (DM) of type 1. It was established that, as the vascular complications of DM, type 1, (i.e. micro- and macroangiopathies) aggravated, the HDLP level, modified by LP heptanophil products, was growing up in the blood serum. The content of the above category of HDLP-associated lipoperoxides depended directly on a quantity of HDLP-bound fructosamine, and it reached a maximum value at the most pronounced dislipidemic disorders accompanied by clinical signs of arteriosclerosis (macroangiopathies).

[Content of lipid peroxidation products, alpha-tocopherol and ceruloplasmin in the blood of patients with vascular complications of insulin-dependent diabetes mellitus].

The content of lipid peroxidation products (LPP), as well as the concentrations of alpha-tocopherol (alpha-TC) and of ceruloplasmin (CP) were examined in the blood serum of patients with insulin-dependent diabetes mellitus (IDDM) without any vascular complications and with diabetic micro- and macroangiopathies. It was established that IDDM is accompanied by an accumulation of LPPs and a reduction of alpha-TC in the blood serum. The development of microangiopathies leads to a relative limitation of LPP, still, the intensity of the process exceeds the normal value. If macroangiopathies are available additionally, this aggravates the deficit of endogenous antioxidants due to an extra reduced concentration of the circulating alpha-TC and a simultaneously decreased CP level.

[Indices of the lipid peroxidation--antioxidant defense system as markers of chronic cardiac failure in cardiomyopathies].

Content of circulating products of lipid peroxidation (LPO), alpha-tocopherol (AT) and ceruloplasmin (CP), was studied in patients with hypertrophic and dilated cardiomyopathies (HTCMP and DCMP). The data obtained correlates with severity of chronic cardiac failure (CCF) and exercise tolerance of the patients. It is shown that the rise in LPO products level and simultaneous lowering of AT concentration in HTCMP and DCMP depend on the functional class of chronic cardiac failure by NYHA (1964). Content of lipoperoxides, AT and CP in the serum reflect the HTCMP patients' tolerance to exercise. Laboratory estimation of the levels of LPO products, AT and CP in patients with cardiomyopathies can substantially objectivize and supplement both clinical and functional-diagnostic assessment of CCF severity.

[Blood level of lipid peroxidation products as an indicator of resistance to infectious inflammatory complications of transurethral electroresection of the prostate].

Study of relationship between the content of lipid peroxidation (LPO) products in the blood of patients with benign prostatic hyperplasia (BPH) and resistance to infectious inflammatory complications (IIC) of transurethral electroresection of the prostate showed that decreased content of circulating lipoperoxides promoted the development of postoperative IIC. Before the operation blood levels of LPO products were increased in BPH patients who did not develop IIC postoperation in comparison with normal controls. Three intravenous injections of ascorbic acid in a single dose of 1000 mg after transurethral electroresection of the prostate led to an increase in the blood level of LPO products and promoted a decrease in the incidence of postoperative IIC.

[Levels of lipid peroxidation products and ceruloplasmin in blood as characteristics of tolerance to physical load in hypertrophic cardiomyopathy].

The effect of standard bicycle ergometry test on the serum concentrations of lipid peroxidation (LPO) products and ceruloplasmin were studied in healthy volunteers and patients with hypertrophic cardiomyopathy. The initial levels of LPO products and ceruloplasmin in the patients were higher than in volunteers. In volunteers bicycle ergometry test did not affect the content of ceruloplasmin in the blood and led to accumulation of circulating LPO products. In patients with hypertrophic cardiomyopathy bicycle ergometry did not lead to increase of lipoperoxide level and promoted an increase in the blood ceruloplasmin concentration.

[Effects of medium-weight molecules isolated from plasma of healthy and burnt animals on the cellular composition of cultured bone marrow erythroblastic islets].

The so-called median-weight molecular fractions (M, B = 0.5-1.5 kDa) exerting a marked hemopoietic effect in the cultured bone marrow erythroblastic islets were isolated from the plasma deproteinates of healthy dogs and animals with acute inflammation (thermal skin burn) by Sephadex G-15 gel chromatography. Acute inflammation is attended by the occurrence of blood medium-weight molecular components that suppress erythropiesis and by the concurrent high rate of granulocytopoiesis in the cultured bone marrow erythroblastic islets. Medium-weight molecular fractions from the blood of healthy dogs substantially were found to suppress erythropoiesis and to stimulate granulocytopoiesis in the cultured erythroblastic islets.

[Morphological analysis of dynamics of myeloid cells in cultured erythroblastic islets].

Morphological analysis of myeloid cells in cultures of erythroblastic islets (EI) was undertaken. All types of myeloid cells (except for myeloblasts) were present in EI preparations isolated from the rats bone marrow. Natural EI include all the cells of myeloid lineage in their "crown". In the course of in vitro cultivation the number of EI including mature granulocytes appeared increased, while the contents of EI associated with promyelocytes, myelocytes and metamyelocytes decreased. The data obtained is discussed in view of probable EI role in bone marrow myelopoiesis.

[Changes in tissue monoamine oxidase activity and resistance to acute hypoxia in rats during stress].

Male Wistar rats exposed to different stresses developed shifts in the brain and liver monoaminoxidase activity. In the so called "cognitive" stimulation, the activity was enhanced in the brain and reduced in liver. Mild stresses also enhanced the activity in the brain. Extreme stimulation (starch peritonitis) caused a significant diminishing of the activity in the brain. All the stress schedules accompanied by enhancement of the brain monoaminoxidase activity increased the rats' tolerance of acute hypoxic hypoxia. Negative correlations between the blood lactic acid contents and the brain monoaminoxidase activity were revealed in rats of both the control and the "cognitive" groups. The findings suggest a direct interrelationship between post-stress shifts of the brain monoaminoxidase activity and the hypoxia tolerance.
