Impression debridement of corneal lesions.

PURPOSE: The technique of impression cytology using cellulose acetate filter paper as a debriding agent was evaluated. METHODS: Lesions were assessed using a slit-lamp biomicroscope before and after the impressions were taken and appropriate antibiotic therapy was started. The filter paper impressions were stained with hematoxylin-eosin and examined by light microscopy. The 72 corneal ulcers and three cases of suture infiltration around exposed corneoscleral cataract-related sutures were effectively debrided. Five of the six foreign bodies and all of the filaments in the six filamentary keratitis cases were removed with minimal damage to the surrounding cornea. RESULTS: Results of microscopic examination showed that the cells that were transferred to the filter paper were primarily the affected cells. In corneal ulcers, the degenerated epithelial cells, inflammatory cells, and organisms were removed. The inflammatory cells superimposed on affected epithelial cells in suture infiltration and the filaments of filamentary keratitis were removed. In cases of superficial corneal foreign bodies, the foreign bodies overlying a sheet of degenerated epithelial cells were lifted up. CONCLUSION: "Impression debridement" improves the quality and efficiency of debridement for most of these conditions, with much less risk to the surrounding tissue than the conventional techniques. Besides being an adjunct to therapy, it simultaneously provides replicas of the lesions that give accurate histologic details after microscopic evaluation.

Correlation between reaction time and intelligence in psychometrically similar groups in America and India.

The relationship between psychometrically tested reasoning ability, or general intelligence (Raven's Progressive Matrices), short-term memory (forward and backward digit span), and measures of reaction time (RT), including visual and auditory simple RT and four degrees of choice RT, was investigated in groups of unskilled workers, mostly of below average, borderline, or retarded mental ability, selected in the United States and in India. Both groups showed parallel phenomena with respect to the relative difficulty of the various RT tests, their factor structure, and their theoretically expected correlations with psychometric intelligence, although the correlations were lower (and generally nonsignificant) in the Indian group, most likely because of this group's greater restriction in range of ability. The findings, overall, are consistent with other recent studies of RT and intelligence, which indicate that our standard IQ tests reflect basic cognitive processes, particularly speed of information processing, involved in individual differences in intellectual ability, and not merely differences in specific acquired knowledge, skills, or cultural background.

Success of monovision in presbyopes: review of the literature and potential applications to refractive surgery.

The monovision concept of correcting one eye for distance and the other for near may be utilized in presbyopes undergoing refractive surgery. We have performed a systematic review of published literature to evaluate the factors influencing monovision success, and to determine the visual outcome in patients with monovision. Articles in MEDLINE and published bibliographies reporting monovision prescription for correction of presbyopia were systematically identified and reviewed. Pertinent data were abstracted and, when feasible, statistically analyzed. The mean success rate was 73%. The success in monovision correlated with distance correction on dominant eye, alternating dominance, less than 50 seconds of are stereoacuity reduction, and less than 0.6 prism diopter of distance esophoric shifts. Monovision resulted in significant reduction of binocular contrast sensitivity function at spatial frequencies higher than 4 cycles per degree, and 2-6% reduction in task performance, but resulted in minimal reduction of binocular visual acuity, peripheral vision, visual field width and binocular depth of focus. The published literature indicates that monovision is an effective and reasonable therapeutic modality for correcting presbyopia. Proper patient selection and clinical screening are essential for monovision success.

Soluble collagen disks for the treatment of dry eye syndrome.

We evaluated the efficacy of soluble ocular inserts made from porcine scleral collagen for the treatment of dry eye syndrome. The 22 patients enrolled in this pilot study were asked to grade their pretreatment symptoms and to document the frequency of artificial tear administration. Schirmer tests, fluorescein and rose bengal staining, and impression cytology of the conjunctival epithelium were also performed at the initial evaluation. Patients were evaluated in this fashion at 3-4, 6-8, and 18-20 weeks after initiation of treatment. We observed statistically significant improvement in symptomatology and reduced necessity for artificial tear administration. There was no significant change in rose bengal staining or impression cytology results. (The absence of objective findings suggests the possibility of a placebo effect.) These findings indicate that soluble collagen inserts may be useful in treating dry eye syndrome.
