Frequency of locoregional and axillary recurrences among patients following breast cancer operations.

Our department performed 554 operations for breast cancer between 01. 01. 1986 and 31. 12. 1995. When following up these 554 patients, we recognized local recurrences, axillary and neck-region lymph node recurrences in 32 cases (5.8%). Breast conserving procedure was introduced at our department within the period above. What our experience proves is, that the raise in the frequency of breast conserving procedures did not make the rate of locoregional recurrences grow. Together with applying breast conserving procedures we extended the application of axillary block dissections, in terms of indications, and also of numbers of lymph nodes removed. The past five years axillary recurrences did not appear among our patients.

Emergency percutaneous nephrostomy in the septic kidney.

From 250 upper tract obstructive uropathy cases we have studied 64 patients hospitalized with toxico-septic shock. The constant symptom was arterial hypotension. Other 3 patients with long-standing urinary infection due to lithiasis developed this dreaded complication after PNL (staghorn stones-2, pyelic stone-1). In complicated obstructive uropathy cases associated with toxico-septic shock, percutaneous nephrostomy for high urinary derivation in emergency is usually made under local anaesthesia. Its aim is rapid and efficient clearance of kidney obstruction, with minimal damage for the patient; then it is followed by strong antibiotherapy associated with other reanimation and intensive care measures. There were 11 deaths. The stone generating obstructive uropathy was removed subsequently, after the improvement of biological constants and general state of the patient, under the protection of percutaneous nephrostomy.

Original transvesical prostatectomy.

Starting out from a series of theoretical considerations, and checked clinically, the authors describe an original technique for prostatic adenomectomy, being more efficient in limitation of surgical haemorrhage, urinary infection and postoperative hypogastric fistulae. The operative technique and its results in comparison with classic techniques are presented.

Langerhans cells in the normal conjunctiva and peripheral cornea of selected species.

The distribution of Langerhans cells (LCs) in human corneal and conjunctival epithelial sheets was investigated by histochemical, immunofluorescence, and immuno-electron microscopic methods. The LCs stained positive with ATPase and with antibodies to HLA-DR antigen and were negative to DOPA-oxidase. Human conjunctiva showed 250 to 300 LCs/mm2 compared to 15 to 20/mm2 in the peripheral third of the corneal epithelium, approximately similar of LCs were present in Lewis rat, fewer cells in guinea pigs and mice, and no detectable cells in the chick.
