A cross-sectional study was performed in which physical examinations of the neck and upper limbs were conducted on 82 currently working female industrial workers with exposure to repetitive work tasks and on 64 currently working referent subjects without exposure to repetitive work tasks. Associations between results of symptom questions and physical examination were sought with variables related to the work environment and to the individuals. In a multivariate model, there were statistically significant associations between exposure to repetitive work and diagnoses in both the neck/shoulders (prevalence odds ratio, POR = 4.6) and elbows/hands (POR = 3.5). In addition, age (POR = 1.9, 75th vs. 25th percentiles), tendencies towards subjective muscular tension (POR = 2.3), and stress/worry (POR = 1.9) were also associated with diagnoses in the neck/shoulders; however, there was not an association between these variables and the prevalence of diagnoses in elbows/hands. Standardized evaluation of videotape recordings in 74 of the industrial workers revealed significant associations between neck flexion, and elevation and abduction of the arm and the prevalence of neck/shoulder diagnoses. In the multivariate model, neck flexion was significantly associated with diagnoses in the neck/shoulders (p = 0.005). In addition, low muscle strength, lack of emotional well-being at work, and a variety of psychosomatic symptoms were associated with diagnoses in the neck/shoulders (all p < 0.001). Lack of strength was also associated with disorders of elbows/hands (p = 0.007). This study demonstrated a substantial prevalence of neck and upper limb disorders associated with repetitive work performed with a flexed neck and elevated and abducted arms, as well as a possible potentiation of these ergonomic factors by certain personal traits in some workers.
The effects of lead and mercury on [3H]-histamine uptake by cultured astroglial and endothelial cells of rat brain were studied. Experimental data showed that both metal ions inhibited the uptake in both cell types of concentrations as low as 1-10 microM. The effects were consistent with non/competitive inhibitions. With either lead or mercury exposure, the inhibition of the uptake was greater in astroglial than in cerebral endothelial cells. Contrary to the above findings, 100 microM of mercuric chloride produced stimulation of histamine uptake and this stimulation was much more pronounced in cultured cerebral endothelial cells than in astroglial cells. Inhibition of [3H]-histamine uptake by lead acetate and mercuric chloride was considered to be association with a loss of the transmembrane Na+ and/or K+ gradient while stimulation of the uptake by high concentration of mercury might be related to a direct effect on histamine transporter. It is noteworthy, that cultured astroglial cells, derived from neonatal rat brain, are much more sensitive to the toxic effects of these heavy metal ions than cultured endothelial cells derived from the brain capillaries of the same species of animals.

In a 47-year-old female patient distal renal tubular acidosis (dRTA) led to the development of osteomalacia following 13 years of the first episode of hypokalemic respiratory paralysis and 7 years of KHCO3 treatment. In spite of the musculoskeletal disability, intense bone pain and myopathy, the values of serum calcium (Ca++) and phosphorous (P) showed minimal deviation from the normal levels. The bone scintigraphy was the first indicator of the bone disease. As the disease progressed the serum level of alkaline phosphatase increased gradually and 25-hydroxyvitamin-D level decreased and bone scintigraphy showed multiple areas of increased radioisotope uptake. By that time the patient's condition deteriorated severely, she became almost unable to walk. Rocaltrol (calcitriol) therapy led to dramatic clinical improvement and the complete resolution of the laboratory values. When the alkaline therapy of dRTA does not prevent the development of osteomalacia, administration of a modern vitamin-D preparation can result complete healing of the bone disease.

