Thallium-201 uptake in myositis ossificans. Potential pitfall in diagnosis.

Myositis ossificans is primarily a disorder of adolescents or adults, whereby an area of muscle mass undergoes progressive ossification. Its radiographic and scintigraphic appearances have been well documented. In this paper, a case of histopathologically demonstrated myositis ossificans in the proximal thigh with unexpected TI-201 accumulation was presented.

Reflex sympathetic dystrophy syndrome secondary to organophosphate intoxication induced neuropathy.

Reflex sympathetic dystrophy or Sudeck's atrophy is a syndrome that can usually be followed by a traumatic insult. This disorder is accompanied by signs and symptoms of vasomotor instability, trophic skin changes, and rapid development of bony demineralization. This report presents a case with reflex sympathetic dystrophy syndrome secondary to organophosphate intoxication induced neuropathy. The patient was threated with calcitonin well.

The value of Tc-99m Nanocolloid scintigraphy in the evaluation of infected total hip arthroplasties.

The early diagnosis of loosening is very important, since it provides a chance to protect the bone structure by means of a good postrevision outcome. Although the delayed stage of infected loosenings can easily be detected by clinical presentations, significant laboratory data and plain X-rays, diagnosis becomes a problem in the early stage. In this study the value of Tc-99m Nanocolloid (NCol) scintigraphy in the diagnosis of infected loosenings was evaluated in 28 patients with painful total hip arthroplasty and 10 controls without any complaint after total hip arthroplasty, by comparing this method with laboratory data, plain X-rays and 3-phase Tc-99m methylene diphosphonate (MDP) scintigraphy. Tc-99m NCol scintigraphy was found out to be a very valuable method with 100% sensitivity, 84% specificity in the diagnosis of infected prosthesis and it was superior to laboratory data, plain X-rays and 3-phase Tc-99m MDP scintigraphy, but requires to be evaluated in conjunction with plain X-rays for more information and in order to prevent false positive results.

Ipsilateral and contralateral testicular blood flows during unilateral testicular torsion by 133Xe clearance technique.

Testicular blood flows during unilateral testicular torsion were measured by Xenon-133 clearance technique. Xenon-133 was injected via the left carotid artery into the heart. Measurements were performed in control and left-sided testicular torsion groups, each consisting of ten rats. Left and right testicular blood flows, which were 29.157 +/- 2.272 ml/100 g/min and 29.773 +/- 2.934 ml/100 g/min in the controls, were 11.983 +/- 3.655 ml/100 g/min and 16.098 +/- 3.757 ml/100 g/min in the experimental group and differed significantly. The decrease in contralateral testicular blood flow may play an important role in the contralateral testicular damage encountered following unilateral testicular torsion.

A simple and efficient method of labelling RBCs with 99mTc for spleen imaging.

A new method of labelling human RBCs with 99mTc by the use of Sn-alpha-D-glucose 1-phosphate (GP) is presented. It was tested for spleen imaging in 16 normal volunteers. The labelling was carried out during the heating (30 min at 49.5 degrees C) to damage the cells and cooling (10 min at R.T.) steps. The labelling efficiency was 95.5 +/- 1.5% with Sn-GP and was better than Sn-PYP (61.5 +/- 25.0%). The radioactivity retention of RBCs was greater than 96% after 6 washings. The spleen was delineated very well in all the subjects. The spleen activity reached a plateau at 20 min post-injection. Spleen-to-liver and spleen-to-cardiac blood pool concentration ratios were high (10.4 +/- 2.2 and 10.1 +/- 3.2, respectively) calculated at 30 min. The method is simple, rapid and efficient.

Functional asplenia and portal hypertension in a patient with primary splenic hemangiosarcoma.

A 60-year-old man with primary splenic hemangiosarcoma (PSH) presented with weakness, weight loss, abdominal pain, and anemia. Physical examination revealed hepatomegaly, ascites, and firm, huge splenomegaly. Ultrasonography showed many nodular structures characterized by hypoechogenic and hyperechogenic areas. The patient also had portal hypertension, which was confirmed by physical findings and by measurement of portal vein pressure during operation. A liver-spleen scan using Tc-99m sulfur colloid and Tc-99m labeled heat denatured erythrocytes failed to demonstrate any splenic uptake, a reliable feature of functional asplenia. Although on a total body scan with Ga-67 citrate there was no splenic uptake, there was gallium uptake in the liver, where the presence of the metastatic lesion was histopathologically verified and confirmed by operation. There was also uptake in the middle zones of the lungs. Ga-67 citrate imaging appears to be helpful in the diagnosis of metastasis of PSH, and PSH can rarely cause portal hypertension.

An improved method for the in vivo labelling of red blood cells with 99mTc in gated cardiac imaging.

Red blood cells of 10 patients were labelled with 99mTc by the in vivo method, 5 of the patients were orally administered with 400 mg potassium perchlorate at the time of stannous pyrophosphate injection and 5 ml blood was withdrawn at 10 min postinjection of pertechnetate to determine the labelling efficiency of RBC's. The RBC and plasma volumes were also determined in all patients by the 51Cr method. Our results indicated higher incorporation of injected radioactivity into RBC's in the perchlorate administered group (83.8% +/- 9.2%) compared to the other (58.5% +/- 22.0%) (P less than 0.01). The thyroid and stomach were visualized in the control group, but not in the perchlorate administered group. In routine application, 50 MUGA studies have been done with perchlorate administration with excellent results. In 60 patients without perchlorate administration previous to the present study the EF could not be determined in 6 patients (10%). Our results indicated that potassium perchlorate may be used routinely for in vivo labelling of RBC's to improve the labelling efficiency and the quality of gated cardiac imaging.

Lymphoscintigraphic evaluation of primary congenital lymphedema.

A-four-year-old girl with primary nonfamilial congenital lymphedema was evaluated by lymphoscintigraphy using 99mTc-Dextran. It was demonstrated that there was no accumulation of radioactivity in the left inguinal and femoral lymph nodes and faint accumulation in the contralateral lymph node. It was concluded that lymphoscintigraphy is a safe, reliable, noninvasive technique which is helpful in the evaluation of a patient with lymphedema, especially small children in whom conventional contrast lymphangiography offers some challenges.

Diuretic renography in children: a non-invasive method for the assessment of upper urinary tract pathologies.

Thirty-eight kidneys from 20 children were studied with diuretic renography and the findings in 32 kidneys were compared with the results obtained using IVP. The diuretic renography findings were consistent with those of the IVP in 72 percent of the patients. The main disparity was the dilated nonobstructed pattern observed in the kidneys with normal IVP's, which is pointed out as being an asset of diuretic renography in revealing the functional status of kidneys. The sensitivity in detecting true obstruction was found to be 67 percent (4/6), and the specificity 95 percent (20/21). The accuracy of the technique was 85 percent with a false (+) rate of 5 percent (1/21) and false (-) rate of 33 percent (2/6). Diuretic renography is a simple, safe, non-invasive technique, easily applied to children with high specificity in excluding obstruction in a dilated renal pelvis.
