Double-blind placebo controlled clinical trial of almitrine bismesylate in patients with chronic respiratory insufficiency.

The efficacy and safety of almitrine bismesylate, a new respiratory stimulant, in patients with the hypoxaemic form of chronic respiratory insufficiency caused by chronic bronchitis and emphysema has been assessed. The multicentre trial of 12 weeks duration was double-blind and placebo controlled, with individual and group comparisons. Twenty three patients received almitrine 50 mg b.d. p.o. and 17 took placebo. In the almitrine group a significant increase in PaO2 was achieved (control value 54.4 mm Hg, rising to 59.1 mm Hg after 6 weeks, and to 59.4 mm Hg after 12 weeks). There was also a significant decrease in PaCO2 in the almitrine group after 12 weeks. No correlation was found between the plasma almitrine concentration, PaO2 and PaCO2. Lung function (FVC, FEV1, FEV1/FVC, Raw, TLC, RV, FRC) did not change in either group, but the degree of dyspnoea and performance in the 6 min walking test were significantly improved in the almitrine group. Adverse reactions appeared in 6 out of 23 patients on almitrine bismesylate (headache, urticaria, breathlessness, diarrhoea, chest pain, nausea and vomiting), causing drop out of 4 patients. Thus, almitrine bismesylate can be considered useful in the treatment of patients with chronic respiratory insufficiency.

Intensive monitoring of adverse drug reactions in infants and preschool children.

Intensive monitoring of adverse drug reactions (ADR) in infants and preschool children in the paediatric outpatient unit covering the town of Karlovac (150,000 inhabitants) was performed over a period of three months. Data were obtained by physical examination of children and the history given by their parents. In all 2359 children were examined. ADR were recorded in 63 children and were reported to the National ADR monitoring centre in Zagreb. Using the algorithm of Hutchinson et al. (1979), all ADR were classified as "definite", "probable", "possible" and "unlikely". Drugs were prescribed in 97.3% of children, 60.24% received an antimicrobial agent (43% of them on the basis of a sensitivity test), and an antipyretic was given to 1878 children, mostly paracetamol. ADR were most frequently caused by antibiotics (49 reactions to penicillin V, and 15 to amoxycillin) and secretolytics (7 reactions). ADR were followed by complete recovery and not a single child was hospitalized because of an ADR. The results, when compared with the very small number of broadly comparable studies, indicate that the incidence of ADR in this population is rather small and of minor importance.

[253 patients with acute pancreatitis treated at the surgical clinic in Zagreb].

253 patients with acute pancreatitis were treated in clinic for surgery in Zagreb through last 23 years. The most frequent cause of pancreatitis were diseases of biliary tract, obesity, vascular deseases, alcoholism etc. In the symtomatology, the pain was present in all patients and majority of them had abdominal symptoms as well. Most of the patients came to the treatment within the firsts 24 to 48 hours. Besides Trasylol various conservative therapy was applied and some patients were operated either on billiary ducts or on pancreas. 85 patients had to be operated again on billiary tract afterwards. From 253 patients treated 24 died (9,48%) because of the necrosis of pancreas and alterations on various other organs.

Acute Disease

[Carcinoma of the stomach].

During the period from 1961 to 1976 884 patients with cancer of the stomach were registered and treated at surgical Clinic in Zagreb. From those cases 607 (68,66%) were men and 277 (31,34%) women. The age of patients varied from 50 to 70 years. More of them were pensoners and farmers. From those patients 196 (22,17%) were smokers and 176 (19,9%) alcoholics. All of them had usual symptoms of cancers of the stomach, mainly combined with loss in weight, loss of appetite and gastrointestinal bleeding. Average duration of symptoms before coming to the Clinic was six months. Preoperatively 88 (9,9%) were inoperable. By explorative laparotomy 175 (17,5%) inoperable cases were found. At 261 (29,5%) palliative operations were done. 63 (7,1%) patients refused to be operated. Using different methods 297 (35,5%) patients were radical operated. The most frequent were adenocarcinoma and after them gastric ulcers which became malignant. The most frequent causes of death were peritonitis and pneumonia. 7,3% of radically operated patients survived 5 years.

The effect of alternate-day prednisone therapy on cortisol secretion rate in corticosteroid-dependent asthmatics.

The effect of alternate-day prednisone regimen (nine-patients) on cortisol secretion rate was compared with daily therapeutic regimen (ten patients) in corticosteroid-dependent asthmatics. Only the alternate-day regimen did not suppress the adrenal function. The therapeutic efficacy of both regimens was equal. The advantages of alternate-day corticosteroid therapy are presented and the optimal way of starting this therapy discussed.

Cortisol secretion rate during beclomethasone dipropionate aerosol therapy in bronchial asthma.

Beclomethasone dipropionate is a new synthetic corticosteroid for the inhalatory treatment of bronchial asthma. The effect of beclomethasone dipropionate aerosol (400 mug/day/j weeks) on the cortisol secretion rate in comparison with prednisone per os (10 mg/day/4 weeks) was investigated in 12 patients with allergic bronchial asthma. There was no suppression of the adrenal cortex in the course of beclomethasone dipropionate inhalation, while prednisone significantly suppressed adrenal cortex function.
