[Low dose clofibrate in the treatment of hypertriglyceridemia in hemodialysis patients (author's transl)].

Sixteen patients on regular hemodialysis were treated with clofibrate, 500 mg three times a week for six weeks. A sustained and significant reduction of serum triglycerides occurred in thirteen patients. Serum cholesterol was also significantly reduced, although to a lesser degree. Predialysis levels of blood urea nitrogen, creatinine, and uric acid were significantly elevated during the treatment period and returned to previous levels after withdrawal of the drug. The incidence of side-effects was elevated. Five patients complained of nausea and upper abdominal discomfort, while four patients had muscle pain. Several muscle enzymes were elevated in a high percentage of patients, returning to basal values after discontinuation of the drug. It is concluded that clofibrate, although it is effective in lowering serum triglycerides in patients on hemodialysis, should not be used in such patients.

Stable derivatives for the gas chromatographic determination of synthetic anabolic stilbene residues (diethylstilbestrol, dienestrol and hexestrol) in meat and organs of treated cattle in the sub-parts per billion (10(9)) level.

A method for the determination of diethylstilbestrol and the related compounds dienestrol and hexestrol residues in meat and organs of treated cattle is described. After extraction and clean-up, these synthetic estrogens are subjected to reaction with pentafluorobenzoyl chloride, which gives very stable perfluoro esters that are suitable for gas chromatographic determination using an electron-capture detector. With the careful clean-up and the very sensitive response of these derivatives, it is possible to reach a limit of detection in the sub-parts per billion (10(9)) range starting with only 5 g of sample.

[The value of high dose steroid therapy in obstructive uropathy caused by a tumor].

The case of an 82-years old female patient with acute renal failure secondary to tumoral obstructive uropathy by neoplasic invasion of the trigone is described. The condition was treated with urinary deviation through percutaneous nephrostomy of the left kidney which achieved an improvement in the renal function. Later, the percutaneous nephrostomy was unintentionally moved not being possible to place a new one in none of the kidneys. The patient remained anuric for 24 hours, and therapy was then instaured with high doses of intravenous steroids (6 Metyl-Prednisolone 1.5 g I.V. in 24 hours), diuresis was recovered and renal function became normalized within a few days. The mechanism of action and therapeutic usefulness of high dosage steroids in tumoral obstructive pathology is discussed.