OBJECTIVE: The aim was to study the association between personal factors and physical and psychosocial work environment factors and disorders of the neck or upper limbs among women in the fish processing industry. METHODS: A cross sectional study was performed on 206 women in the fish processing industry and 208 control women. Several physical and psychosocial work environment factors were evaluated. Subjective complaints about the neck or upper limbs were assessed by questionnaire and by a clinical examination. RESULTS: The study showed a high prevalence (35%) of diagnoses in the neck or shoulders of the exposed women. All prevalence odds ratios (POR's) were substantially higher in young women. There was a pronounced dose-response relation between disorders of the neck or shoulders and duration of employment for women < 45 years old. When studying 322 former workers, the proportion who claimed musculoskeletal complaints as the reason for leaving was highest among the older women. Muscular tension, stress or worry, work strain, and the largest fraction of the work time spent with highly repetitive work tasks were clearly associated with disorders of the neck or shoulders. The measurements of the wrist movements also showed that the work was performed almost without any pauses and that the median flexion and extension velocity was high (41 degree/s). The results of observation showed good agreement with the measurements of wrist motion. CONCLUSION: Work in the fish processing industry is a risk factor for disorders of the neck and upper limbs. Due to the homogenity of the physical work load in the exposed group, we could not show any associations between the objective measurements and disorders. In cross sectional studies the risk may be underestimated due to a healthy worker effect.
It often occurs during ultrasonography, that the gallbladder is not visible. The cause of this is most frequently cholelithiasis, but one must know the less common causes as well. Authors review the characteristic reasons and give accounts of their own experience, which was gained in the course of more than 60,000 examinations.
The author commemorating professor Géza Hetényi, discusses the relation of endoscopy and x-ray diagnostic. He observes that the spreading of recent fibre optical endoscopes did not solve the problem of the early gastric cancer diagnosis. The experiences of the author based on his practice for several decades, data from the literature and, especially, the Japanese results show that x-ray did not lose its position but rather strengthened it in the diagnostics of the upper gastrointestinal tract. For examinations at identical levels, both diagnostic methods have equal opportunities to reveal gastric cancer at an early stage. In accordance with the type variety of early gastric cancer, the author discusses the relationship between early gastric cancer and gastric ulcer, and he proposes a rational algorithm for its diagnosis and follow-up. He shows that the most effective method for the diagnosis of the upper gastrointestinal tract is a reasonable combination of all available methods, and "primus inter pares" of them is x-ray examination, in addition to endoscopy.
The authors studied the effectiveness of the cervical cerclage operation in the therapy of cervical insufficiency during the pregnancy. Two Hundred sixteen pregnant women took part in this trial in whose pregnancy the necessity of cerclage operation was not unequivocal. It means, that the therapeutical cerclage operation was not necessary, but the necessity of prophylactic cerclage operation was doubtful as well. From the randomized 216 cases they performed cerclage operation in 108 pregnancies and in 108 cases they did not perform it. In the respect of pregnancy care they did not make any difference between the two groups. The authors from their trial draw the conclusion, that in doubtful cases where the necessity of cervical cerclage operation is not unequivocal--e.g. at serious gestational case history--it is advisable to perform the operation. The danger of premature birth can be diminished in this way.
The transabdominal ultrasonography has less importance in the diagnosis of colonic tumors. To increase the diagnostic efficiency, the authors examined the colon with ultrasound - during water enema. They found 5 tumors and 4 polyps from 85 patients. They found, that this method is suitable for the intraluminal tumors' diagnosis. They want to investigate the other changes of the intestinal wall in the future.
EMG was recorded with surface electrodes from the trapezius and deltoid muscles during a static endurance test at approximately 20% of maximal voluntary contraction. Objective parameters for localized muscular fatigue were derived from the time course of the root mean square (RMS) and mean power frequency (MPF) of the EMG recordings. Isotonic regression is introduced as a tool for assessment of such parameters. The most pronounced sign of fatigue for trapezius was an increase in the RMS values, while for deltoid it was a decrease in the MPF values. This could be explained by the different functions of the two muscles. The endurance time for a group of 11 women in industrial work with repetitive short-cycled work tasks who were diagnosed with neck/shoulder disorders (tension neck) was significantly shorter (p less than 0.05) than for a group with the same work, but without neck/shoulder disorders (n = 11), and shorter than for a control group (n = 11). Regarding the EMG fatigue measures, there were no significant differences between the three groups. We did not find any relationships between endurance time and the EMG parameters. The results indicate that neck/shoulder disorders were not associated with divergent mechanisms for developing fatigue in the muscles, as recorded with surface EMG.

Authors--beside the admission of ultrasound examination's obvious priority--do not think that the complete cease of the gall-bladder's X-ray examination is reasonable. They have been found this on the basis of the literature and their own observations. Authors suggest the gall-bladder's examination after a negative ultrasound, if the complaints of the patient are unchanged.
A series of 137 patients suffered in clinically documented angina pectoris were analyzed by 12-lead exercise ECG, exercise body surface potential mapping and exercise thallium scintigraphic methods and the results were compared to that of selective coronary angiography and left ventriculography. If coronary artery stenosis were considered to be significant in the presence of more than 70% vessel narrowing, the sensitivity figures were 76, 93 and 88% for exercise 12-lead ECG, exercise body surface potential mapping and exercise thallium scintigraphy, respectively. In considering 50% coronary artery narrowing to be significant, the same figures were 78, 94 and 89%. Specificity figures at the same order were 59, 65 and 80% for more than 70%, and 64, 70 and 88% for more than 50% coronary obstructions. Exercise body surface potential mapping and exercise thallium scintigraphy applied parallelly gave a sensitivity of 100% and specificity of 53%. False-negative and false-positive exercise body surface potential mapping and thallium scintigraphic tests were analysed taking into consideration left ventricular function indices and respective patients. The authors suggest that the outstanding high sensitivity of the above mentioned two tests applied parallelly reveals that they highlights partially different aspects of coronary artery disease, and that is why the overlapping between the methods is relatively small. The majority of false-positive tests characterize a pathological state, and in these cases the exact diagnosis should be cleared up by other noninvasive/invasive methods.
Nickel deficiency was induced in 2- to 4-year-old goats by feeding 0.1 mg Ni/kg dry matter with a semisynthetic diet. The control group consumed 5.0 mg Ni/kg d.m. Activity of several enzymes (SDH, LDH, HBDH, AST, ALT, ALD, CK, CHE) was determined in the serum, liver, heart and kidneys. Serum urea-N level was also measured and transmission electron microscopic (TEM) examinations were performed. Signs characteristic of nickel deficiency (retarded growth, increased mortality of dam and offspring, parakeratosis of the skin) appeared in the low-nickel group. The activity of SDH and ALD, as well as the level of urea-N was significantly lower in the serum of Ni-deficient animals than in the control. Ni-deficient animals also had significantly lower enzyme activities in the heart (SDH, HBDH, AST, ALT, ALD and CK), liver (SDH and CHE) and kidneys (HBDH and CK). Electron micrographs showed degeneration of cardiac and skeletal muscle in the Ni-deficient animals. Ni deficiency elicited changes primarily in the heart and these resulted in depressed activity of several enzymes.
Authors introduced a nickel-containing device into the uterus of 20 rabbits, then performed the cytochemical study of the endometrium. During calcium cytochemical study in the endometrium intramitochondrial and partly intracytoplasmic localisation while with dimethylglyoxin method intracellular reaction precipitate were found. The nickel-containing device caused local nickel and intracellular or intramitochondrial calcium accumulation.
