The exact analysis of an anomalous pulmonary venous return in patients with congenital shunt disease can lead to some difficulties when using the standard catheterization techniques. A left-sided anomalous pulmonary vein was suspected in a case with atrial and ventricular septal defects, but could not be evaluated during the catheterization procedure. The pathological vessel could be demonstrated exactly only by venous digital subtraction angiography. Thus, the digital subtraction angiography showed a diagnostic benefit in this complex shunt disease and should be considered in further problematic cases.
In this paper the advantages and disadvantages of different arteriographic methods, i.e. conventional angiography, intraarterial DSA and intravenous DSA, are discussed for the purpose of examining the extracranial supraaortic branches. The analysis of individual vascular regions in arteriography of the aortic arch is assessed. In addiction, the value of i.v. DSA is demonstrated, comparing own results with data in the literature. Indications of the three different angiographic techniques are outlined.

The article reports on a 14-year old girl with periostal new formation of bones at the long bones of the lower arms, the femora and the lower legs the individual phalanges and metacarpalia after colitis ulcerosa which had lasted for several years and had progressed stagewise. After a clinically recorded new attack the periostal new formations of bone progressed. Some time after the last attack of colitis the periostal changes in the bones partially receded. The article discusses the hypothetic explanations aiming at interpreting the pathogenesis of hypertrophic osteoarthropathies and periostoses, as given in the literature.
Periostal new bone formation can be observed as a symptom secondary to numerous diseases. In gastrointestinal disease this hypertrophic osteoarthropathy occurs very rare. The present case report demonstrates an extensive periostal proliferation involving the long bones in a 14 years old girl with longstanding ulcerative colitis.
Cork-screw formation or kinking of the subpelvine ureter, mostly bilateral, are common findings in the routine urogram of the infant without functional effects on the urine flow. Otherwise true ureteral valves with obstructive effect, a congenital malformation in early infancy, are relatively rare. We present two cases with recurrent infection of the urinary tract and clearly visible subpelvine ureteral valves in the urogram. In one patient an obstructive pyeloectasia could not be demonstrated in spite of significant multiple valves.
Case-report of a mongoloid child (Trisomy 21), age 9, operated on in his neonatal period for congenital stenosis of the duodenum; now presenting with acute clinical signs of gastric outlet obstruction and increasing weight loss. Radiologic work-up showed this to be due to a large gastric and duodenal polybezoar, claimed to be due to emotional disturbance as part of this patients primary disease.

The validity of noninvasive (iodine-131 iodohippurate renogram, iodine-131 ortho-iodohippurate clearance, indium-113m EDTA--technetium-99m DTPA sequential renal scan) and invasive (xenon-133 washout) radionuclide screening tests was evaluated in the diagnosis of 105 patients with unilateral renovascular hypertension (RVH) and in 45 patients with essential hypertension (EH). In RVH positive findings on the stenosed side were noted in 73% of renograms, 73% of o-iodohippurate-clearance tests (N = 22), 81% of sequential renal scans, and 90% of xenon-washout studies (N = 67). In a subgroup of 55 retrospectively selected patients with normal or improved blood pressure following renovascular surgery, the preoperative findings had been positive on the stenosed side in 78% of renograms, 75% of o-iodohippurate-clearance tests (n = 20), 85% of sequential renal scans, and 93% of xenon-washout studies (n = 23). The sequential renal scan appears to be a sufficiently reliable method in noninvasive screening for unilateral RVH, although invasive xenon-washout studies show a higher percentage of hemodynamic alterations in the stenosed kidney. o-iodohippurate clearance tests, and in particular xenon-washout studies, can reveal arteriosclerotic lesions in the contralateral, non-stenosed kidney, which may be of importance when the decision for renovascular surgery is pending.
To improve diagnostic procedures and preoperative prognosis in hypertensive patients with angiographically proven renal artery stenosis pre- and postoperative studies of renal hemodynamics (133xenon washout technique) were carried out in patients with normalized, improved and unchanged hypertensive blood pressure values following renovascular surgery. The results demonstrate that renal blood flow in both the operated and non-stenosed kidneys has to be in a normal range expecting a normalization of blood pressure. The existence of a preoperative normal cortical flow rate in the stenosed kidney may be a hint to a normalization of hemodynamic of the involved kidney following reconstructive surgery.
This study was carried out to improve the diagnostic procedure and preoperative prognosis in patients with unilateral renovascular hypertension. The vascular sensitivity of the non-stenosed kidney was tested by the influence on renal hemodynamics (133xenon washout technique) of dihydralazine applicated intrarenally and was correlated with selective renal arteriograms. Comparative studies were done in patients with essential hypertension including those with and without nephrosclerotic lesions. The results demonstrate that the pharmacodynamic effect of the vasodilator used in this study is obtained only in kidneys without arteriosclerotic lesions of the small vessels. A postoperative normalization of blood pressure in patients with unilateral renal artery stenosis depends on a patent renal bypass and a normal vascular response of the contralateral kidney.

1. The effects of oral administration of 4.5 mg of prazosin/day on blood pressure and on plasma renin activity were assessed in patients with essential hypertension and in healthy normotensive volunteer subjects. 2. Mean blood pressure in the hypertensive group fell significantly within 2 weeks of treatment, whereas heart rate was little affected. None of the parameters measured in the normotensive subjects during treatment with prazosin revealed significant changes. 3. Sub-classification of the hypertensive patients into low-, normal- and high-renin categories revealed significant decreases of renin in all six patients with high renin- and in four out of 12 patients with normal renin- but not in the two patients with low renin-essential hypertension. 4. The results indicate the prazosin exerts its hypotensive action and suppresses plasma renin activity particularly in patients with high renin-essential hypertension. These may be considered to have increased sympathetic drive. 5. Conceivably, the blood pressure-lowering effect of prazosin is affected by peripheral vasodilatation and also by a decreased venous return to the heart.

The validity of invasive preoperative diagnostic procedures in patients with unilateral renovascular hypertension was assessed. A significant lowering of blood pressure following reconstructive surgery can be predicted if following stimulation with Dihydralazine the renal-venous renin ratio of the involved to the noninvolved kidney is at least 2.0. Furthermore, it is of importance to determine the renal perfusion rate in the contralateral non-stenotic kidney which should be within normal limits. If these two criteria are met then in the majority of cases surgical intervention in unilateral hypertension is meaningful.
